Opportunity to Comment on Order Sets Used for Cerner Build Closes May 14

Nearly 200 Computerized Provider Order Entry (CPOE) order sets are available on CHIOneCare.net for review and comment by CHI clinicians. May 14 is the deadline to provide feedback and engage in discussion on the versions of the order sets that will be deployed for electronic health record go-lives in 2013. View order sets open for comment.

Clinicians are encouraged to revisit order sets they have commented on to see additional comments and discussion as the order sets are updated and re-posted. Several anesthesia order sets were recently re-posted to the website.

If you have not yet visited CHIOneCare.net, register now to review and comment on order sets. If you have already registered, you can support physician participation in Kentucky by encouraging colleagues to register and participate in order set reviews.

Clinicians and other representatives have been specifically designated to answer questions about the order set process. Watch for posters with commenting deadline information throughout KentuckyOne Health facilities.

KentuckyOne Health Forms Regional Clinical Council

The newly established Regional Clinical Council (RCC) for KentuckyOne Health is the lead regional oversight body of the OneCare program in Kentucky. Comprised of clinical and operational leadership from across KentuckyOne Health, the RCC is a decision-making body that provides direction and leadership to the Clinical Advisory Group, Physician Advisory Group and project steering committees. Collectively, the group will advise senior KentuckyOne Health leadership and serve as liaison between the region and CHI national clinical and operating committees, including the OneCare Executive Steering Committee, to escalate issues and ensure standardization in Kentucky.

Bev Weber, KentuckyOne Health senior vice president and chief operating officer, will chair the committee with co-chairs from the regional OneCare core team. KentuckyOne Health RCC members include; Gary Ermers, Ron Farr, Cheryl Fugatte, Dr. James Ketterhagen, Dr. Mike Marvin, Chris Mays, Eric Miller, Debbie Molnar, Dale Ragone, Chris Schweighardt, Ronnie Simpson, Sherry Tichenor, Dr. Dan Varga and Dorothy Zimmerman. Additionally, Dr. Tod Russell, Brian Norris, Tom Wittman, Teri Gaskin and Elaine Polinak from the regional OneCare core team will be represented.

Additional clinical representatives will be added at discretion of the RCC.

A kick-off meeting is scheduled for May 21 at the Heart and Lung Center in Louisville. Going forward, the RCC will meet at least monthly, alternating locations as appropriate. Back to top

National Meaningful Use Objectives and Measures Approved for Ambulatory Electronic Health Record

CHI’s Employed Physician Integration Council (EPIC), with guidance from CHI’s Ambulatory Quality Workgroup and Clinical Project Team, has approved the Stage 1 Meaningful Use measures that will be used by CHI physician practices. The Centers for Medicare & Medicaid Services (CMS) measures for eligible professionals include 15 core objectives and five additional objectives that can be selected from a menu of 10 objectives. CMS also has six clinical quality measures, including three that can be selected from a menu of 38.

To provide consistent measurement and clarify the process for CHI practices, EPIC’s decision standardizes the five menu-set objectives all practices will use in Stage 1. The decision also standardizes the clinical quality measures for CHI practices based upon whether a practice is in OB/GYN, pediatrics or family practice/internal medicine. The Meaningful Use objectives and measures will be built into the national Ambulatory Electronic Health Record (AEHR) system. (In Des Moines, Iowa, one of the five menu-set objectives will be different because the AEHR is already live.)

Eligible professionals will not capture or report data on Meaningful Use objectives and measures (attest to CMS) until after their AEHR go-live.

The Latest from the Clinical Advisory Group

The OneCare Clinical Advisory Group (CAG) is responsible for broad decision making and ensuring the alignment of clinical standards across KentuckyOne Health clinical technology. During the April 26 CAG meeting, members reviewed and discussed a number of decision briefs and voted to make the following electronic health record build recommendations:

Moderation Sedation: Incremental doses should be documented in IView to alleviate the current process of nurses toggling between screens to document doses in two places in the system.

APRN Message Center Access: When entering orders, an ARPN should be listed as the ordering “physician” and will be responsible for forwarding the order to the inbox of the appropriate physician for co-signature.

Telephone Admission Orders: Nurses should be allowed to convert home medications into orders.

Patch Removal: To eliminate the current dual process of having continuous patches documented on the eMAR while interval patches are listed on the task sheet, the group recommended documenting all patch removal on the eMAR.

In addition, CAG members were updated on the interim change control process currently being followed. All change requests are now being placed in folders on MethodM as guided by the design team leads who then notify the regional and national informatics teams for review. These change requests will be finalized in the coming weeks. A long-term change management strategy is being determined and will be communicated when finalized.

Elaine Polinak Named Organizational Effectiveness Consultant

KentuckyOne Health welcomes Elaine Polinak, RN, MSN, to the Cerner project team. In her new role as organizational effectiveness consultant, Kathy will be focus on various Cerner adoption and benefits activities and support the Clinical Advisory Group, Physician Advisory Group and Regional Clinical Council.

Elaine served as a clinical nurse specialist in acute and critical care environments, and worked for Cerner as a healthcare executive and with various organizations with Dearborn Consulting. Elaine has experience implementing electronic health records, building education programs, and developing CPOE and provider documentation.

Elaine will be a direct link to CHI’s national Organizational Effectiveness team and will work closely with Tod Russell, MD, KentuckyOne Health regional chief medical informatics officer, and Brian Norris, RN, vice president and regional chief nursing informatics officer. She will also partner with KentuckyOne Health informaticists on the Cerner project. Back to top
