The PCMH model from Blue Cross Blue Shield of Michigan (BCBSM) is the largest of its kind in the U.S. with more than 1,800 physicians in 500 medical practices throughout the state.
Approximately 3,200 additional physicians are currently seeking PCMH designation through the BCBSM program.
