WelcomeAs members of the West Moreton community we want you to feel you are in the best possible care at our hospital. You will be cared for by doctors, nurses and other healthcare professionals who are members of our local community.

When you first arrive, your nurse will show you to your room, and answer any queries you may have. It is likely that you will experience a different schedule to your normal routine. To assist with the change in routine, please let staff know of any preferences you may have, so they can make you feel comfortable.

We understand that our patients manage their conditions and their health everyday - through the foods they eat, the activities they engage in, how they monitor their health, which medicines they choose and remember to take. The staff at our hospital will encourage you to participate in your care during your stay and to actively join in discussions about your healthcare. Do not hesitate to tell your care team if you have any concerns.

Visiting hoursWe understand that support from family and friends is important to our patients in hospital. Generally, family members and friends are invited from 9.30 am to 8 pm. Visiting hours may vary in each ward, with a scheduled rest time for patients in the afternoon.

Your care team will be able to let you know the visiting hours while you are in hospital. One parent/carer can stay overnight in Children’s Sunshine Ward.

STOP We ask that your visitors clean their hands when visiting and advise them not to visit if they are sick or have recently been unwell.

General information

EnquiriesRelatives and friends can contact the hospital on (07) 3810 1111, and they will be directed to the correct ward. Patients do not have phones at the bedside, however staff will be keen to assist if urgent messages need to be passed on to the patient.

To help nursing staff deliver the best possible care, please ask your family to contact you directly on your mobile phone (if suitable in clinical area). Alternatively, you may nominate one person from the family to call the ward for patient updates and then share this information.

Transport and parkingThere are three (3) main options for staff, patients and visitors to Ipswich Hospital:

1. Free parking Ipswich Hospital provides a free ‘Park & Ride’ service from the following locations:

– Limestone Park (Quarry street entrance)

– Deebing Street (water tower)

‘Park & Ride’ operates Monday–Friday from 6.20 am to 5.50 pm (excluding Queensland public holidays).

2. Paid parking Ipswich Hospital Foundation operates three (3) car parks under agreements with West Moreton Hospital and Health Service:

– P1: A covered, multi-story car park located opposite the main entrance of Ipswich Hospital on Chelmsford Avenue

– P2: A covered, multi-story car park located opposite the main entrance of Ipswich Hospital on Chelmsford Avenue

– Ipswich Health Plaza located on Bell Street in the same structure as Ipswich Railway Station

Casual parking rates are $3.00 per hour or part thereof for a $21.00 a day maximum. These car parks have allocated disabled parking.

3. Ipswich City Council provides limited ‘Pay and Display’ on-street parking around Ipswich Hospital. On-street parking fees are set at $1.20 per hour.

A free taxi phone is located at the Chelmsford Avenue and East Street entrances.

What to bring • medications that you are currently taking

• medical information, such as x-rays if requested

• information about allergies and/or adverse medication reactions

• letters, reports, referrals and consent forms relating to your current admission

• a copy of any completed advance care planning documents – for example, an Advanced Health Directive, Statement of Choices and Enduring Power of Attorney forms.

Things to bring from home • comfortable clothes for day time wear

• comfortable sleep wear and robe (loose fitting)

• rubber soled slippers/footwear

• items such as magazines, books, music with headphones and a small amount of money

• toiletries including any ointments, eye/ear drops or patches

• any aids, such as glasses, hearing aids, CPAP if you have sleep apnoea, a walking stick or frame or any other mobility aid if told to bring in. The nursing staff caring for you will assist you with identifying what you require.

• if you are staying with a child, bring their favourite toy, nappies or any special items (with personal name labels attached)

• Valuables are your responsibility. Please consider leaving them at home.

What you need to know during your stay

• We will orientate you to your bed space including the use of the features in the room such as the nurse call buzzer, bed control, bathroom and shower facilities.

• You know yourself better than anyone. If you feel you need immediate care please press the nurse call buzzer.

