NSW FALLS PREVENTION NETWORKThe NSW Falls Prevention Network has existed since 1993.

ObjectivesThe specific objectives of the NSW Falls Prevention Network are to: 1. Support networking of people and organisations working to prevent falls and harm from falls among

older people in NSW (for example, health professionals, government, community and residential aged care providers, non-government organisations and researchers).

2. Support sharing of falls prevention resources and initiatives developed by Network members and exploration of opportunities to undertake collaborative projects and/or combine resources in joint initiatives.

3. Support access to and dissemination of information on local and international falls prevention research findings, policy and practice initiatives, tools and resources.

4. Support the communication and implementation of the NSW Health falls prevention strategies through Network activities.

5. Ensure the Network remains valued, respected, and responsive to the needs of current and potential members and key stakeholders.

6. Promote prevention of falls among older people as a key health issue.

Plenary Presentations at this forum will be filmed and a DVD produced. This will be distributed following the forum, and you will be notified by the NSW Falls Prevention Network website and email list.

PLENARY SPEAKER ABSTRACTS AND BIOGRAPHIES

PLENARY SESSION 1Pam Albany Guest Lecture - this lecture honours the late Pam Albany, a passionate advocate for accident and injury prevention in her various work roles. She was the key facilitator for promoting evidence-based practice through the NSW Falls Policy and NSW Falls Prevention Program and Network.

The 2017 Pam Albany Guest Lecturer is Associate Professor Kim Delbaere, NeuRACan fear of falling make you unstable on your feetFear of falling is common in older adults, with prevalence rates often extending those of falls themselves. Fear of falling may reflect a realistic appraisal of reduced functional abilities, however it often leads to a reduction in activity levels and community participation. Excessive fear can change how people walk - often referred to as ‘cautious gait’ - which decreases a person’s postural stability. Fear of falling is, therefore, not just a consequence of falling, but also a risk factor for future falls. Exercise can reduce this fear, however, other psychological treatments are required to achieve a lasting effect.

Associate Professor Kim Delbaere is Principal Research Scientist in the Falls, Balance and Injury Research Centre at Neuroscience Research Australia and Associate Professor, University of NSW. She has been supported by a National Health and Medical Research Council Career Development Fellowship since 2012. Her research has contributed to the understanding of physical, psychological and cognitive factors causing falls. Her multidisciplinary approach incorporates elements from physiotherapy, psychology, brain imaging and software engineering towards preventing falls and promoting healthy ageing.

Professor Stephen Lord, NeuRAFalls prevention research update Three recent studies aimed at understanding fall risk will be presented. These cover brain activation in high-functioning older adults, causes of unexplained falls, and the role of gait adaptability in fall avoidance. In addition the findings of three randomised controlled trials will be presented and discussed. These include studies that have evaluated virtual reality treadmill training, social dancing and Tai Chi in older people with mild cognitive impairment as interventions.

Professor Stephen Lord is NHMRC Senior Principal Research Fellow and Director of the Falls, Balance and Injury Research Centre at Neuroscience Research Australia. He has published over 400 papers in the areas of applied physiology, instability, falls and fractures in older people and is acknowledged as a leading international researcher in his field. His research follows two main themes: the identification of physiological risk factors for falls and the development and evaluation of falls prevention strategies. A key aspect of this research has been the design, implementation and evaluation of exercise programs for the general population of older people as well as for those identified as being at increased risk of falls, i.e. people with Parkinson’s disease, stroke, dementia and frailty. His methodology and approach to fall-risk assessment has been adopted by many researchers and clinicians across the world and he is actively engaged in initiatives aimed at implementing falls prevention evidence into policy and practice.

PLENARY SESSION 2Associate Professor Gideon Caplan, Prince of Wales HospitalDelirium and fallsDelirium is the most common diagnosis and adverse event affecting older people in hospital, with overall 25% suffering from delirium, and over 50% on some wards. Delirium causes increased mortality and nursing home placement as well as greater cognitive and physical disability. Delirium is also a trigger adverse event, leading to higher rates of falls, pressure sores, readmissions and other hospital-related problems. The higher rate of falls is a combination of delirium itself, the medications and restraints used during episodes of delirium. Multicomponent delirium interventions reduce falls by 62%, as well as having many other benefits. Therefore better care of delirium is a very important strategy to reduce falls.

Associate Professor Gideon Caplan is Director of Post Acute Care Services and Director of Geriatric Medicine at Prince of Wales Hospital and a Conjoint Associate Professor at the University of New South Wales in Sydney, Australia.

His research involves developing and rigorously testing new health services, he has been a chief investigator on 5 investigator-initiated RCTs, and the pathophysiology of delirium, where he works with a consortium of international investigators.

He is President of the Australasian Delirium Association and chaired the drafting committee of the Delirium Care Standards for the Australian Commission on Safety and Quality in Health Care.

Ms Niccola Follett, Southern NSW LHD and Mrs Dean’s daughter, Ms Christine EwinMrs Dean’s StoryMrs Dean’s story is an engaging and emotional journey of a 76 year old who suffers a delirium and a serious fall in hospital. Her daughter Christine will recount her mother’s loss of confidence and decline in the months that follow the fall. Key issues related to communication, dementia and delirium as well as family/carer involvement during a hospital admission will be highlighted.

Ms Niccola Follett is the Falls Prevention Coordinator for Southern NSW Local Health District. This position covers community, aged and acute care across nine Local Government Areas. Niccola is passionate about rural communities and their people. She has been working in the field of Public Health and Nursing since 2002.

Ms Lorraine Lovitt, Clinical Excellence CommissionNSW Falls Prevention Program update The CEC NSW Falls Prevention Program continues to develop and drive system improvements to reduce falls and harm from falls in hospitals and community. This presentation will highlight key resources and the latest program implementation for the Ministry of Health Leading Better Care - Better Value Care, Falls in Hospital initiative.

Ms Lorraine Lovitt is the Lead of the NSW Falls Prevention Program at the Clinical Excellence Commission (CEC). The CEC has a key role in building capacity for quality and safety improvement in health services in NSW and in her role Lorraine is working to support improved care for older people as they engage with NSW Health services.

Lorraine has a nursing background (registered nurse with post graduate qualifications in gerontology) with considerable experience in aged care (management, consultation and co-ordination) in both community and acute care settings and with the residential aged care sector.

Lorraine is the Board Chair for Twilight Aged Care and is a Graduate of the Australian Institute of Company Directors (AICD). Lorraine’s is motivated to ensure that people in our care are supported to continue to live their lives well and with dignity.

Ms Ingrid Hutchinson, Clinical Excellence CommissionApril Falls Competition winners presentation

This presentation will present an overview of April Falls Day®/Month activities and announce the winner of the April Falls Competition.Ms Ingrid Hutchinson is the Project Officer for the NSW Falls Prevention Program at the Clinical Excellence Commission. She co-ordinates a variety of State wide projects in Falls Prevention and supports Local Health District Falls Prevention Coordinators in NSW. Ingrid has a Bachelor of Applied Science (Nutrition) and a Post Graduate Diploma in Health Promotion.
