Thyroid HormonesProduction of Thyroid Hormones

The Thyroid gland produces its two main hormones by using the amino acid tyrosine and the mineral iodine. First of all, tyrosine is synthesized in the follicular cells of thyroid and is attached to a protein called Thyroglobulin. Then, iodine is trapped in the thyroid gland and is attached to tyrosine forming iodinated tyrosine. When thyroid is stimulated to secrete hormones, certain enzymes break down iodinated tyrosine from Thyroglobulin to form T4, and from T4, T3 is produced. T4 has four iodinated tyrosine molecules while T3 has three. About 80% of thyroid hormones are T4 while 20% is T3.

Common Causes of HypothyroidismHypothyroidism has many causes. It is significant that in order to stay away from hypothyroidism, you should have in mind all the factors that may lead to this disease. Following is an account of the various causes of hypothyroidism:

Insufficient Iodine in Diet

The most common cause is dietary insufficiency of iodine. Iodine is the most necessary component of thyroid hormones and the major source of iodine is your diet. If you are not eating enough iodine in your food, thyroid hormones cannot be produced and you will suffer from hypothyroidism. This type of hypothyroidism results in goiter or swelling of the thyroid gland as incomplete form of thyroid hormone is stored in the thyroid gland and the size of the gland increases and appears as a lump in front of the neck.

Early Stage SymptomsSymptoms in the early stage of hypothyroidism include the following:

Cold intolerance – Due to marked or increased sensitivity to cold, patients cannot tolerate a very cold weather. Patients opt to stay in the warmer areas around the house and use more layers of clothing than usual to fight the cold.Decrease in activity – Patients easily gets tired with simple activities. One can observe that they easily get fatigued and lack the energy. Because of this, patients take comfort in doing little or nothing.Cold limbs – Even when it is summer season, patients have clammy, cool skin and their body temperature is definitely lower than normal. Patients may complain of feeling cold all the time. This at times may be the first symptom and may last for years before diagnosis.Constipation – A very common symptom of hypothyroidism, it becomes more severe as the condition progresses.Heavy menstrual periods in females.

CHAPTER 4: Diagnosis Of HypothyroidismTesting for Hypothyroidism

The most basic test for testing hypothyroidism is to test the blood levels of thyroid hormones namely; free and total T3 and T4. Other tests include TSH, TRH, and antibody tests.

If your doctor wants to confirm whether you have hypothyroidism or not he would most likely order T3, T4 and TSH levels. The total T3 and T4 levels tell the amount of thyroid hormones present in your blood. Some of these hormones are free while most of it is bound to proteins in the blood. If you are suffering from hypothyroidism your total T3 and T4 levels will be below the lower limit.

Treatment Using T4 (Thyroxine) Supplementation

This is the most common mode of treatment practiced by majority of the doctors. T4 is the non active form of thyroid hormone and it is believed that the body cells can convert it to T3, the active thyroid hormone. Various prescriptions medicines such as Synthroid, Levoxyl, Levothyroxine etc. are used to supplement T4. However, the conversion of T4 to T3 may happen in some patients, but most of the cases do not get any help from this. This method of treatment is only replacing the hormones not produced in the body and not curing the actual problem of the underactive thyroid gland. In the long run the unused T4 may even slow the function of thyroid gland, aggravating the hypothyroidism.
