Osborne Park Hospital – AccreditationThe hospital has operating theatres, procedure rooms, five inpatient ward areas, birth suites, a day therapy unit, outpatient clinics, diabetes education, assessment and discharge follow-up and community services. There are also on-site pathology (PathWest) and radiology services at the hospital.

The hospital is accredited by the Australian Council on Healthcare Standards and has the longest accreditation of any metropolitan secondary hospital in Western Australia. This reflects our sincere commitment to providing high-quality patient care at all times and across all services.

In an Australian first, the hospital is also designated as a Pathway to Excellence site through The American Nurses Credentialing Centre (ANCC).

The mission of the ANCC, a subsidiary of the American Nurses Association (ANA), is to promote excellence in nursing and healthcare globally through credentialing programs. ANCC’s internationally renowned credentialing programs certify and recognise individual nurses and midwives in specialty practice areas; recognise healthcare organisations for promoting safe, positive work environments; and accredit continuing nursing education organisations.

Osborne Park Hospital is a Baby Friendly Health Initiative (BFHI) accredited facility. This accreditation acknowledges that we have met World Health Organisation (WHO) and United Nations Children’s Fund (UNICEF) global criteria for implementing the Ten Steps to Successful Breastfeeding. OPH encourages, supports and promotes breastfeeding as the optimal way for a woman to feed her baby, and provides a high standard of care for mothers and babies.

The hospital is located approximately 13km north of Perth City. The site is easily accessed via the Mitchell Freeway and is located approximately 1km from the Stirling Train Station.

What to BringThere are a few important items you should bring with you to hospital:

• Medicare card (if eligible).

• Health insurance card (if applicable).

• Any medicines/supplements/vitamins you are taking, in their original packaging.

• Private x-rays related to your condition.

• Spectacles, hearing aids, dentures or walking aids.

If you are staying overnight or longer, you should also bring:

• Sleepwear, underwear and suitable footwear.

• Toiletries such as a toothbrush, toothpaste and shaving equipment.

• Small amount of cash or credit card for newspapers, snacks and TV hire, if desired.

• Reading material or suitable entertainment, if desired.

LaundryThe hospital does not have laundry facilities available on-site. Please ensure you make alternative arrangements for washing your clothes.

ValuablesThe hospital does not have the facilities to safeguard money or valuables. PLEASE DO NOT BRING large amounts of money or valuables. The hospital is not liable and does not accept responsibility for any loss of valuables during your stay with us.

Patient Information

On ArrivalPrivate PatientsIf you have private health insurance you can opt to be treated as a private patient. Private patients can often choose which doctor treats them. They can also enjoy other benefits such as free newspapers from Monday to Friday, free television and phone credit.

We encourage patients with private health cover to use it. The money we receive is directly invested in clinical services for the benefit of all patients.

On the WardWhen you are admitted to the ward, please let staff know:

• If you have any impairments such as hearing or vision.

• If you have any dietary or other special needs.

• If you need a medical certificate for your employer.

• Your first contact person’s details.

• Any allergies.

• If you plan to leave the ward for any period of time for any reason.

You will receive a plastic wrist band with your name, date of birth and patient identification number, which must be worn at all times.

It should only be removed when you are discharged from the hospital. Please inform staff if your name and date of birth are incorrect, inaccurate or illegible.

A doctor will record your medical history, conduct a physical examination and discuss your treatment plan.

It is very important that you inform your doctor about any allergies and medicines you are taking, including over-the-counter remedies or alternative supplements/vitamins.

Aboriginal Hospital Liaison Officers

Aboriginal Hospital Liaison Officers attend OPH to visit patients with Aboriginal and Torres Strait Islander backgrounds and ensure their needs are met. These officers act as a link between hospital staff, patients, family and community health services. They assist with providing patients with the best possible care while recognising their cultural needs. Please ask nursing staff if you would like to speak to an Aboriginal Hospital Liaison Officer.

Informed ConsentThere is always a risk involved in any surgical procedure. Informed consent means that you have received all the information you need to enable you to make a decision on any surgical procedure available to you. Your doctor or health professional will discuss the risks and benefits, expected outcomes and known complications so you can decide whether you agree to proceed. If there is anything you are unclear about, you can ask for clarification or even a second opinion. Your treatment is your choice.

