Introducing free services for these zones OVERVIEW Northern NSW Local Health District has recently introduced a number of new services as part of the Leading Better Value Care (LBVC) Program. These new services complement existing services designed to support ongoing management of people with chronic disease within our region, and include: High Risk Foot Service Knee and Hip Arthritis Service (KAHAS) Renal Supportive Care (RSC) Osteoporosis Refracture Prevention Program. This event is designed to ensure that participants have a clear understanding of the 4 LBVC services in our region, referral pathways and evidence base behind these services. Participants will have an opportunity to engage and ask questions of the local service providers. LEARNING OUTCOMES By attending this workshop, participants will have increased their confidence, knowledge and skills to enable them to: Explain to patients and colleagues the services offered by: High risk foot service Hip and knee osteoarthritis service Renal supportive care service Osteoporosis refracture prevention program. Explain the evidence base underpinning these services. Refer patients appropriately to these services based on service- specific indications for referral. Use HealthPathways to assist with assessment, management and referral of patients eligible for these services.
