Open Enrollment is your opportunity to make changes to your current benefits without a qualifying
IRS event. This year’s open enrollment is passive which means if you do not make any changes to
your current benefits, they will roll-over to the next calendar year (with the exception of Flexible
Spending Accounts).

FLORIDA BLUE HEALTH INSURANCE
Effective January 1, 2018, the PPO and HMO premiums will increase an average of 4.88%. The plan designs will
remain the same and continue to be compliant with the Affordable Care Act, providing preventative care
in-network at no cost. Please refer to the Open Enrollment webpages at www.palmbeachstate.edu for plan a
comparison.
ENROLLING AND MAKING MEDICAL INSURANCE CHANGES
All eligible employees who are making changes or adding medical will be using the FBMC Benefits Management
Center. If you are not currently participating in the HMO plan and are selecting it for 2018 or are switching to the
HMO, you will need to choose a primary care physician for you and your dependents at www.floridablue.com
under the “BlueCare” network. You will need to provide the National Provider ID# (10-digit number) of your
primary care physician in order to complete your enrollment. Children may be covered until the age of 26. In
addition Florida law allows children to be covered until age 30 if they are unmarried, have no children of their
own, reside in the State of Florida or are an out-of-state student. Dependent enrollment is pending until
documentation is verified.
In Florida, employees who are unable to afford health care premiums may be eligible for assistance through Medicaid or

All Open Enrollment changes must be
completed online by October 31, 2017.
Full-time employees may elect group dental insurance and choose either DentalCare USA FL48N (DHMO) or Delta
Dental PPO. When choosing a dental plan, you want benefits that fit the needs of you and your family. DentalCare
USA FL48N (DHMO) and Delta Dental PPO both offer comprehensive dental coverage. Dependent children will be
covered to age 26.
Delta PPO Plan - The employee payment is based on a "percentage" for covered services; the maximum the plan
will pay for services in the calendar year is $1,000. An annual deductible is applied to services beyond diagnostic
and preventative care and there is no orthodontic benefit.
DeltaCare USA (Plan number FL48N) - Almost all services require a copayment even though some are very
inexpensive. There is no cap on services provided for the year and there is an orthodontic benefit. A dentist must
be selected from the Delta Care USA network by visiting deltadentalins.com. You must have a provider ID# for your
dentist of choice in order to complete your enrollment.

PERSONAL ELECTRONIC DEVICE INSURANCE
Provides coverage for computers, laptops, smart phones, tablets, netbooks, e-Readers and printers for loss due to
theft or accidental damage and includes Identity Theft Expense Reimbursement Protection. It is available for
purchase at: www.collegestudentinsurance.com/fl through Arthur J. Gallagher & Co. Annual premium is $90 per
policy plus a $5 processing fee.
SICK LEAVE POOL
Open Enrollment allows eligible full-time employees to apply for membership in the sick leave pool. New members
can enroll by completing the enrollment form provided in your packet. Completed forms must be submitted to the
Office of Human Resources, MS# 10 by October 31, 2017.
The Sick Leave Pool is intended to be used by eligible employees to assist in covering the gap between the
exhaustion of all accumulated paid leave and the 90 day waiting period until long-term disability insurance
benefits may become effective. You must be a full-time employee, employed with the college for one (1) year and
have accumulated 40 hours of unused sick leave after your donation to the Pool. Participation in the Pool is
voluntary. Sick leave requested from the Pool must be used for the member’s own personal illness, accident or
injury. Sick leave may not be withdrawn for the purpose of childcare or the care of other family members.
Participants who are eligible for payment under the College’s Worker’s Compensation or Disability Plans are not
eligible to receive sick leave from the Pool. Completed forms must be submitted to the Office of Human
Resources, MS 10 by October 31, 2017.
VSP Vision plan rates will not change for 2018. Visit a VSP provider to get the most out of your benefit and have lower
out-of-pocket costs. To find a VSP doctor , visit vsp.com or call (800) 877-7195. Dependent children will be covered
until age 26.

