The official evaluation must be completed on an original FWPE form from AOTA. CPCC provides this original form, and it is the student's responsibility to deliver it to his or her Fieldwork Educator. This SAMPLE document is intended as a guiding and learning tool.

A score of "2" in this section may be grounds for a fieldwork failure. It is important that the student achieves at least a "3" on ETHICS and SAFETY by the end of the 8-week period.
