STEWARDSHIP
Peninsula Agency on Aging continues to operate in a position of strength and stability, with 92% of expenses being program related.
CEO2014 was a landmark year as the Peninsula Agency on Aging (PAA) celebrated its 40th year of service to seniors and family caregivers on the Virginia Peninsula, but there was no time to rest on our laurels! During 2015 we worked to develop new collaborations and programs as we continued to expand new initiatives like the Community-based Care Transitions Program (CCTP).
As part of the Eastern Virginia Care Transitions Partnership (EVCTP), PAA was recognized by CMS as one of the top performing Community-based Care Transitions Programs (CCTP) in the nation. The success of this initiative has opened doors of opportunity with new partners like Managed Care Organizations (MCOs). PAA continues to partner with EVCTP to provide long term supports and services that enable health care providers to maintain successful clinical outcomes when patients return to their homes and communities. It is well documented that social determinants are a key element for improving population health and PAA, along with EVCTP, are helping healthcare providers bridge the gap between hospital and home.
One service that helps fill transportation gaps is the paratransit PAA/RIDES Program, serving the Greater Williamsburg area. The RIDES program continued to perform well during 2015 and has played a leadership role in developing a one call system for paratransit in the Williamsburg area. The One Call initiative includes PAA/RIDES, Williamsburg Area Transit Authority (WATA) and Williamsburg Faith in Action (FIA) among others.
Another innovative program, the Advance Care Planning Coalition of Eastern Virginia (ACPCEVA), continued its efforts to educate health care professionals and lay-persons about the importance of being in charge of one’s health care by having an advance directive included in electronic health records. This area agency on aging, health system partnership is a model worthy of replication across Virginia and the nation.
PAA’s Aging and Disability Resource Center (ADRC) based in Greater Williamsburg strengthened its presence in the community during 2015. In addition to linking individuals with services, the ADRC has helped increase awareness of PAA as a resource for seniors and family caregivers in Williamsburg.
With guidance and support from the Board of Directors, PAA has been able to achieve much in 2015. We are well positioned for 2016 and anticipate continued innovation and growth as we provide services and support to the communities we serve. With your continued support we can achieve great things and continue to transform challenges into opportunities that ensure senior will be able to age in a place of their choosing.