Falls PreventionA nurse or midwife will assess your risk of falls on admission, when your health condition changes and if you move wards. Staff will discuss your personal risk of falling and ways to prevent falls and minimise harm from falls in order to reduce this risk.

Pressure InjuriesA pressure injury, commonly called a ‘bed sore’ is a redness, break or blister of the skin caused by constant pressure on an area of the body.

A nurse or midwife will assess your risk of a pressure injury on admission, when your health condition changes and if you move wards. Staff will discuss your personal risk of pressure injuries and ways to reduce your risk in hospital.

During Your StayOur TeamYou will be looked after by a highly skilled multidisciplinary team throughout your stay – nurses, midwives, doctors, allied health professionals and others. All staff wear an identification badge, which you can ask to see at any time. They will introduce themselves to you and are committed to assisting you with your queries and concerns.

Speak Up if You Feel UnwellNobody knows your health like you and you are the best person to notice any changes in your health. Likewise, your carer, family and loved ones who spend a lot of time with you, may notice changes in how you look or act before health professionals notice these changes.

If you or other people close to you notice changes in your health, please speak up and let one of the healthcare team know. If you think it is urgent then use the call bell immediately.

Managing Your MedicationsManaging your medications safely at home and in hospital is vital for your health. It’s important to know what medicines you take, and why you take them, as mistakes can occur when medicines are prescribed, taken or used incorrectly. Managing your medication safely in hospital means:

• Letting staff know if you are allergic to any medications.

• Finding out what your medicines are for.

• Making sure you understand how to take your medicines.

• Asking questions if you are unsure about your medicines.

• Ensuring you have a written list of all your medications and understanding the reason you need to take them, before you are discharged.

Confidentiality & PrivacyYour medical information will always be treated as confidential. If you do not wish for any information to be disclosed to parties other than the staff who are caring for you, please advise the ward coordinator.

Visiting HoursWe encourage your family and friends to visit you during your stay. Visiting hours are:

Women’s and Newborn Services

10am to 8pm.

General Wards

8am–1pm and 2pm–8pm.

Rest period 1pm–2pm.

Please speak to the nursing staff if your circumstances require flexibility in visiting hours.

Hand HygieneThe single most important factor in preventing or reducing the risk of infection is clean hands. Staff must clean their hands before touching you or your surroundings and we encourage you to ask staff if they have cleaned their hands. Hand hygiene compliance rates are on display throughout the hospital.

Osborne Park Hospital staff have the highest hand hygiene compliance rate out of all WA metropolitan hospitals.

No SmokingSmoking is not permitted anywhere inside or outside the hospital. Staff can provide patients with advice to assist with quitting, as well as nicotine replacement therapy to manage cravings during the hospital stay. Do not hesitate to ask staff for more information.

Chaplaincy & Pastoral Care ServicesThe hospital chaplain is available to offer support and sacraments to all patients and families, regardless of religious affiliation. Please ask staff for a referral.

Patients, visitors and staff are welcome to visit the Chapel on the Ground Floor in E Block, near Ward 4. An ecumenical service is held every Sunday morning at 10:45am, except for holiday periods, with dates of absence being posted outside the Chapel entrance. Please note that there are no washing facilities contained within the Chapel for religious rituals.

Interpreter ServiceIf you need to discuss your health in your own language, staff will provide accredited interpreters and translators via phone or if necessary, in person. This includes Auslan interpreters (sign language) for patients with a hearing impairment.

Healthy EatingOur food service delivers nutritionally balanced, tasty meals three times a day.

Breakfast is delivered between 7:30am–8:15am, lunch between 12pm–12:45pm and dinner between 5pm–5:45pm.

Morning, afternoon and evening light refreshments are also provided. Be sure to tell your nurse or midwife if you have any dietary requirements related to allergies or religious or cultural beliefs.

Café & ShopThe Friends of Osborne Park Hospital Incorporated operates a kiosk in the entrance foyer of the Rehabilitation and Aged Care Unit in F Block, as well as ‘Café Osborne and Shop’, which is located in the hospital walkway between B and E Blocks, where dining and alfresco facilities are available.

