These measures have led to a big jump in the numbers of medical students graduating, which continue to increase by about 300 medical graduates per year, according to the AMSA president Ross Roberts-Thomson. However, hospital internship places have failed to keep pace with the rise in numbers of medical students. Last year, there were about 2280 medical graduates but only 2243 internships available, so about 40 graduates of Australian medical schools are yet to be offered hospital internships. And the trend is continuing. IF you’re saving for retirement, you’ll almost certainly think of superannuation — it is often the preferred way to save for retirement because of its tax advantages. For a complying super fund, the income earned from the fund is concessionally taxed and there are further tax advantages when the money is eventually taken after retirement.

For many doctors, that means joining either a not-for-profi t industry fund, or a retail fund run by one of the large fi nancial institutions.

But there’s another alternative — one that offers all the tax advantages of a larger fund, but with more fl exibility; and that’s for the doctor to own and run a “self-managed super fund”, or SMSF. There are now over 428,000 SMSFs in Australia, with more than 2000 new ones being established each month.

In many respects, an SMSF is the same as a large industry or retail fund — a trust fund run by trustees which pools members’ contributions and invests them in cash, bonds, shares, property and managed funds.

But unlike a large fund, which may have many thousands of members, an SMSF has only one to four members, who are usually family members or business associates (most have two members; for example, the doctor and his or her partner). The SMSF has its own trust deed and trustees (who are usually the members). The trustees run the fund, collect contributions and make decisions about the assets — what to buy, hold or sell — usually with the assistance of fund

administrators, accountants and fi nancial advisers, says David O’Callaghan, a Certifi ed Financial Planner and senior adviser with Fintuition Pty Limited.

The big plus of the SMSF is the broader range of investment choices available when compared to an industry or retail super fund.

Of particular interest to doctors is that the fund can own the practice premises and the doctor can lease it from the fund, which can be a very tax-effective way of owning the

premises.Normally, an SMSF can’t purchase

property from a member. But there’s an exemption for business real property; that is, the premises out of which one or more businesses — such as a medical practice — operate.

Here’s how it works. Using the business real property exemption, a doctor can sell his or her practice premises to their fund (it must be at a commercial rate). The doctor then enters into a lease agreement with the fund to rent the rooms. (Again, there must be a lease agreement and it must be leased at a commercial rate, or severe penalties apply.)

Also, bear in mind that when the doctor sells the practice premises to the fund, capital gains tax may be payable.

The purchase must also be allowed by the fund’s written investment strategy which all SMSFs are obliged to have. This will usually stipulate that there must be diversity and liquidity in the fund’s assets, but this doesn’t necessarily preclude an SMSF holding one major asset such as the practice premises.

Recent changes to the Superannuation Industry Supervision Act, introduced over the past 12 months, mean that it’s now possible for the fund to borrow to fund the purchase of assets such as the practice

premises. SMSFs may now enter into borrowing on a limited-recourse basis, provided certain conditions are met.

So where previously the fund had to have the cash to pay for the purchase of the premises, it can now borrow to fund the purchase.

The premises are placed in a separate “bare” trust with the SMSF as benefi cial owner, until such time as the amount owing is paid off — from members’ contributions and rental income. The property is then transferred into the fund.

Other benefi ts are tax and asset protection.

The fund pays tax on earnings at a maximum rate of 15 per cent; that is, the rental income the fund receives from the doctor is taxed at 15 per cent. And remember that the doctor generally gets a tax deduction for the amount paid in rent to the fund, usually at a higher rate. So the whole arrangement is quite tax effective.

On retirement, if the self-managed fund is converted to a pension fund, the practice premises can then potentially be sold free of tax, O’Callaghan says.

Also, under the Bankruptcy Act, assets held by a super fund have a degree of protection from creditors in case of fi nancial pressure — from being sued, for example.

An SMSF typically costs between $400 and $600 to set up and between $2000 and $5000 a year to run. To be economically viable, the fund generally needs to have combined assets of about $200,000 or more.

