As a testing grant Minnesota is trying to determine: • Can we improve health and lower costs if more people are covered

by Accountable Care Organization (ACO) models?

• If we invest in data analytics, health information technology, practice facilitation, and quality improvement, can we accelerate adoption of ACO models and remove barriers to coordinated/integrated care?

• How are health outcomes and costs improved when ACOs adopt Community Care Team and Accountable Communities for Health models to support integration of health care with non-medical services, compared to those who do not adopt these models?

Overview

• Seeking proposals to plan, implement, facilitate, and evaluate a Learning Community.

• Learning communities give care providers tools to improve quality, patient experience and health outcomes, while actively engaging communities and reducing health care expenditures.

Learning Community is defined as: learning teams who have common goals or interests, share best practice knowledge, and are actively engaged in implementing transformation in a focused, structured environment with the goal to advance patient centered, coordinated, and accountable care.

Four Learning Communities Grants will be funded • Three General Learning Communities

• focused on a selected transformation topic.

• One Accountable Communities for Health (ACH) Learning Community • focused on topics specific to implementation of an ACH.
