Dear Friends of PA Connecting Communities, HAPPY NEW YEAR!Hope your holidays were wonderful and relaxing. Are you ready for a new season of classes and events! PACC continues to pro-vide programs and services in Butler, Washington, Beaver, and Allegheny Counties. Please contact our office with any questions regarding programs. PACC provides:

Please refer to www.hcsis.state.pa.us for new service definitions and waiver crite-ria for eligibility or contact the “Partnership” at 412-995-5000 ext.579 and ask to speak with Jeanine Schultz for information on WAIVER FUNDING.PACC has a new gym at our Butler Street Office. You must be accompanied by a PACC hab staff. This is ideal for those that do not belong to a fitness center or gym but would still like the experience of a good work out. PACC wellness coach will help develop a plan to meet your needs.NEW CLASS! Join us for OPEN GYM: Sweat off those inches with your friends. Join in playing basketball, volleyball, and much more every Wednesday from 6:30pm – 7:45pm at St. Thomas Moore in the South Hills. Begin the year with movement starting January 11, 2012.PACC has many exciting opportunities for volunteerism and employ-ment. PACC’s Gardening program in Lawrenceville continues to grow. Interested gardeners and artists have many choices to choose from. Please refer to our booklet for dates. Friday Nights will never be the same: Join us for a leisure eve-ning out. Play Wii, games, craft projects, bingo, pizza, and more. Join your friends at the Lawrenceville location for Coffee House (see Lawrenceville section of booklet for dates). The Partnership and South Western PA HCQU staff continue to provide training at our dances. See the flyers at the dances for a

listing of trainings for family/caregivers and participants.PACC believes in a holistic approach to providing services. Therefore, PACC has developed partnerships with Rehabilitation Innovations, Inc www.rehabinnovations.net to provide Speech, Occupational, and/or Physical Therapy. In addition, PACC has developed a partnership with North Point Psychological Services to provide assessment, consultations, and treatment. Call our office for more details.PACC has received a grant for our Advocacy Committee made up of participants, families/caregivers, and professionals. Let us help inform you of the issues and how you can help yourself to becoming an advocate. The “Citizen Leadership Network” and “Disability Voting Coalition of PA, Project Vote”, will be included in this advocacy group. Interested individuals may call the office for details. A PACC hab staff must accompany the individual to the center. PACC continues to provide Breast Cancer Education and Safety Awareness Programs as a result of our previous Komen grants. Call to set up a class or program at your location. All programs

and events offered by PA Connecting Communities are designed to provide CHOICE. All activities teach the skills needed to integrate into the community, maximizing ones potential to live as independent as possible with outcomes that can be measured and achieved. These programs are planned for individuals who require assistance in the acquisition, retention, or improvement of skills related to living and working in the community. In recognition of requirements to ensure individuals’ health and wel-fare, we offer services, at multiple levels of staff to participant ratio for individuals or small groups, or by staff with specialized training. We offer 1:1, 1:3, 1:6 and 1:12 ratios for waiver funding billing. We provide programs within the community, at various community centers, places of service such as fitness centers and/or at group homes for your convenience. Programs and events are planned in accordance to the need for services established through the assess-ment processes in the participant’s IEP or ISP. PA Connecting Communities follows the principles of self-determina-tion. All programs and events are consistent with our Mission and are directed towards community inclusion. We value the rights of all individuals and emphasize the strengths of each person while devel-oping areas of weakness. As always, programs may be self-paid and special arrangements may be made. Advocacy Committee MembersJoin us at the offi ce once a month for the PA Connecting Communi-
ties (PACC) Advocacy Committee. This committee was created to bring together individuals with a variety of disabilities, their family members,
support persons, professionals, and other allies to focus on policy education, peer support, communication, and participation in the civic process. Our purpose is the empowerment of individuals with disabili-ties and their allies to become educated about policies that aff ect them
and their well-being and become self-advocates by engaging in the civic process of voting, telling their stories to legislators, and engaging
policymakers in order to create systems change.

PACC provides 4 “Day Activity Centers” for your convenience in Allegheny County. Call for needed information. (412) 621-6151. Tours and trial visits are encouraged. Come Join US! Improve your life skills, social and vocational skills. Relax and Play bingo, cards, create arts and crafts projects, improve your fitness, expand your reading skills and learn some new cooking and baking recipes. What ever your needs are, we will try to accom-modate you. Job readiness skills can be developed while applying for jobs. Practice your interviewing skills. We allow for flexible days and times to meet your needs. Waiver funds may cover cost using W7057, W7058 and W7060 if PACC is listed as a provider in your ISP plan. Private pays are also welcome.
