Anna is a 26-year-old mother of an 18-month-old toddler. She is 4 months pregnant.

MEET ANNA

After the baby is born, Anna will need reliable birth control and regular reproductive health care.

She is interested in receiving WIC benefits and is in need of prenatal care.

In the fall, Anna will return to Adagio Heath

for a flu shot.

Erik is a 20-year-old man and a member of the LGBTQ community.

MEET ERIK

Erik is in need of STI testing including HIV and screening for Hepatitis B & C.

Erik is also interested in a prescription for PrEP to

lower his risk of contracting HIV.

While in the office, Erik decides to receive a

COVID-19 vaccine because he feels very comfortable

with the Nurse Practitioner who is providing his care.

He also expresses a desire to quit smoking. The NP connects him to the Tobacco Free Adagio Health team, who will provide him with cessation counseling, support, and nicotine replacement therapy.

In a few months, Erik will return to Adagio Health for a pre-employment TB test and a flu shot.

Tatiana is a 16-year-old student in a local school district.

MEET TATIANA

As a younger student, she loved the fun nutrition lessons offered at her school by Adagio Health Power Up staff.

Now, she appreciates the in-depth reproductive health and life skills lessons that are taught at her school by a member of Adagio Health’s education team.

Tatiana also plans to seek reproductive care including a gynecological exam and birth control from Adagio Health as she makes informed decisions about her health and her future.

MEET LAUREN

Lauren is a 45-year-old uninsured woman veteran.

She recently discovered what feels like a grain of rice in her breast. She is in need of breast cancer screening services and will later find herself in need of more comprehensive diagnostic care and breast cancer treatment.

Lauren could also benefit from patient care navigation – she is confused by the health insurance marketplace and needs help filling out forms – she could also use assistance finding better housing.

Although she puts on a brave face for friends and family, Lauren is anxious and depressed about her health and financial situation. She will benefit from the behavioral health services offered to her at Adagio Health, and a referral to additional care.

There are multiple paths to better health for every patient, client, and student we serve. It is our goal to create opportunities for interconnection, while continually

aligning our services with the needs of vulnerable populations.
