This document is about how we want to communicate and engage with you about our work for West Yorkshire and Harrogate Health and Care Partnership.

We already work in six local areas. These are:

• Bradford District and Craven• Calderdale• Harrogate• Kirklees• Leeds • Wakefield

A lot of communication and engagement work is already taking place in these places.

We are planning to do more communication work about West Yorkshire and Harrogate health and care across a bigger area.

Our plans

We will set up a Partnership Board that will oversee the West Yorkshire Harrogate Health and Care Partnership.

We will make sure that public, patient, carers and communities are involved in all our plans for West Yorkshire and Harrogate.

We will listen to them and think about what they have said when we plan and make decisions.

It will be made up of people from NHS services, local councils, Healthwatch, voluntary and community organisations.

We have many areas of work we think are very important. A few examples of these are:

• Community services, urgent and emergency care;

• Hospitals working together;

• Maternity services;

We will make sure that our communications and meetings are inclusive to people with disabilities.

• Stroke and cancer.

Our objectives for the next year

We plan to raise awareness of how important it is to join up health and care across West Yorkshire and Harrogate.

We want to make sure that people, their families and carers have a say in plans of changes to the health care services they use. This will include providing information and support so people can stay well.

We want to make sure we involve the people that work for us in developing our plans.

We want to better use digital information by updating our website and social media and by making information available in different formats.

We want to work with medical professionals, health organisations, voluntary groups, communities and politicans to make sure they are involved with our work and our plans.

We want to keep the public, our partners and staff updated on the difference our work is making.

We will tell people simple, clear messages through our communication and engagement work.

Key messages

For example, we want people to understand that health and social care services need to change so that people’s health and care needs are met to help them take better care of themselves, where safe to do so.

We also want people to know that working with other organisations, such as the fire service, police and universities, and communities will help to keep them well.

And we know that education, where you live, poverty, housing and employment are important too.

We will listen to the public, patients, carers, staff and the community and make changes to this plan if we need to.

We will use what we learn from this to make the way we work better, to make sure that our communication and engagement reaches everyone.

Evaluations

As we work, we will look at our activity, to see what is working well and what isn’t.

We also want to let people know that a lot of work is being done to join up social care and health services to improve their health.
