Welcome to the AAO Winter Conference, “Adult Treatment: A Clinical Conference for Orthodontists and Restorative Dentists.” AAO members and restorative dentists alike will benefit from this exploratative meeting of the specialties which will focus on how collaboration can produce outstanding clinical results.

The team aspect of the event is just the beginning, as there will be a compre-

hensive set of presentations from recognized clinical experts, covering topics

ranging from new treatments in adult esthetics to sleep apnea to breakthrough

orthodontic and restorative materials as well as computer assisted diagnostics

and treatment.

You will hear perspectives about the clinical and medico-legal risks inherent in

adult treatment, and an update on the most current considerations in the man-

agement of TMD. Methodologies to change the speed and quality of treatment

through changes in patient physiology will be discussed with focus on both ac-

celerated osteogenic orthodontics and vibratory stimulation of tooth movement.

This will undoubtedly prove to be a cutting edge practical program designed to

be shared with our restorative dental colleagues. As we see increasing numbers

of adults requesting orthodontic treatment, the information is timely and im-

mediately applicable for your practice.

Conference Objectives

The primary objective of this conference is to provide a shared learning experience between orthodontists and restorative dentists for mutual success in addressing the dental care needs of adult patients. The program will:

• Emphasize the importance of dental professional teamwork in managing adults;

• Inform orthodontists and their restorative colleagues about new materials, technologies and treatment options for adults;

• Illustrate and discuss the importance of establishing realistic treatment objectives for adults;

• Describe appropriate methods of reducing the risk of adverse results when treating adults;

• Identify controversial topics in adult treatment and provide Point-Counterpoint debates between experts on these topics;

• Provide a forum for discussion between the audience and speakers.

This meeting is respectfully dedicated to Dr. Kokich in memory of his many contributions to dentistry and specifically to interdisciplinary treatment as featured in this program.

Dr. Vince Kokich served as the AAO’s Winter Conference Co-chair and worked to develop the scientific program prior to his untimely passing.

Adult patients with multiple missing teeth present challenges for the Interdisciplinary Team. These patients often have malocclu-sions with tooth size and/or site discrepancies, deficient alveolar ridges, gingival recession and periodontal disease. Therefore, it is imperative that an appropriate sequence of care is prepared for each of the team members to follow in order to achieve the desired outcome. This presentation will discuss protocols utilized by Drs. Kokich, Janakievski and Kinzer to optimize the results for our patients.

Apart from mainly esthetic advantages, CCLAs have proven to deliver reliable and individual treatment results in the hands of a well-trained practitio-ner. Because of the favorable caries preventive effect of brackets



bonded on the lingual surfaces of the teeth, CCLAs should also be considered in comprehensive treatment of children and adolescents.

Point/Counterpoint: Treating Adults with Sleep Apnea

11:30 AM - 12:00 PMPoint: The Case for Oral Appliances in the Treatment of Sleep Disordered Breathing

Orthodontists, with their knowledge and training of functional appliances and established skills to evaluate jaw position, are ideally suited to provide oral appliance therapy in this field.

Oral appliances have a direct effect on tongue posture during sleep and help to stabilize the mandible in a closed vertical position. These appliances are commonly regarded as a simple, silent, bed partner-friendly, less invasive, reversible, tolerable and efficacious choice and have been demonstrated to have a beneficial impact on a number of essential medical outcomes, including polysomno-graphic data, subjective and objective measurements of sleepiness, cardiovascular functions, neuropsychological behaviours and quality of life indices. Several factors appear to contribute to the effectiveness of oral appliancess – appliance design and titration, the severity of the sleep apnea, the amount of advancement, the vertical opening of the mandible, positional sleep apnea and the Body Mass Index (BMI). Oral appliances appear to work by enlarg-ing the upper airway patency at multiple levels and by improving muscle airway tone and thus decreasing upper airway collapsibility.

alan lowe is a professor and chair in the Divi-sionofOrthodonticsattheUniversityofBritishColumbia and has maintained a private orthodon-tic specialty practice in Vancouver, Canada for 35 years.Hehaslecturedextensivelybothnationallyand internationally in the areas of neurophysiol-ogy and orthodontics with a particular emphasis

ontheetiologyandtreatmentofsnoringandObstructiveSleepApnea.Hisresearch activities have been funded by both provincial and federal gov-ernmentsandhehaspublishedover120papers,150abstractsand15bookchapters.HehassuccessfullyfiledCanadian,U.S.andinternationalpatents for each of three inventions. Dr. Lowe was a founding member of the American Academy of Dental Sleep Medicine and has served many years as a member of their Board of Directors, a research advisor to the Board and chair of their Awards Committee.

Effective communication and collaboration between members of the interdisciplinary dental team is essential if you are to accom-plish the goals you have established for treatment. It is therefore crucial that each team member understand what modalities other team members use to accomplish various outcomes. For example, the orthodontist on the team should have basic knowledge of what options are available to the restorative dentist to meet the func-tional and esthetic goals of the patient, and the restorative dentist should understand what orthodontic options exist. Towards that end, this presentation will discuss various materials for anterior and posterior tooth and implant restorations, to improve orthodon-tists understanding of the strengths and limitations of each.

The use of implants for anchorage in orthodontics has changed the way orthodontic treatment is planned and carried out – essentially a paradigm shift in managing the correction of dental malocclu-sions. Implants can provide anchorage to correct malocclusions in patients with debilitated dentitions that have compromised toothborne anchorage and/or require tooth movement that is not possible with conventional orthodontic mechanotherapy. These dentitions are typically characterized by having certain skeletal disparities that may have required orthognathic surgery in the past, multiple missing teeth with inappropriate spacing to facili-tate prosthodontic treatment, periodontally compromised teeth, overerupted teeth, and/or lack of posterior support to maintain occlusal vertical dimension. Implants for orthodontic anchorage include two basic types of implants, those that are integrated with the surrounding bone and those that are not. Bone- or osseoin-tegrated implants typically include cylindrically shaped implants thatarenormallyusedtoreplacemissingteeth.Non-integratedimplants, commonly known as temporary anchorage devices, typi-cally include screws or screw-retained plates. This presentation will focus on the use of osseointegrated prosthodontic-type implants. The indications, use, advantages and disadvantages of these implants will be presented in some detail.

TMD is a multifactorial pathology, and it is difficult to demonstrate a direct correlation between one of the causes, such as occlusion, and TMD. The variables are so many and so mixed that, nowadays, we do not have adequate diagnostic instruments to establish a clear correlation or to know if how and when a malocclusion can unbalance the stomatognathic system. Based on this concept, orofacial pain and TMD require a comprehensive team approach. Treatments should address not only the physical diagnosis, but also the psychological distress. When the treatment protocol in-cludes a dental intervention, this must be done to address patient discomfort and obtain an occlusion that is stable, the treatment goals being to reduce pain and to improve function.

Evidence-based data on two control groups and 47 adults who were treated with non-surgical expansion will be presented to demonstrate the effi-cacy and long-term stability of the correction. Possible unfavorable side effects were also examined. Treated cases will demonstrate the feasibility of this non-invasive and least costly approach to adult maxillary expansion.

With tooth loss it occurs through resorption of the alveolar bone, whereas, in areas absent of tooth development the alveolus fails to adequately develop. Enhancing or developing alveolar ridges suitable for prosthodontic purposes can be established surgically through various distraction and/or grafting procedures and non-surgically through tooth movement.
