The EPJB Rapid Round
The Electronic Patient Journey Board (EPJB) is a module within the Patient Flow Portal (PFP) designed to facilitate Care
Coordination as part of the Patient Flow Systems Framework
The EPJB Rapid round is a multidisciplinary team (MDT) discussion whose primary focus is on the safety and care coordination
needs of every patient.
Who needs to be there?
Team Leader: Responsible for allocating tasks to members, and ensuring tasks are completed within the agreed EDD.
Any member of the MDT can lead the team.
Nursing/Midwifery representative. A Clinical Coordinator such as Nurse Unit Manager (NUM), in-charge or nurses or
midwives within the ward who provide clinical, social and psychological input.
Medical representative. A senior medical officer to provide expert clinical guidance and make decisions on the
treatment plan, EDD and transfer of care planning.
Allied Health representative. A member of each allied health team for advice on referrals; assist in coordinating
referrals with non-core disciplines, advise on EDD and transfer of care planning.
What needs to be discussed?
Approx. 30 second review of each patients current care plan
Estimated Date of Discharge (EDD). Update the EDD if required. What needs to be coordinated and achieved? Are the
patient and their carers/family aware and involved in their EDD?
How many days till discharge? (EDD#) Has appropriate transport, medications and discharge summary been arranged?
Waiting for What (W4W) Have delays to care or discharge for each patient been captured in? What is required to fix to
the delay. All expired EDD’s must have a W4W.
Referral status for each patient should be discussed and managed by the MDT.

Patient Flow Portal Access
All users must have PFP access to use the EPJB.
By logging onto the PFP or the EPJB the user is acknowledging that they are abiding by the principles as set out in the NSW Health Privacy Manual and the Electronic Information Security Policy 2013 PD2013_033.

Log in using StaffLink details.
The first time a user logs onto the PFP the Security Administration window Request Access will open:
• Select the Access Level you require from the drop down menu. • Select the LHD, facility and ward from the Facility List. Drag and drop across to the Access Table on the right. • Clicking “Save” will show a list of approvers • Click on the approvers email address to email requesting approval.
Deactivated User: How to re-request Access After 3 months of inactivity your access will be deactivated. Click on the deactivated access status and drag it into Facility List. You will then need to request the access level again as shown above.
Approving Access to the PFP
Clinical /Patient Flow/Facility Managers, Patient Flow/LHD Administrators and Connecting Care Users have the ability to approve other users who request the same or lower access to their domain.
• Access your StaffLink Number on the top right hand side of the PFP screen to open the Security Administration window
• Click on the ‘Approve Access’ tab. Highlight the record in the list and click ‘Approve’ or ‘Reject’. • Click ‘Yes’ or ‘No’ when asked to confirm.

Patient Flow Portal Security
All PFP users need to have authorised access to their Local Health District information systems via a StaffLink Logon.
More than three failed attempts to access the PFP will disable your access to both the PFP and your network access.

Accessing the EPJB Module Using Unique PFP Access
To access the EPJB via the PFP the user must click on the blue ‘Patient Flow Portal’ tab and log in using StaffLink details.

Using a Generic Ward Password (Kiosk Mode) Kiosk mode must be used when logging onto your large screen EPJB.
Click on the green ‘Electronic Patient Journey Board’ tab
Select the LHD, facility and active EPJB ward from the drop down lists
Enter the generic password for the ward. Passwords can be obtained from the facility PFP Administrator or by emailing:
Viewing and updating the EPJB as a Generic Ward User (Kiosk Mode)
To update or view a patient’s record, view a popup window or update the status of a transfer the user must login to the EPJB with their unique PFP log-in, by clicking either:
• A Hyperlink for the popup windows. • The Red ‘Login’ button located at the top right of the screen. • Any row with a patient in a bed.
If any of these are clicked the Login to Update Patient Details window will display:
Once logged in, the ‘Login’ button will turn green and display as ‘Logout’, with a timer counting down from 120 seconds. The StaffLink number/username of the user logged in also displays.