But it’s important to get advice from a professional adviser experienced in this area before entering into an arrangement of this type as the rules are complex and penalties for breaching them severe.

What aspect of this job appealed to you most strongly when you applied or were appointed? And now?

What I like about epidemiology is that it lets you get to the bottom of things, to work out what’s really going on. Instead of simply believing the results of a study, you can make a judgement about whether the claims are valid and have a good chance of being right. Sometimes you come to very unexpected conclusions. My career has been a constant fl ux between epidemiology and journalism; much more in medicine, but I’ve never been able to completely walk away from reporting either. They have a surprising amount in common.

Career highlights? Any lowlights/disappointments you’d like to share?

A couple of highlights. Seeing evidence-based medicine fi rmly established in medical curricula. Now it’s almost mainstream. That was hard to imagine 15 years ago.Winning Eureka prizes for my radio programs on ABC Radio National’s The Health Report (for the series “Facing the Evidence” http://www.abc.net.au/rn/healthreport/stories/2006/1735075.htm and “Cancer Screening” http://www.abc.net.au/rn/healthreport/stories/2005/1440410.htm).

What have been the biggest surprises in your working life?

Evidence-based medicine (EBM) has resulted in some major upsets in clinical practice, including some big surprises. For example, who would have thought that the Women’s Health Initiative trial would show so many downsides — deep vein thrombosis, stroke, breast cancer, maybe heart disease — of hormone therapy? I think that really took a lot of people by surprise. Or that the CRASH (Corticosteroid Randomisation after Signifi cant Head Injury) trial would fi nd that intravenous corticosteroids actually increased the risk of death and disability after head injury? Or that sleeping babies on their tummies would make them more likely to die from SIDS (sudden infant death syndrome)? And on and on. We’ve been misled so many times because we didn’t get good enough

evidence before implementing treatments or making public health recommendations. The one thing I’m sure of is that you must stay open to the possibility that you may be wrong.

What do you hope to be doing – or have achieved – in 5 years’ time?

Much the same — fi nding things out, thinking critically, teaching, applying evidence in practice and fi nding time to do a bit more writing and radio work.

The person/people who have had the greatest infl uence on your career — mentors, sources of inspiration or unexpected opportunities

There are so many. Professor Les Irwig, the best critical thinker I’ve ever met. He’s been a mentor and teacher for most of my professional life. Professor Gordon Guyatt, a Canadian and one of the international leaders of EBM, whose energy and advocacy for EBM are awesome. The chief of staff at the Sydney Morning Herald, where I did my cadetship as a reporter, who taught me to write for a deadline — fast, short and accurate, and Norman Swan, who taught me how to do radio as well as print journalism. Dr Christine Read and all the other wonderful doctors at Family Planning who helped me work in clinical medicine again.

What interests do you have outside medicine?

Other than my family, I’ve been a bit hopeless at commitment to things outside of work. So I’m learning salsa dancing now, I love the energy of it, and I can do a whole lot of other things a bit. I can sail, play soccer, ski … but none of them very well. Basically, I just like nature and being in it — I don’t really care what I’m doing. I’m appalling at housekeeping, cooking and gardening. My mother taught me badly (or well, depending on how you look at it).

What are you reading now, for business and pleasure?

Vamps and tramps by Camille Paglia (there’s a brave, thoughtful and inspiring woman).I, the divine: a novel in fi rst chapters by Rabih Alameddine (beautiful to read).

If I could change one thing in the world of medicine it would be …

To stop public funding of medical services that aren’t effective, safe and cost-effective. I don’t think we should increase the health budget at the expense of education, environment, infrastructure etc. That means we need to do much better at using health care dollars wisely and fairly.

will be needed, the summit heard. And while the government has signifi cantly expanded prevocational and vocational GP training positions — doubling the number of general practitioner training places from 600 to 1200 by 2014 for example) this will not be enough to address the expected shortfall. Hence Australia faces a widespread doctor shortage over the next 15 years.

