Anyone who is not a member of DDB, PDEA, PNP, AFP, NBI, PCG, BUCUS or any government agency exercising enforcement function who can provide information that shall result in confiscation or destruction of dangerous drugs and marijuana plantations.

Legibly fill out the Information Report Form (IRF) at the back page and send it to PDEA by fastest means as follows:

a) Fax No: (02) 9272899; or b)By mail/courier to:

c) Personally submit to the nearest PDEA office

NOTE: Two (2) forms should be filled out . Black ink should be used for thumb print. Make sure that the thumb print is clear (not too light nor too dark).

Once your information resulted in a positive operation, you’ll learn about it through tri-media news report, or get in touch with us through telephone number 927-9702 local 149 or 150 mentioning only your codename and IRF number. Submit the following require-ments to the OPE Secretariat:

1. Letter stating your intent to file a formal claim. Preferably handwritten, with your codename signed with your right thumb mark.

2. Photocopy of duly received IRF.

Payment will be made in cash or check. Special arrangement will be made to keep your identity secret. In exchange for the reward, you must turn in to the Operation “Private Eye” Secretariat the original copy of the IRF, signifying that you have already received the reward.

NOTE: Purity of the confiscated precursors and essential chemicals shall be considered in the computation of the reward.

Ten percent(10%) of the current local market value as determined by the board shall be

given as reward when the following are confiscated:

Ephedrine; Lysergic Acid;

Pseudoephedrine; 3,4 - methelenedioxyphenyl-2-propane;

N-Acetylanthranillic Acid; 1-phenyl-2-propane;

Ergomentrine; Piperonal; and

Isosafrole; Safrole

Five percent (5%) of the current market value of essential chemical shall be given as

reward for the following:

Acetone Acetic anhydride Anthrallic acid

Ether Acid Ethyl Ether Hydrochloric acid

Methy ethyl ketone Phenylacetic acid Piperidine

Potassium permanganate Sulfuric acid toluene

Two percent(2%) of the value of the confiscated/destroyed crops and its derivative up to

a maximum of P500,000.00 shall be given to informants who gave information provided

that the cultivator/planter is arrested

Dismantled clandestine laboratories for shabu shall be classified and be given the

following reward:

1. Small Clandestine Laboratory

(Production Capacity is less than 10

kilos/batch)

Reward of P500,000.00 + computed

reward under Section 11, para a,b and c of

the guidelines.

2. Medium Clandestine Laboratory

(Production of 10 kilos or more but

less than 50 kilos per batch)

Reward of P1,000,000.00 + computed

reward under Section 11, para a,b and c of

the guidelines.

3. Large Clandestine Laboratory

(Production Capacity is 50 kilos or

more/batch)

Reward of P1.5 million + computed

reward under Section 11, para a,b and c of

the guidelines.

The following should be submitted with tabbing to OPE Reward Committee c/o IIS, PDEA

1.Endorsement/Certification from Head of Office that the successful operation resulted from the timely information received from the informant.

2.Certification from Handler/Team Leader that informant is not a member of the OP unit or any law enforcement agency.

3.Copy of Inventory Sheet of Item Seized. 4.Final Laboratory Test Result (for dangerous drugs and CPECS) 5.Purity Test Result (for shabu, cocaine, heroin, MDMA and ephedrine)

6.Laboratory Certification as to size (for clan lab) 7.Spot Report and After-Operations Report 8.Referral to the prosecutor/court 9.Status of the case 10.Commitment Order 11.Newspaper Clippings 12.Pictures (with captions)
