Medtronic supports the Regulatory Agency requirements (AABB, FDA, CAP, CLIA and JCAHO) of Manufacturer recommendations in respect to Annual PM/Calibration schedules. Our goal is customer satisfaction with the level and quality of service and products — therefore it is critical the medical devices and accessories be maintained with the utmost care. Service Contract coverage would begin after the expiration of the first year manufacturer warranty.

There are many benefits to using Manufacturer Provided Service. Your service technician is part of a nationwide network of field service technicians with decades of experience maintaining Medtronic devices. They receive annual training on all of Medtronic devices, upgrades, and PM procedure updates. They are 100% committed to maintaining your Medtronic equipment at the highest level possible.

U.S. Policy on Purchase OrdersAll orders must be for full case quantities only. Minimum order is one case.

Payment shall be net 30 Days from date of invoice. Modification to credit terms may be made only in writing and signed by an authorized representative of Medtronic.

If Medtronic takes any adverse action with respect to the extension of credit, applicant has a right to request a statement of specific reasons within 60 days of Medtronic’s notification to applicant of the adverse action taken.

Prices are subject to change without notice. No terms or conditions contained in any document created or sent by applicant will be effective unless specifically accepted in writing by Medtronic. All modifications to the terms and conditions contained in this document must be in writing and signed by Medtronic. No course of dealing or usage of trade or actual course of performance shall be relevant to explain or supplement any terms used herein.

Shipping TermsAll products will be sold and delivered freight FOB shipping point, where Medtronic will prepay freight, which will be added as a separate item to the invoice.

CancellationCancellation charges for special order (made to order) will include all cost incurred for all work completed and/or in progress.

WarrantyThe written product warranty enclosed with each product when delivered to the Customer sets forth the entire warranty applicable to each product.

Extended Service AgreementsMedtronic Cardiac Surgery Service offers customers an economical, efficient program of reliability and service. Our Extended Service Agreement will ensure continued optimum performance to all U.S. users of Cardiac Surgery Instruments. This agreement provides for service at your location or at the Medtronic Cardiac Surgery Service Depot.

Returned Products1. Authorization must be obtained from an authorized Medtronic representative before the return of any product.

2. A Returned Goods Authorization (RGA ) number will be given to you and should be clearly identified on the carton of any returned product. a. Information for an RGA number includes the product catalog number and quantity of product to be returned, the reason for the

return, the customer P.O. number, the Medtronic invoice number, and the date of invoice. b. RGA number will be valid for 30 days from date of issuance. If product is not returned within 30 days, RGA will be canceled.

3. Product returned may be subject to a minimum 20% restocking fee.

4. Medtronic reserves the right to refuse credit and return of product.
