C Qualified person’s statements and certifications
As the qualified person, I certify that:
A phase one environmental site assessment of the record of site condition property, which includes the evaluation ofthe information gathered from a records review, site reconnaissance, interviews, a report and any updates required,has been conducted in accordance with the regulation by or under the supervision of a qualified person asrequired by the regulation.
A phase two environmental site assessment of the record of site condition property, which includes the evaluation ofthe information gathered from planning and conducting a site investigation, a report, and any updates required,has been conducted in accordance with the regulation by or under the supervision of a qualified person asrequired by the regulation.
The information represents the site conditions at the sampling points at the time of sampling only and the conditionsbetween and beyond the sampling points may vary.
As of 2015/11/16, in my opinion, based on the phase one environmental site assessment andthe phase two environmental site assessment, and any confirmatory sampling, there is no evidence of anycontaminants in the soil, ground water or sediment on, in or under the record of site condition property thatwould interfere with the type of property use to which the record of site condition property will be put, asspecified in the record of site condition.
Ground water sampling has been conducted in accordance with the regulation by or under thesupervision of a qualified person as required by the regulation.
As of 2015/11/16, in my opinion, based on the phase one and phase two environmental siteassessments and any confirmatory sampling, the record of site condition property meets the applicable full depthgeneric site condition standards prescribed by section 36 of the regulation for all contaminants prescribedby the regulation in relation to the type of property use for which this record of site condition is filed,except for those contaminants (if any) specified in this record of site condition at Table 2, Maximum contaminantconcentrations compared to standards specified in a risk assessment.
As of 2015/11/16, the maximum known concentration of each contaminant in soil, sedimentand ground water at the record of site condition property for which sampling and analysis has been performedis specified in this record of site condition at Table 1, maximum contaminant concentrations compared toapplicable full depth generic site condition standards.
I am a qualified person and have the qualifications required by section 5 of the regulation.
I have in place an insurance policy that satisfies the requirements of section 7 of the regulation.
I acknowledge that the record of site condition will be submitted for filing in the Environmental Site Registry, thatrecords of site condition that are filed in the Registry are available for examination by the public and that theRegistry contains a notice advising users of the Registry who have dealings with any property to considerconducting their own due diligence with respect to the environmental condition of the property, in addition toreviewing information in the Registry.
The opinions expressed in this record of site condition are engineering or scientific opinions made in accordancewith generally accepted principles and practices as recognized by members of the environmental engineeringor science profession or discipline practising at the same time and in the same or similar location.
I do not hold and have not held and my employer HALLEX ENVIRONMENTAL LTD.does not hold and has not held a direct or indirect interest in the record of site condition property orany property which includes the record of site condition property and was the subject of a phase one orenvironmental site assessment or risk assessment upon which this record of site condition is based.

1.0 REPORT PURPOSE
The RSC property is identified as former Hope Centre (municipal address 179-181 East Main Street,
Welland, ON), currently depicted as municipal address 179 East Main Street, Welland, ON. Historic land
use has included community use (Housing Help Centre and Hope Centre 2005-2015) in addition to
various commercial businesses since the early 1920’s. Hallex Environmental Ltd. (Hallex) was retained
by Mr. Tony Calzetta to conduct a Phase 2 Environmental Site Assessment (ESA) of 179 East Main
Street, Welland, ON (the site) following the completion of a Phase 1 ESA report that documented an Area
of Potential Environmental Concern (Hallex, July, 2015), being a former dry cleaner at the west adjacent
site.
This Phase 2 ESA conceptual site model provides an overview of the work completed to date as well as
representative characterization of soil and groundwater conditions for the RSC property. This RSC filing
is being completed for the purpose of changing land use from a less sensitive (commercial) back to a
more sensitive (residential) land use. The RSC property location and reference plan are located on Figures
1 and 2. The chronology of Environmental Site Assessment work at the RSC property includes:

• November 2015: Phase 2 ESA

2.0 REPORT ORGANIZATION
The phase 2 conceptual site model (CSM) is organized to follow the requirements as set out in O.Reg
153/04 Part IV, Schedule E section 43(2) to 43(9), as amended by O.Reg 511/09. The CSM is compiled
as per the Table of Contents.

3.0 CURRENT CONDITION OF THE PROPERTY OR, WHERE REMEDIAL ACTIONS HAVE BEEN UNDERTAKEN, THE CONDITION OF THE PHASE TWO PROPERTY BEFORE THE REMEDIAL ACTIONS WERE UNDERTAKEN
The current condition of the property is the former location of community use building: Hope Centre.
The majority of the study site is occupied by the building footprint with parking at the rear. The Phase 1
ESA activities identified one (1) Potentially Contaminating Activity (PCA) that designated the RSC
property with one (1) Area of Potential Environmental Concern. The Phase 2 ESA results of soil and
groundwater sampling did not reveal the presence of any method group target contaminants in exceedance
of the applicable Site Condition Standards.