More resources and a coordinated approach

There’s an urgent need for the government to inject more funding into hospitals, so that all graduates are guaranteed an internship, the summit concluded in a combined position statement. Currently enrolled international full fee-paying medical students must be provided with internships. Both prevocational (intern) and vocational (specialist) training places must be increased to match medical graduate numbers. There should be no new medical schools or places in medical schools offered until there are enough training posts and clinical supervisors

to provide adequate training for those already enrolled.

Federal and state or territory governments must work together with all the medical bodies and stakeholders to create a framework to plan ahead and coordinate the training of medical graduates to better match the needs of the Australian population.

This means getting more accurate data on medical workforce needs in the years ahead. Participants called on Health Workforce Australia to establish a specifi c Medical Workforce Planning Advisory Committee, which would study community demand for medical services through to 2025. This committee would calculate the projected increases in local graduates and the demand for international graduates. The data would

then be used to help federal and state or territory health ministers to determine the numbers of undergraduate medical school places and new medical schools, and intern and specialist medical training places needed over the next 5–10 years. Other recommendations included:• reducing Australia’s reliance on international full fee-paying medical students by reviewing funding arrangements for medical schools;• allowing the states to share information on intern applications and acceptances in order to avoid the current problem of medical graduates applying for positions in multiple states (with some states having diffi culty fi lling all the intern positions on offer, while others are unable to provide enough places);• continuing to develop and fund innovative training programs, including training junior hospital doctors to do some of the teaching of interns; and• improving training programs outside of public hospitals ie in general practice, private hospital and community settings, with appropriate accountability mechanisms.

A vacancy exists in our new Emergency Department for a suitably experienced and/or qualified medical practitioner for a full-time position either at the level of Emergency Medicine Specialist, or suitably experienced Emergency Career Medical Officer. The position is full time, and involves shift work with on-site and on-call emergency medical officer support.

The Department The Emergency Department treats 17,000+ adult (85%) and paediatric (15%) patients per annum with 3,000+ admissions, and is equipped with ‘state of the art’ equipment. On-site cardiac catheterization including primary angioplasty is provided, along with a full range of diagnostic services. With the exception of neurosurgery and cardiothoracic surgery, a full range of surgical and medical specialities support the efficient functioning of the emergency department, with rostered consultant support across all craft groups.

The successful candidate will be a medical graduate with experience in medical publishing, writing and editing. Previous participation in postgraduate training, research, or a postgraduate qualifi cation is desirable, but not essential. Excellent English language skills are essential.

The Deputy Medical Editor will be involved in, and have a responsibility for, the editorial processes of the MJA, including critically reviewing submissions, facilitating the peer review process, commissioning contributions, liaising with authors and copy editors, writing for publication and making a contribution to the evolution of the Journal.

Trust Grade Senior House OfficersRotational posts for a maximum of two yearsPositions have become available due to the internal promotion of existing Trust Grade Doctors to Speciality Doctors within the Derby and Nottingham area.

These positions will offer successful candidates excellent experience and training in general orthopaedics both in elective and trauma work, with the opportunity to learn more specialist skills involving upper limb, revision joint surgery and spinal surgery.

There are eight designated laminar flow Orthopaedic theatres. All Senior House Officer level positions have the opportunity to undertake the following:

• Protected clinic time both elective and fracture clinics • Joint injections (shoulder, knee & elbow)• Attend and assist in 3-4 theatre lists per week• Following recognised supervision, basic knee/shoulder arthroscopies• Following recognised supervision, DHS, hip hemi arthroplasty

and ankle fracture fixation

The Department places considerable emphasis on teaching, both practical and theoretical with regular weekly teaching sessions. Successful candidates will have 10 days study leave allowance per annum and weekly teaching sessions.

The Department would support any Doctor wishing to undertake the AO basic trauma course or the ATLS course.

The hospital has a new state-of-the art Postgraduate training library and holds a variety of teaching and clinical audit sessions throughout the month. The Trust also holds links with Nottingham University.

The department runs a recognised firm structure, working alongside a Consultant and Registrar to maximise continuity of patient care and training of junior doctors.
