This is open to all dental chapter/affiliate society, but, is entitled for one entry only and one vehicle only. No entry fee is required. 2. Specific materials (i.e., product packaging, labels, and containers) will be required in coming up with the float entry upon finalization of corporate sponsorship. The float entry should represent the cultural highlights and/or famous festivity celebrated in your local community. 3. No specific requirement regarding the vertical and horizontal dimensions of the float to be used is to be implemented, granted that the stability of the float and the strength and integrity of the materials used will not compromise the safety of the event. 4. Official Entry Form should be completely filled out and should be passed starting December 2014 until January 15, 2015. 5. A waiver was also provided, thus, shall be passed, too, along with the official entry form. 6. The float entry should be at the designated assembly area at Cuneta Astrodome not later than 5:00 AM but can come as early as 1:00 AM. The order of entry appearing on the parade will be based on the chapter registration for Alay-Parada. 7. Movement of parade will start at exactly 6:00AM at the north-bound lane, outer lane of Roxas Blvd., in front of Cuneta Astrodome. 8. Judges, identified only by the committee during the movement of the parade, will be positioned in strategic areas according to the route of the Alay-Parada followed by the float parade. 9. Entries will be judged based on the following criteria: CRITERIA PERCENTAGE WEIGHT Depiction of the Theme 35% Creativity 25% Concept and Presentation 25% Over-all Impact 15% Total 100% 10. Prizes and special awards are as follows: PLACE PRIZE GRAND CHAMPION PHP 50,000.00 + TROPHY FIRST RUNNER UP PHP 30,000.00 + TROPHY SECOND RUNNER UP PHP 20,000.00 + TROPHY 11. Twenty consolation prizes amounting to P5, 000.00 each for non- winning entries is allotted. 12. The board of judges’ decision is final and irrevocable.

Dentistry at its Best Festive Float Parade

GENERAL GUIDELINES:

1. This is open to all dental chapter/affiliate society, but, is entitled for one entry only and

one vehicle only. No entry fee is required.

2. Specific materials (i.e., product packaging, labels, and containers) will be required in

coming up with the float entry upon finalization of corporate sponsorship. The float

entry should represent the cultural highlights and/or famous festivity celebrated in your

local community.

11. Twenty consolation prizes amounting to P5, 000.00 each for non- winning entries is

allotted.

12. The board of judges’ decision is final and irrevocable.

REGISTRATION CONFIRMATION OF PARTICIPATION

1. Standardized entry form will be drafted and passed to all dental chapters and affiliate

societies. Please kindly find necessary attachment.

2. Complete guidelines will be distributed.

3. Deadline for submission of form will be on December 2014 until January 15, 2015.

4. Contact person to facilitate registration, passing and receiving of forms will be assigned.

5. Final count of entries must be reported.

6. Upon passing of registration form and confirmation of entry, final advisories must be

forwarded.

FLOAT REQUIREMENTS

Upon reaching the final list of corporate sponsorships, materials to be used, as to the

packaging, labels and containers of the sponsor’s products will be used in coming up

with the design of the float entry.

Strong reminder for the stability of the output and integrity of strength of the materials to

be used.

Theme and basic concept – showcasing the cultural highlights and/or best festivity each

entry is representing as to the province or city they belong.

WAIVER (STRONGLY NEEDED)

Agreement that waives PDA and its organizing committee in case of unwanted

incidences within the duration of the contest. Please kindly find necessary attachment.

FORWARDED SHORT DESCRIPTION OF ENTRY

In one hundred words, the chapter should pass a brief yet comprehensive description of

the festivity their float displays.

Judge selection and criteria for judging

Road management for event proper

SPECIAL PROVISIONS

Working area designation in assembling the float entry is to be assigned, but, definitely

within or along the venue.

MATRIX OF IMPLEMENTATION

Registration forms and guidelines will be distributed to all the presidents of dental

chapter and affiliates societies.

Deadline for submission of entries will be defined.

Final count of confirmed participants will be reported to the general committee.

Another correspondence will be distributed regarding the finality of the matters,

especially, the flow of the program for the day. This includes time requirements for the

assembly and the point of origin and end point of the parade.

Identified judges will be oriented of the

criteria to be used in judging the entries and will be designated

in posts where they can rate the entries.

Tally sheets will be collected after the parade, then, it will be

submitted to the committee for tabulation and interpretation.

Eventually, announcing of winners follows.

An aftermath committee will be created to operate from the last day of the program

counting to a week to entertain any possible inquiries (claiming of cash prizes or

undistributed plaques, plaques wrongly awarded).

OTHER MATTERS

Trophies for awards are forwarded to the NDHM Awards Committee

It was understood and agreed that special awards will be given based from the over-all

impact of both Alay-Parada delegation and float entry parading as one.

The product labels, packaging and containers will be used according to the sponsorship

given by sponsors.

In the absence of a festival notably celebrated by a city or province, one may maximize

the cultural highlight their city or province is known for; or both can be used as theme of

entry.

One float entry in one vehicle is strongly advised.

In case of changes and updates, a separate correspondence will be initiated in various

medium; text messages, phone calls, e-mails.