• You are the most important member of the healthcare team and therefore your safety is a priority.

Coming for surgery?

Important notice• Please shower and wash your hair prior to coming in for surgery

• Do not shave for your operation

• Do not apply deodorant, powder, make up, creams or perfumes

• All nail polish must be removed and at least one gel or acrylic nail from each hand

• To avoid cancellation of your surgery, please remove all body, facial or oral jewellery

• Leave jewellery at home – wedding bands can be taped except for Orthopaedic, Breast/Axilla surgery where they may need to be removed.

ChildrenIf your child requires surgery there are volunteer staff who provide support to parents and families during this time. When possible they will escort you into the Operating Theatre so you can be with your child before they go off to sleep.

Parents or carers should have received and read the My Going to Hospital for an Operation Book. If not, please ask staff to provide this to you to read with your child.

What to expect after surgery?After your operation you will go to the recovery area until you wake up from your anaesthetic. Nurses will measure your blood pressure, pulse and temperature. They will also do a thorough skin assessment early in your admission or postoperative period to identify any potential pressure areas which may need management. Once you have recovered from your anaesthetic, you will be taken to the relevant ward. If you are an Adult Day Case then you will be taken to Ward 4E until you are ready to go home.

Nursing staff will ask if you are comfortable during your admission and sometimes you may be asked to give a score from 0–10 for any pain or discomfort. Staff will explain to you that a score of 10 would be the worst possible pain imaginable and zero is no pain at all. Relief of pain is important for your recovery and will enable you to perform normal activities as soon as possible. You should expect pain management to be discussed with you during your stay. There will be some pain after surgery but our goal is for this discomfort to be mild when resting. Additional pain relief will be available for you to request and some patients may be visited by the Acute Pain Service team.

Going home after surgeryYou need to have a responsible person to take you home from hospital. This should be arranged prior to coming in for your procedure. If you are having trouble with transport, please notify us prior to your procedure date.

Before your operation think whether you will need family or friends to help you with daily activities when you go home e.g. showering, cooking.

You also require a responsible adult to stay with you during the 24 hour period following your anaesthetic. In this time you should not drive a motor vehicle, sign any legal documents, consume alcohol or non-prescribed medications or be responsible for looking after children.

Driving after anaestheticYou should not drive a motor vehicle or operate heavy machinery for 24 hours after general anaesthetic. You should not drive after a local anaesthetic or after strong pain relievers such as oxycodone as they impair motor, perceptual or cognitive function.

ImportantWater only – No tea, coffee, milk or other drinks allowed. Medications as instructed may be taken up to one hour before your operation with a sip of water.

This is to ensure your safety when having an Anaesthetic. If you do not follow these instructions your surgery will be postponed.

Infants• It must be six (6) hours since the infant has had a commercial bottle feed.

• It must be four (4) hours since the infant was breast fed. Infants may have water as indicated for adults/children up until two (2) hours prior to the procedure.

IdentificationYou are required to wear an identification bracelet during your stay. This will be placed on your leg or arm. For your safety, staff will check your Name, Date of Birth and Hospital Number (UR Number) at regular intervals throughout your stay, including clinical handover, change of shift and administration of medications.

Infection preventionInfection prevention is important in our hospital. Please ensure that you help us by:

• washing your hands regularly, especially after visiting the toilet and before meals. There are hand sanitiser dispensers located throughout the hospital to help you with this

• notifying ward staff of any previous infections

• advising unwell family/friends to stay home and not visit until they are feeling better

• advising your nurse if you have had any vomiting/diarrhoea, coughs/colds or any open wounds or sores

• reporting any signs of infection surrounding drips/cannula, drains or wounds – this may be redness, pain or swelling.

Nursing staff may have to conduct swabs during your admission in order to identify any infection that may require isolation. Please speak with your nurse if you have any questions.

It’s Okay to Ask! Effective Hand Hygiene is the single most important strategy in preventing health care associated infections and we want our patients to feel empowered to ask any member of staff if they have cleaned their hands before they touch them.

