PRIORITIZATION AND JUSTIFICATION NURSING DIAGNOSIS ACTUAL RISK JUSTIFICATION 1. Anxiety related to upcoming surgery secondary to stage 1 breast cancer Fear of the unknown or also called anxiety is a vital part in taking care of the patient and this problem can be seen in Maslow’s hierarchy which is in the 2 nd part which is the safety and security having the absence of safety and security will impede the patient in attaining self-actualization but considering this will only be categorized as not the main problem but will appear in the later. 2. Risk for infection related to ongoing disease process Is prioritized as the second highest problem. According to Maslow’s hierarchy of needs infection falls under safety and security, having the absence of safety and security will impede the patient in attaining self- actualization but considering this will only be categorized as not the main problem but will appear in the later. Though it is a risk it is given a highest prioritization since the patient will undergo surgery and infection is always present. 3. Acute pain related to ongoing disease process Pain is considered as the fifth vital sign and can be a main factor affecting various problems which can be linked to activity since the patient rated the pain as mild pain this was considered as a problem that can be solved in the later part of patient care. 4. Fatigue related to ongoing disease process Fatigue is caused by exhaustion of the body or body weakness and can be seen in the 2 nd level of Maslow’s hierarchy of need that is having no fulfillment of the physiologic need which is in the activity category. 5. Self-care deficit related to body weakness We also need to address this problem in order to perform her usual activities of daily living. Activity is also a physiologic need that we need to provide.

Knowledge is categorized as one of the highest needs of man and is at the top of Maslow’s Hierarchy of needs that enables the individual to reach self-actualization, thus, is considered as the least prioritized problem
