STEP 1: Print Provider Packet & Schedule Appointment A. Go to www.rowan.edu/healthforms and select the Mandatory Pre-Entrance Health Forms tab. Click on the
Provider Packet link and print it. B. If you have had a physical, within 12 months of the first day of classes, have your healthcare provider complete the
packet based on that exam. If not, schedule an appointment with your healthcare provider to have this packet completed. Also, talk to your provider about any chronic conditions you may have and how you can be prepare to deal with these conditions when you are at school.
STEP 2: Complete Online Medical History Forms & Alcohol-Wise Program
A. Go to https://studenthealth.rowan.edu to access the Online Wellness Link (OWL). Having trouble with your username or password? Go to https://id.rowan.edu/.
B. Select FORMS from the menu at the top. Complete and submit each of the three forms one time each. C. Check Yourself: You should notice a “submit date” next to each form once they have been completed.
D. FRESHMEN ONLY - Complete Alcohol-Wise Training (Phase 1 & Phase 2). Use the link provided in OWL or visit www.rowan.edu/hci and click on the Alcohol-Wise Link.
STEP 3: Enter Immunization Dates & Upload Provider Packet to OWL Make sure your name, date of birth, and student ID# are on all forms. Physicals must be stamped, signed and dated. Scan and save your documents into separate files in the below categories. Make sure your file is in one of the following acceptable formats: .gif, .png, .tiff, .tif, .jpg, .jpeg, .txt or .pdf. Do not use any numbers or special characters in your file name.

To enter the immunization dates from Page 3 of the Provider Packet log into OWL using the instructions from STEP 2A above. Select IMMUNIZATION and then ENTER DATES from the top menu. Follow the instructions provided to enter your immunization dates. Once you have completed the immunization entry select UPLOAD DOCUMENTS from the top menu.
A. Under “Choose Document” select the type of file to be uploaded. B. Click “Browse” to search for the scanned document. C. Click “Attach”. D. Repeat Steps 3A through 3C until all files have been attached. E. Click “Upload”. F. Check Yourself: Each file you uploaded should now have an upload date next to it. If there is no upload date, we
have not received your file(s).

Authorization of Treatment Statement
I hereby authorize Rowan University Wellness Center staff and physicians to provide health care evaluations, treatment and other medical services
as necessary. I certify, to the best of my knowledge that the information provided in my health record is complete and accurate.
In case of emergency, I authorize the Wellness Center to secure emergency medical evaluation, treatment and/or surgery at a hospital if such
treatment is deemed necessary. I authorize Rowan University Wellness Center staff and physicians to share any medical information with hospital
or emergency medical personnel in the case of an emergency or subsequent treatment. I understand that in the event of serious illness or injury,
my parent(s) or legal guardian(s) may be notified at the discretion of the Wellness Center staff. I understand that in situations in which Wellness
Center staff are concerned about suicidality, homicidality, child abuse or court order, information can be shared with appropriate parties (e.g.
emergency room staff, an identified potential victim, NJ Department of Children and Families, etc..) to ensure safety, wellbeing or compliance.

Matriculated students may be seen at the Wellness Center regardless of insurance provider. If I have purchased the student health insurance plan,
I understand the plan will be billed for most services received at the Wellness Center. Medical claim information related to healthcare services
received, will only be available to students by going to the health insurance provider’s website, firstudent.com, and logging into the My Account
center.
Please refer to our Privacy Practices for Protected Health Information for further information regarding your patient rights. A paper copy can be
found in our waiting room, is available on our website, and is available for you for review when you initially check-in at the start of treatment.
Certain therapies in Counseling & Psychological Services utilize email, text, videoconferencing and online support which may not be secure. These
communication techniques italicized above will be described in detail should they apply to your treatment. I understand that I may decline these
communication techniques in consultation with my therapist.