Café Osborne Opening Hours:

Monday to Friday 9am–4:15pm.

Saturday from 11:30am–3pm (Closed Sunday and public holidays).

The Kiosk in the Rehabilitation & Aged Care Unit Opening Hours:

Monday to Friday 9am–1pm (Closed weekends and public holidays).

A trolley service with a selection of reading material, gifts and food comes through the wards on weekdays. Please note this is a cash service. Drinks and snacks are also available from vending machines throughout the hospital.

Patients on a special diet must first consult with their nurse or midwife before consuming any snacks.

TelephonesThere are bedside telephones in the wards available for hire from HTR rental company.

Please check with your ward clerk, nurse or midwife before using your mobile phone as it may interfere with some medical equipment. We ask you to respect other patients by restricting mobile phone use when within earshot of others.

Televisions & RadiosDigital television can be accessed from the HTR rental company after paying a connection fee. Payment can be made via credit card (using your telephone or when the HTR customer service representative attends), or by voucher. Vouchers can be purchased from the HTR customer service representative and Café Osborne. You can listen to the radio for free through the headset on your bedside table. Please access the free patient information channels for important information about healthcare at Osborne Park Hospital.

Electronic EquipmentPortable DVD players, game consoles, laptops, iPods, iPads etc. are permitted at your own security risk. However, headphones must be worn in the interest of other patients. All mains-powered equipment must be tested by a hospital electrician before being used. For more information, please speak with your ward clerk.

EnquiriesYour family and friends can telephone the Osborne Park Hospital switchboard on (08) 9346 8000 (6:15am–10pm, seven days a week) for general enquiries.

When You LeavePreparing to Leave Your doctor or nurse can provide you with information on how long you can expect to stay in hospital. With the help of your chosen support person, it would be helpful for you to start preparing a list of questions that you need answered before you are discharged. On the day you are discharged, you may be given some medications (if required) and a summary of your current medications. You will need to contact your GP for additional supplies.

Surgical patients are generally given prescriptions, which you will need to fill yourself. If you are going to a nursing home or hostel, medications will be organised for your arrival.

If you are discharged home, prior to discharge the occupational therapist may plan a visit to your home to assess safety issues and any home modifications required. Also, an overnight trial in your own home may be arranged. Home meal services, home nursing care, home help (e.g. with cleaning or gardening) and aids and appliances (e.g. walking sticks or frames) can also be arranged for you as required.

The multidisciplinary Rehabilitation and Aged Care Intervention Liaison Service (RAILS) team can provide same-day assessment and facilitate a fast response to referrals for a variety of community-based services.

If you need to attend the Day Therapy Unit for ongoing rehabilitation therapies, you will be given regular appointment times between 8am–4pm, Monday to Friday. If you cannot attend or need to discuss changes, please notify the Day Therapy Unit Coordinator as early as possible or leave a message on our answering machine by phoning (08) 9346 8101.

DischargeWe aim to discharge our patients by 10am.

Please arrange travel and/or pick-up with this in mind. If you are experiencing difficulties arranging transport, please advise your nurse or midwife as early as possible.

On discharge, you will receive:

• Discharge letter for your GP.

• Your private x-rays.

• Medication prescription and/or equipment, if required (a pharmacist will conduct a thorough medication reconciliation review).

• Medical certificate, if requested.

• Details of support services, if required.

• Follow-up appointments and/or referrals, if required.

• Please ensure that staff answer all your questions before you are discharged.

Your Rights & ResponsibilitiesAs our patient you have a number of fundamental rights and responsibilities.

You have the right to:

• Choose to receive free public hospital services as a public patient, or choose to be treated as a private patient in a public hospital with an associated cost.

• Receive treatment based on your health needs and if you need to wait for this treatment, to be told how long you can expect to wait.

• Have access to a range of public hospital services regardless of where you live in Western Australia.

• Have access to an interpreter.

• Agree or refuse to participate in medical/nursing student training or medical research.

• Be treated with respect, dignity and consideration for your privacy and special needs.

• Be accompanied by a family member, friend, carer or person of your choice where appropriate.