FLEXIBLE SPENDING ACCOUNT
Sometimes referred to as a cafeteria plan, flex plan, or a Section
125 plan, a Flexible Spending Account lets you set aside a certain
amount of your paycheck into an account before paying income
taxes. During the year, you have access to this account for
reimbursement of expenses for which you regularly pay, such as
healthcare and dependent daycare. When you use tax-free
dollars to pay for these expenses, you realize an increase in your
spending power and substantial tax savings.
Current participants must select their annual amount for 2018
through www.myflexonline.com. Please refer to the “FSA Online
Enrollment Instructions” in your Open Enrollment packet for
details on how to complete the process. If you no longer want an
active debit card, you must notify Custom Benefit Services in
writing by December 31, 2017.
New enrollees must use enclosed form which must be returned
by October 31, 2017. Debit cards will be available to those who
select and pay a minimal fee and can be ordered online after
open enrollment has ended. Flex debit cards still require receipts
if not a standard physician co-pay or pharmacy only charge. The 2018 annual maximum for medical flexible
spending is $2,600 and the annual maximum for dependent care flexible spending is $5,000.
LONG TERM CARE INSURANCE
Provided by Northwestern Mutual. Long term care insurance provides protection against the financial impact
of needing care and the freedom to arrange for care when it’s needed and where it’s needed. Jon Maples,
Financial Representative at Northwestern Mutual will meet with interested employees individually to choose a

Employees receive a 5% discounted group rate through the College.
The Benefits Fair is your opportunity to stop and visit with insurance carriers, vendors and your benefits team.
Those attending include representatives from our 403(b) providers, Area Agency on Aging - Medicare, Mental
Health Association of Palm Beach County, Tri-Rail commuter services, Fresh Meal Plan, Custom Benefit Services
and more. The information provided will also aid you in your selections of benefits during the Annual Benefits
Open Enrollment.

Benefits Fair and Health Assessments
Earn points towards the Blue Rewards Program for participating
in the Health Assessments. In addition, help the College qualify for
a monetary incentive based on the percentage of participation of
full-time benefit-eligible employees in the onsite Health
Assessment. The incentives are as follows:
35-49% participation of full-time benefit eligible employees= $ 5,000.00
50-59% participation of full-time benefit eligible employees= $10,000.00
60+% participation of full-time benefit eligible employees= $15,000.00
Palm Beach State provides an online benefits enrollment system through FBMC's Benefits
Management Center (BMC). The BMC provides benefit eligible employees the ability to make
benefit selections and changes online during open enrollment. The BMC portal is a secure
website and gateway to your personal benefits information.
The system is available through the web 24 hours a day, 7 days a week for your
convenience. It provides information about your benefit election options, premiums and plan
information so that you can make informed decisions.

If this is your first visit, you must register and create a username and password.
Review current benefit options and make any elections or changes.
Print out your enrollment confirmation containing all of your benefit elections including your life insurance
beneficiary designations.

HOW DO I ENROLL MY ELIGIBLE DEPENDENTS?
You must provide your dependent’s full
legal name, date of birth, and full Social
Security Number. Please be sure you
have this information and documentation
readily available when you enroll. The
Affordable Care Act requires all
employers to report social security
numbers to the IRS for all employees/
dependents offered medical insurance.
New dependents will not be added to
plans without the required eligibility
documentation.
ACCESSING THE FBMC BENEFITS MANAGEMENT SYSTEM
DEPENDENT VERIFICATION
If adding a dependent to your health plan for the first time, you
will be required to submit verification of their eligibility during
the enrollment process. Upload your documents during your
online enrollment process. Documentation such as a marriage
certificate, birth certificate, adoption record, legal guardianship/
custody document or court order making you the legal guardian
of a child must be uploaded during the enrollment process. If
adding a spouse or domestic partner, two documents are
required. A complete listing of acceptable supporting
documentation is provided to you during the enrollment process
online at https://bmc.myfbmc.com.
403B VOLUNTARY SAVINGS PLANS
Take advantage of the college’s voluntary savings plans — 403(b). These plans can help you secure a financially-stable
retirement. All employees are eligible to participate in the 403b plans immediately upon employment. Participants are
fully vested in their contributions and earnings at all times.
Open enrollment is an excellent time to review your retirement savings and consider enrolling in a voluntary program or
increasing your current contribution. You may enroll or change your contributions at anytime of the year. The 403(b)
change form can be found on the Human Resources website.