PFP and EPJB User Preferences
All users, except Ambulance Users, can set up their preferences which open when they logon to PFP or switch between facilities. For instance, a user can define an EPJB ward as their default login preference.
Click on the StaffLink number/user login name at the top right of the screen to open the Security Administration window, then select the ‘User Preference’ tab.
User Preferences
1. Login Module: Bed Board, Dashboard, EPJB, Predictive Tool or Reporting
2. Login LHD and 3. Login Facility: LHDs and Facilities based on the user’s access rights.
4. Bed Board Primary View: Admission Risk, Age, Alerts, EDD, Financial Status, Gender, Good to Go, LOS, LOS >, Outliers, Waiting for What.
5. Primary View Filter: Based on the filters for each Primary View.
6. Display Ward Description: Tick the checkbox to display Ward Description instead of PAS ward code.
7. Bed Board Default Filter LHD and 8. Bed Board Default Filter Facility: LHDs and Facilities displayed based on user’s access rights.
9. Bed Board Default Filter: Ward Type, Ward Group, AMO and Specialty.
When using the Bed Board, the default filters can be removed by clicking “Clear all Filters”. The filters will be re-applied when switching back to the given facility.
10. Bed Board ED Accessible Filter: Tick the checkbox to see only ED Accessible wards in Patient Mode, Patient List View and Bed Mode.
11. Clear Filters: Allows the user to clear any filters they have setup as a Bed Board default.
12. Reset Defaults: Resets to PFP default settings. LHD and Facility will be default to the first LHD and Facility the user has access to, in alphabetical order. Login Module will be defaulted to Bed Board. Primary View will be defaulted to Alerts and Clear. Display Ward Description will not be selected. Ward Type, Ward Group, AMO or Specialty filters will be cleared for all facilities.

EPJB Ward Summary
The Ward Summary is shown at the top of the EPJB with the following information.

Login or Logout of the EPJB in Kiosk mode. PFP Help for PFP user guides and resources and Logout of PFP.
Short Term Escalation Plan Indicators (STEP). Clinical Managers can change STEP level for their ward, Patient Flow Administrators/Managers can set whole facility and individual wards.
Ward Occupancy % and count of current Inpatients (excluding patients in Boarder, Well Baby and On Leave beds). % of patients with a Clinician Defined EDD. DWA: Direct Ward Admissions for the ward. Click the number to see patient details or enter a DWA.
Count of patients with an Expired EDD, and number of those with Waiting for What (W4W) entry.
Inter-hospital Transfers (IHTs) and Inter-ward Transfers (IWTs) for the ward. Click on number to see patient details.
Outliers in the ward, and Outliers in other wards. Click numbers to see patient details.
Patients Waiting in ED to be transferred to an inpatient ward. Click total to see patient details.
Free-text for ward Discharge Target to be entered. Free-text for In Charge details.

EPJB ward and bed setup for Clinical managers to change bed configuration and for Patient Flow Administrators to adjust ward EPJB columns
Ward Summary Popup Windows. Any number in the ward summary can be clicked to see patient details (DWA, IHTs and IWT, Outliers Out and Total Waiting in ED) which can be exported as PDF.
Row colour indicates the patient’s gender: pink (female), blue (male) and white (indeterminate/unknown).
Size and position of columns can be changed. If logged into EPJB with a unique username and password, changes will be
saved as preference. Click ‘Reset’ to return to system default.
Columns can be sorted alphanumerically by clicking on the column header.
Status of IHT, IWT and DWA’s cannot be updated unless logged-in.

EPJB Patient Summary
The Patient Summary shows a list of all the ward patients currently admitted Refer to the Appendix to see all the available columns.
EPJB Row Colours
Blue/Pink/White: patient gender - male, female or indeterminate/unknown.
Light Grey: Empty bed status has been set as Reserved, Off-ward or Boarder
Green: Bed is open and available
Orange: Bed is empty but has been set as unavailable.
EPJB Data
Many EPJB elements are entered directly by logged-in users, such as transfer requests (IHT, IWT, DWA) PTS bookings, and W4W entries.
EDD and EDD# are directly entered in most LHDs. Contact your local administrator to find out if your facility is able to do this.
There are multiple column choices in the EPJB with either free-text or drop down ability directly into the EPJB
PAS Data
Certain data comes from the local Patient Administration System (PAS) and can’t be updated on the EPJB. This data includes Name, Gender, Age/DOB, ID/MRN, Alerts, Admission Date, Admission Reason, Financial Status, AMO/Specialty, Address, Care Type and the admitted location (ward and bed/room).
PFP calculates Length of Stay and Time in Ward based on the PAS admission or transfer data.
eMR Data eMR Data feeds are available in most LHDs for Alerts, Diet and selected Referrals. Contact your local administrator or the Ministry of Health Patient Flow team to find out which these are activated in your facility.