• Receive safe and high-quality healthcare provided with professional care, skill and competence.

• Receive a clear explanation of any proposed treatment, including possible risks and alternatives, before agreeing or refusing to have the treatment.

• Seek a second opinion.

• Be given information about your continuing healthcare before you leave the hospital and have your contact details kept up to date.

• Apply for access to your medical records under the Freedom of Information (FoI) Act 1992 and to have personal information kept confidential.

• Compliment, comment or complain about the healthcare you receive and to be given information about how to lodge a complaint, without compromising your healthcare.

In return, you have a responsibility to tell hospital staff:• About your own (and your family’s)

medical history and about any treatment or medication you were receiving when you were admitted to hospital.

• Of any changes in your condition, including problems you may have with any treatment you are receiving.

• Any special needs you have.

• Follow treatment instructions, or let hospital staff know if you are unwilling or unable to do so.

In consideration for staff and other patients you should:• Try to keep hospital appointments. If

you need to cancel an appointment, you should advise the hospital as soon as possible.

• Be considerate to the needs of other patients. Respect hospital staff and their right to a safe and pleasant work environment.

• Be aware that public hospitals have a special role in training health professionals. With your consent, your treatment may provide an opportunity for such training.

A Message for Family & Friends Who Provide Care to PatientsWe acknowledge that family members and friends provide significant support and ongoing care for patients, especially when they are discharged from hospital.

To assist carers of hospital patients, Carers WA has developed a Prepare to Care resource pack that includes a place to record information provided by hospital staff, a health and medication diary, plus information on support services for the carer.

The clinical staff can assist you with obtaining a ‘Prepare to Care’ resource pack.

Community Advisory CouncilThe Osborne Park Hospital Community Advisory Council (CAC) consists mainly of members of the community who volunteer their services. The aim of the CAC is to provide a community perspective in planning, delivering and evaluating healthcare services at OPH. The CAC meets once a month.

If you would like more information or are interested in joining the CAC, please contact the OPH Executive Secretary on 9346 8141.

Friends of OPHOPH volunteers, also known as The Friends of Osborne Park Hospital, complement the care provided by hospital staff by generously giving their time to enhance the comfort and well-being of patients and their loved ones. They also run the hospital café (Café Osborne) and kiosk (K Block) and provide administrative support to hospital staff.

More than 100 volunteers give their time working at Osborne Park Hospital for the benefit of our patients, visitors and staff. Our volunteers are valued members of the team who support our patients and their families and friends, often through difficult times.

1. Check your Blood PressureHigh blood pressure often gives no warning signs and it isnecessary to have it checked regularly by your doctor. Highblood pressure sharply increases the risk of heart attack,stroke and other conditions if it goes undetected anduntreat-ed. With proper medical treatment this diseasecan be brought under control in most cases.

2. Don't SmokeSmoking greatly increases your risk of heart attack. It isnever too late to stop, as evidence shows that damage canbe very quickly reversed. Do not smoke cigarettes anddis-courage your family and friends from smoking.

3. Reduce Blood FatsA high level of blood fats (cholesterol and/ortriglycerides) increases the risk of heart attack. Withmoderated changes in your diet, your blood fats can bereduced to a safer level. This means reducing total fat intake,particularly saturated fats, and cholesterol in rich foods. Abalanced diet is necessary for good health. Ask your doctorfor advice or contact the National Heart Foundation in yourState for information on nutritional guidelines for you andyour family.

4. Maintain Normal WeightIf you or your children are too fat the chances of developing health problems are increased. Ask your doctor for a sensible weight reducing diet or get the Heart Foundation's publication "Guide to Losing Weight." Bad food habits formed in childhood are hard to break in later life, so it is important your childrenfollow a healthy eating pattern.

5. Improve Physical ActivityExercise should be fun not a chore. Choose a physical activity you enjoy, make it a family affair, walk the dog, cycle around the park, swim, play tennis, take the family for a sail. Exercise regularly, your doctor can tell you what kind of activities will suit your age and physical condition.

6. Have Regular Check-upsRegular check-ups enable your doctor to detect and treat conditions that can lead to heart attack and other forms of heart and circulatory disease.
