Objectives

• Understanding of how to support youth transitioning out of; Juvenile Probation, Child Welfare and/or Behavioral Health

• Identifying supports in the community to assist in the transition process

• Develop meaningful plans and utilize service delivery tools which will support a healthy transition for youth and families

• Utilize Internal and external strategies to build support for transition age youth
