Preferred Therapy Solutions therapists are experienced and competent clinicians trained to work with all patients requiring therapy services at Whitney Rehabilitation Care Center. Our services are tailored to the unique needs and goals of each patient requiring short term rehabilitation as well as those requiring long term care.
Patients in need of therapy services will receive a thorough and comprehensive evaluation by physical, occupational, and/or speech therapy. Based on the results of the evaluation, an individualized treatment plan will be developed to address any problem areas identified. For patients requiring short term rehabilitation prior to returning home, the plan may include regaining independence with walking, climbing stairs, and managing daily routines. For long term care patients, the focus may be on preventing falls, improving independence with eating, or designing a customized seating plan to ensure safety and comfort.
Our services and programs are designed to seamlessly integrate with nursing and the entire interdisciplinary team at Whitney Rehabilitation Care Center to improve patient care and outcomes.

PARTNERS INREHABILITATIONMANAGEMENT
Whitney Rehabilitation Care Center partners with Preferred Therapy Solutions on the Falls Prevention & Stroke Rehabilitation Program. This program is a comprehensive, interdisciplinary approach for patients that suffered a Cerebral Vascular Accident (CVA) which is often referred to as a Stroke. Included in this program is the education and rehabilitation on falls prevention.
FALLS PREVENTION & STROKE REHABILITATION PROGRAM
HOW DOES A STROKE IMPACT MY SAFETY AND MY RISK OF FALLING?
• Loss/reduced sensation and muscle control can lead to tripping and a possible fall
• Visual changes could impact ability to move safely in your home
• Potential loss of balance increases the risk of a fall
WHAT CAN I DO TO REDUCE MY RISK FOR FALLS?
• Maintain a good diet and hydration throughout the day
• Keep active and exercise daily
• Make sure your home has adequate lighting, space to move about safely, and is clutter free
• Reach out for medical or family assistance when needed
WHAT CAN MY REHAB TEAM DO FOR ME?
• Assess the type and severity of stroke you had and develop your treatment plan
• Provide education and training on safe mobility and activities of daily living
• Assess need for adaptive equipment
• Provide education on proper diet and nutrition
Whitney Rehabilitation Care Center partners with Preferred Therapy Solutions on the Dementia Program.
This program is a comprehensive, interdisciplinary approach for the education, treatment and monitoring of
residents with dementia. This program is designed to ensure that all residents identified as having dementia
are part of the “Just My Journey” program, with a goal of keeping the patient functioning at their optimal
physical and cognitive abilities. Residents who have a dementia or dementia related diagnosis, as well as
Alzheimer’s disease, would most benefit from the Dementia Program.
TREATMENT FOCUS FOR REHABILITATION• Treatment approach based on the specific stage of the resident
• Use of the clinical model for communication techniques and consistency, including the “90-second rule”
• Establish functional independence modification for activities of daily living and mobility as the resident moves through stages
• Use of the “Just My Journey” model to establish a comprehensive daily plan to facilitate resident independence and quality of life
CLINICAL PROGRAMS• Develop an interdisciplinary treatment plan based on the specific stage of dementia a resident presents based upon testing
• Determine the resident’s baseline functional status through a comprehensive assessment by all team members
• Provide education to the resident and family members on the disease process
• Develop an interdisciplinary discharge plan, based on individual resident needs
DEMENTIA PROGRAM

The ultimate goal for a patient admitted for short term rehabilitation is to safely return home at the highest possible level of function. Preferred Therapy Solutions “Transitions to the Community Program” is designed to accomplish just that: to get patients home safely.
At the core of the program is a comprehensive evaluation by the interdisciplinary team designed to develop a customized treatment program to meet the patient’s needs and goals. Patients will receive a combination of physical therapy, occupational therapy, speech therapy, and skilled nursing services.
While therapy and nursing services are integral parts of a patient’s recovery, a safe return to home requires more. In addition to the comprehensive interdisciplinary evaluations, our “Transitions to the Community Program” includes:
TRANSITIONS TO THE COMMUNITY PROGRAM
• Patient educational modules including fall prevention, home safety, medication management and self- monitoring
• Training in the necessary daily tasks that patients may need to perform upon returning home, such as dressing, taking their medications, and preparing a light meal
• Remote or in-person home assessments
• Performing a transition to home practice day where the patient completes all of the required daily tasks under the supervision of the healthcare professionals
• Pre discharge collaboration with home-care services to ensure a smooth transition home