Medication safetyWe need to know what medications you take at home: tablets, herbs, vitamins or any other substance you may buy from a supermarket or chemist. Please advise staff if you have allergies to any medicines or food. Please don’t be afraid to ask what medications you are being given: what it is for, what are the side effects, does it interact with my other drugs.

Blood clotsDeep Vein Thrombosis (DVT) and Pulmonary Embolism (PE) are blood clots that can form in the leg or lung. Both of these conditions can occur in hospital, and can cause long term complications.

How to help prevent blood clots?• Wear the compression stockings provided by your nurse at all times if it is

recommended for you. Compression stockings improve blood flow to the legs

• The hospital may also apply inflatable compression stockings to your legs, these are connected to a machine that pumps air and causes the stocking to inflate

• Blood thinning medication may be prescribed to you

• Do regular foot and leg exercises (wiggling toes and rotating ankles)

• Get out of bed and start moving around as soon as possible.

Pressure injuriesA pressure injury is commonly known as a ‘bedsore’. These can develop when you spend long periods sitting in a chair or lying in a bed.

What you can do to prevent pressure injuries?• Keep moving. Change your position as much as possible, staff will help you

with this if needed

• Don’t lie on a sore if you already have one

• Notify staff immediately if you notice a sore or red painful area

• Keep your skin clean and dry

• Ask your nurse to check your skin

• Eat a balanced diet and drink plenty of water.

Preventing fallsFalls can prolong a hospital stay, as well as your recovery period.

What can you do to help prevent falls?• Make sure that your buzzer is within reach and use it to call staff

for assistance

• Turn on the light so you can see clearly

• Never walk in socks or stockings

• Always wear flat non-slip shoes – ‘bring flat non-slip shoes with you to hospital’

• Bring in your glasses or hearing aids from home

• Never climb over the bed rails

• Make sure you are orientated to your hospital room and know where your toilet is.

Concerned about your condition?

Ryan’s Rule – patient, family and carer escalationWhat is Ryan’s Rule?Ryan’s Rule offers patients, their family and/or carer’s an opportunity to ‘escalate’ their concerns independently when they believe the patient in hospital is getting worse, is not doing as well as expected or shows behaviour that is not normal for them.

Step 1. Discuss concerns with treating nurse, doctor or other clinical team member

Step 2. If not satisfied with the response, the patient, family or carer can ask to speak to the Nurse In Charge, Senior DoctorOr at night, this will be the Hospital at Night Clinical Nurse Consultant or Team Leader.

Step 3. If the patient, family or carer remains concerned, they can call a Ryan’s Rule Clinical Review by telephoning 13 HEALTH (13 43 25 84) to request an independent clinical review, or ask staff to call on their behalf.The caller should state they are requesting a Ryan’s Rule Clinical Review, and provide:

– Name of the patient and date of birth

– Hospital

– Ward and Bed Number.

FamiliesFamilies are an important part of our healthcare team. We acknowledge that a child or young person’s family know them best and can provide an important perspective for our health professionals when they are involved as members of their child’s care team. By partnering with parents and carers, our staff can better understand a patient’s individual needs and make more informed decisions. Likewise, parents can make better decisions for their children when they have the information and support they need.

Communication about your careClinical handover is the sharing of information about you and your care between the on-duty clinical staff and the clinical staff who will take over your care at shift changes. Clinical handover is necessary to help keep you safe. It ensures that important information about your care and medical condition is accurately passed on between staff. We encourage your participation in this process at all times.

We are committed to ensuring your privacy and confidentiality and therefore we only share general information about your care at the bedside. If you have any concerns in relation to privacy please feel free to speak with your treating team.

Discharge planningWe want to ensure your discharge home is planned in advance. Your care team will discuss discharge plans with you early in your hospital stay including:

• what goals you need to meet to be discharged

• when you are likely to go home

• what preparations will need to be made.

