1. Open web browser (google chrome/Mozilla firefox/IE) type http://training-svr.philhealth.gov.ph/ihcp (via LAN) or https://training.philhealth.gov.ph/ihcp (via Internet)

2. Click on the Primary Care Module

3. There are 3 sub-modules under Primary Care module-Member Inquiry-Health Profile-Case HistoryClick on Member Inquiry to search the PIN of Member (Required fields: Last name, First name and date of birth) then click

Member information will appear with PIN and Middle name (based on MCIS Database)4. Copy the PIN of member

5. Click Health Profile Module.6. Paste the PIN of Member then click

7. Members Profile will be viewed then click Generate Patient Health Profile

8. Automatically the Portal will ask who the Patient is. If Member is Patient Select Patient Type: Member Patient If Dependent is Patient Select Patient Type: Dependent Patient

Member Patient:

Portal will preview Patient Information and Member Information.1. Click Submit Information to proceed to eProfiling.Portal will preview Patient Information and Member Information.Click Submit Information to proceed to eProfiling. (No. 9)

Dependent Patient:

Portal will preview Patient Information and select the patient in the list of Dependents.Click Submit Information to proceed to eProfiling. (No. 9)

9. Accomplish the Online Patient Information (see below)

10. Then click Generate Case Number

11. Under Case History sub-module, portal will create a unique Case Number that will automatically save in PHILHEALTH Database.