Patient Details Window The Patient Details window can be opened by double-clicking on a patient’s surname in the EPJB or Bed Board, EPJB information can be updated from here, such as drop-downs and free-
text. IHT/IWT requests, PTS Bookings and W4W entries can also be made by right-clicking on the patient’s name.

Estimated Date of Discharge (EDD) The Estimated Date of Discharge (EDD) predicts the likely date that a patient will be transferred from hospital back into the
community. It provides everyone involved in the patient’s care, including the patient and their family/carer(s), with a tentative
date to coordinate the patient’s requirements.
Changes to the EDD should be recorded and the history of changes kept. If a patient is clinically ready to be transferred but is
delayed due to a delay to a service/diagnostic/etc., then their EDD should not be changed. This is so the delay to transfer can be
captured. By identifying delays, transfer processes can be improved. Staff should track the most common delays for patients. This
data can be then aggregated to understand the main constraints in the system.

The PFP in your LHD may receive PAS EDD updates. EDD will and EDD# will not be editable if this is the case. In most LHDs, the
EDD can be changed in the EPJB Patient Summary, Bed Board Patient List and Patient Details Window.
Click on the EDD box to see the calendar, then click the EDD date and update,
or
Click on the EDD# box then free text the number of days till discharge, or use the up and down arrows and update.
Hovering over the EDD cell in the EPJB summary will display EDD information. This can also be viewed and changed in the
Patient Details window.
Expired EDDs will display in a red cell. They must be either clinically redefined to match the patients plan of care, or have a W4W
entry added

Waiting for What (W4W) Waiting for What (W4W) is used to record delays to care or discharge so that they can be fixed and aggregated to improve
patient care.

All expired EDDs should have a W4W. Do not change an EDD if the patient is clinically ready to be discharged or transferred.
Click the green plus icon next to Waiting for What in the Patient Info. Tab of the Patient Details window to open the W4W
reasons. Right-clicking on a patient’s name in the EPJB summary will also give you the option to create a W4W.
The W4W reasons are grouped into five categories:

Diagnostics/Treatment
Discharge Process
Out of Hospital Referral
Transfer/Transport
Select the reason, enter any free text then click on Start Date to open a calendar. Click on the calendar date when the W4W
delay started. This cannot be before the admission date. After clicking Save, the W4W reason will be shown.
The W4W will display in the Patient Details window

A W4W reason can be edited by clicking on the icon
A W4W reason can also be closed by clicking in the “Close Date” calendar icon and selecting a date on the calendar. The details will still be displayed.
An open W4W reason can be deleted from the W4W summary window by clicking on the delete icon
The W4W initial will display in the W4W cell if added to the EPJB summary. An Expired EDD with W4W will display on a grey
background

Isolation requirement (I/R) The can be used to EPJB to show a patient’s isolation requirement (I/R). All patients in single rooms must have the I/R field
completed.
Update an I/R by:
• Completing the I/R field in the Patient Details window Patient Info. tab.
• Clicking the I/R cell on the EPJB Patient Summary (if available)
Select the reason from the dropdown list. Free-text is optional, click to X save.
The I/R cell will now display the Initial of the reason from isolation, and hovering over the cell will show the reason with the free-
text entry in brackets
The updated information will also be seen in the Bed Board Patient List view and any transfer requests made. If an
I/R is entered or updated in a transfer request, it will cross-talk with the I/R column or Patient Details window.

Good to Go (G2G) The EPJB can be used to highlight todays confirmed discharges so that the whole ward team are aware, and the Patient Flow
manager can see discharges at facility level.
G2G can be updated by:
• Completing the G2G section in the Patient Details window Patient Info tab.
• Clicking on the G2G cell in the EPJB Summary (if available) and select from the drop down.
The entry will be seen in the following colours.
Hovering over an entry will show the who last updated and when.