4.0 CSM FORMAT
As required, the conceptual site model consists of diagrams, cross-sections and figures as well as text and
tables presenting site history, environmental site assessment activities and results, and remediation efforts
and results. The information contained herein is obtained from site investigative and assessment work for
the RSC property from June-November 2015.

5.0 AREAS OF POTENTIAL ENVIRONMENTAL CONCERN
5.1 Description of Each Area

additional PCA’s or APEC’s relating to previous site use were noted.

5.1.1 APEC-1: Operation of Dry Cleaning Equipment (where chemicals are used)
The off-site up-gradient operation of a former Dry Cleaning business along the east adjacent property
boundary had the potential to impact the soil and groundwater. Contaminants of concern include Volatile
Organic Compounds (VOC’s) as chlorinated solvents. The location of the former dry cleaning business
presented an off-site PCA-1 and on-site APEC-1.

6.0 PHYSICAL SETTING
6.1 Topography
Review of topographic map # 30M/3 and 30M/6, Google Earth data, and site survey information
indicated the site sloped north, northwest. The area of the site is generally flat lying with a slope of
approximately 1% northwest. The approximate mean elevation of the site is 180 masl. Figure 5 details
topographic contours of the study site.
6.2 Hydrogeological Characteristics
Surface water drainage on-site was via runoff into the municipal storm sewer system. Site
hydrogeological conditions were identified regarding the unconfined, ‘water table’ regime via monitoring
wells MW-1, MW-2 and MW-3. Depths to groundwater were measured at 3.6-3.7 metres below ground
surface. The groundwater flow direction was calculated from survey data as northwest, with hydraulic
gradient of i = 0.01 based on the groundwater elevation survey.
6.3 Geology
Review of map: Quaternary Geology of Niagara-Welland (Ontario Geological Survey Map 2556),
indicated that the native overburden underlying the subject site was: Glaciolacustrine basin and quiet
water deposits of Silt & Clay as confirmed on site during borehole drilling, to depths exceeding 6 metres.
Depth to groundwater was measured at 3.6 metres below ground surfaces. Figure 6 provides N-S and E-
W cross sections of the study site.
6.4 Approximate Depth to Bedrock
Depth to bedrock at the RSC property was greater than 6 metres and not encountered during site
investigation. Regional well records indicated approximate depth to bedrock ranged from 36-42 m bgs.
6.5 Any Respect in which section 41 or 43.1 of the Regulation Apply
The RSC property is not located within what O.Reg 153/04 section 41 and 43.1 constitute as an
environmentally sensitive area or “if the property includes or is adjacent to a water body or includes land
that is within 30 metres of a water body”. The overburden at the RSC property is greater than 2.0 m in
depth and therefore does not constitute what is called a “shallow soil property”. Additionally, the soil pH
at the property fell within the acceptable range of 5 to 9, with Hallex’s samples indicating a pH of 6.94.
6.6 Areas Where Soil has been brought from another property
No soil has been brought to the RSC property originating from another property, during the ESA process.
6.7 Locations of Proposed Buildings or other structures
The location of the building will remain the same (see Figure 1). No proposed renovations to the existing
building structure other than interior renovations exist.

7.0 CONTAMINANTS
7.1 Each area where contaminants exceed applicable site condition standards
Following the Phase 2 soil and groundwater ESA investigation it was confirmed the APEC area
(identified above in Sections 3.0 and 5.1) did not exhibit any exceedance for contaminants of concern:

7.2 Each Medium in Which a Contaminant is Present
No contaminants were identified in the mediums soil and/or groundwater.
7.3 Vapour intrusion of contaminants into buildings
Given the current conditions of the site, the potential for vapour intrusion into planned structures is
negligible, as VOC’s were not encountered during field screening, nor in soil and groundwater samples.
7.4 Subsurface Structure or Utilities that may affect Contaminant Distribution and Transport
No target contaminants were identified therefore the potential for underground utilities to affect
contaminant distribution and transport was negligible.

8.0 RECEPTOR EXPOSURE TO CONTAMINANTS
Since no contaminants were identified, there did not exist a potential for exposure to human and/or
ecological receptors.

9.0 CONCLUSIONS
The Phase 1 & 2 ESA investigations at the RSC property revealed one (1) PCA resulting in one (1) APEC
on the RSC property. Contaminants of Concern in soil and groundwater were assessed, with analytical
results indicating all MOE criteria was met for target contaminant VOC’s. Hallex therefore concludes
that the soil and groundwater site conditions satisfy MOE SCS Table 2 Residential Land Use criteria
(2011). The subject site is considered suitable for change to residential land use, within the definition of
O. Reg. 153/04 and 511/09 as of November 16th, 2015. No further environmental site assessment work is
considered necessary and the site conditions justify filing a record of site condition for residential land
use.
