If you wont be able to obtain profound anesthesia today manage the pain medically and refer. If you can extract today and provide a good experience then consider offering same day extraction and grafting. If they will want an implant consider referring. Please think of the best interest of the patient.

Having a general dentist preform oral surgery may be an excellent idea? Two thoughts.

Many general dentists can provide oral surgery services with the same result and in the same time as a specialist. When the clinical outcome between specialist and GP is equal your oral surgery referral should be based on several factors: complexity, convenience for patient and doctor, time spent in treatment and fee (HMO is an absolute referral).

Where some general dentists and oral surgeons are equal in quality and speed on single tooth extraction, most are much different with full mouth extractions, implant placement and complex restorative surgery. Thus, if you have an efficient and quality general dentist they should do the single tooth extractions that they want to, and staff your oral surgery days to the number of cases that require more advanced care.