Short Term Escalation Plan (STEP) EPJB users can view the STEP level for their Facility, Ward and ED in the Top Right of their EPJB screen
Clinical Managers can change the STEP level for their ward on the EPJB. Patient Flow Managers/Administrators can change
Facility, ED and Ward STEP levels by clicking on the drop down arrows when they are logged-in.
The changes will be seen in the PFP dashboard and Bed Board. Hovering over the STEP colour will show the level, description
and update details.

EPJB Handover Double-click on a patient’s name in the EPJB Patient Summary to access Free-text Handover information in the Patient Details
window.
When information is written in any of the fields, a “H” icon will appear in the ID/MRN of the patient summary. Hovering over
this icon will show the information written.
Click on the PDF icon on the right of EPJB Ward Summary create a handover PDF in multiple formats.
Please note that due to privacy concerns users will not be able to view / printout their EPJB handover from any
largescreen EPJB logged in via kiosk mode.

Referrals Referrals can be added, viewed or updated from within the Referrals/Diet tab of the Patient Details window.
To add a Referral:
Click the green plus to open the Referrals List
Select a Referral: the selected referral will display in the referrals table as ‘Requested’. Update the status by clicking on
the Allied Health, Medial, nursing or other tab and selecting the Referral.
Referrals with an asterisk * have been set to link to eMR feeds for the facility.
Referral status can be viewed in the EPJB Patient Summary if the column has been added. The colour will depend on the status:
Red for Requested/Acknowledged
Orange for In Progress
Green for Completed
Grey for Cancelled
Hovering over the cell will display the description, status and last update date/time of the Referral

Referral email notifications
The Patient Flow Administrator can setup email notifications when a referral has been ‘Requested’ or ‘Cancelled’. The email will
display the name of the facility, ward, patient ID and the referral requested or cancelled.
In-scope LHD’s will be able to see read only eMR referrals on the EPJB. These will not be able to be manually changed in the
EPJB.

Diet
Diets populated by PAS or eMR are read only: they cannot be added to, edited or removed via the EPJB. Facilities that are not
receiving diets from PAS can manually add, edit or remove them via the EPJB. The Diet column can be updated in the EPJB Patient
Summary by single clicking in the cell in the Diet column, or within the Patient Details window.
To select a diet:
Highlight one or many (Ctrl or Shift and click) diets in the list on the left then drag and drop them to the list on the right
(or click the arrow) OR start typing the diet in the free text field at the top of the window to filter the list. E.g. typing
‘fish’ will only return values with the word ‘fish’ in the description.
Click ‘Save’
The diet will display in the Diet column on the Patient Summary window.
Hovering over the cell will display the full description of the Diet(s).
To remove a diet, open the Add or Remove Diet window and move the diet from the list on the right to the list on the
left of the window and click on the Save button.
Meal Assistance can be edited from the drop down with Assistance Required (Blue) and Full Assistance (Red) options. The EPJB
Summary diet cell will display the appropriate colour in the background

Patient Transport Service (PTS) Bookings PTS bookings can be made for in the PFP for admitted patients in NSW Health public hospitals. Non-admitted Emergency Department patients can be seen in the PFP in some LHDs. Non-admitted patients (OPD / Clinics) will still book PTS transport via telephone number 1300 233 500. You will not be allowed to complete the booking if the patient is:
• likely to deteriorate or • Falling outside “Between the Flags”
Without altered calling criteria documented. Inter Hospital Transfers (IHT) Inter-Hospital Transfer (IHT) requests must be made before the PTS can be booked. You will be asked if you want to book PTS each time you save an IHT request. If a booking has not already been made, click on the edit IHT icon in either the EPJB Patient Details window PFP Bed Board - Outgoing IHT tab and select Add PTS at the bottom of the IHT Request window.
The Patient’s details, from and to address and doctor details will be auto-populated from the IHT request.

The status of a bed can be changed by double-clicking on an empty bed row to open the Update Bed Status window

Enter optional notes, Nurse Name, a patient name if the bed status selected is Pt Off Ward, Boarder or Reserved and click on the