The day before discharge, you are required to make arrangements for your transportation home and ensure that you are happy with the plan for your ongoing care. If you need assistance with arranging transport, please let your care team know. Before you leave hospital the nurse looking after you will ensure you have the following:

• any follow-up appointments

• medical certificates, if required

• discharge medications or a script to supply from your usual pharmacy

• education information to assist with your ongoing care information for any community support services you may require.

Transfer to rural health facilitiesYour treating team may identify that while you no longer require acute care at Ipswich Hospital, you may need to remain in hospital for ongoing care prior to recovery at home or discharge to a residential aged care facility. This could involve a transfer to one of our rural health facilities. The medical and nursing teams at our rural facilities will tailor your care to meet your needs and are supported by a range of allied health professionals including social work, physiotherapists and dieticians. Interim care facilities are available at the four rural facilities within our health service: Boonah Hospital, Esk Hospital, Gatton Hospital and Laidley Hospital.

Hospital pharmacyPlease bring your regular medications with you to hospital. The hospital can provide new medication when needed. A list of medications will be provided on discharge which will list medications to be continued or any changes to your medication regime. Discharge medications can be dispensed at our hospital pharmacy at a fee comparable to your local pharmacy.

No smoking policySmoking is now prohibited at all Queensland public and private hospitals and health facilities, and for five (5) metres beyond their boundaries. If you are a smoker, please talk to nursing staff about accessing help with nicotine replacement during your stay in hospital. We also ask for your assistance by informing your visitors that this is a non-smoking facility.

The Quitline 13 QUIT (13 78 48) is available to help all smokers wanting assistance with quitting or alternatively speak with you treating teams for information about how to quit smoking. Please note that patients do not have permission to re-locate or move hospital property outside of hospital grounds. This includes medical devices such as intravenous pumps, fluid stands/poles etc.

MealsA choice of meals from the hospital menu is available for most patients. If your doctor requires you to have a therapeutic diet, your menu card will be adjusted accordingly. Please advise nursing staff if you have any special dietary requirements such as food allergies or food intolerances.

Breakfast cereal is supplied for parents and carers in the children’s ward. Meals are only provided for parents of infants under six (6) months of age.

Understanding advance care planningAdvance care planning is a process of planning now for future health and personal care so your values, beliefs and preferences are made known. This can help to guide decision-making at a future time if you cannot make or communicate your decisions. Advance care planning is entirely voluntary. As part of your advance care plan, you can complete the following:

• Advance Health Directive—a legal document which allows you to give instructions about your future healthcare, and appoint an enduring power of attorney. It comes into effect if you have impaired capacity.

• Enduring Power of Attorney—a legal document giving another person(s) (substitute decision-maker) the authority to make personal (including healthcare) and/or financial decisions on your behalf when you have impaired capacity.

• Statement of Choices—focuses on your wishes, values and beliefs. It is a document used in many health facilities including hospitals, residential aged care facilities and general practice and supports advance care planning conversations. It helps those close to you make health care decisions on your behalf if you are unable to make those decisions for yourself.

• Wills—a legal document that states what you would like to happen with your money, belongings and other assets (your estate) when you pass away

Even if you are fit and healthy, it is never too early to plan your future health care. If you require extra support, please speak with your healthcare team. WMHHS offers a service which can assist you to complete advance care planning documents.

Applying for your medical recordsWest Moreton Hospital and Health Service promotes the right for patients to access medical, confidential and personal information that is collected and held about them. You can access your patient records on request by completing a Release of Information form, which can be obtained by contacting us on (07) 3810 1447 or by emailing [email protected].

Ipswich Hospital Worship Centre and Chaplaincy ServiceIf you would like any emotional or spiritual support during your stay at Ipswich Hospital, there is a chaplaincy service. Please advise staff if you wish to see the chaplain – chaplains of different faiths available.

A worship centre which can be used by people of all faiths is located on level 7, Tower Block of Ipswich Hospital and is open all hours.
