Monthly Project Construction Report Level 1 Build Out – June 2018

Safety- CAL OSHA:

o Construction commenced with NTP, issued February 9, 2017. No safety incidents in June.

Regulatory Compliance/OSHPD (Notice of Non-Compliance):

o 3 Notices of Non-Compliance remain open, as follows:

1. Required valves to the AHU’s missing (opened 1/25/18: GC Action: agreed

solution underway)

2. The maximum allowable gap has been exceeded at 6 fire doors. (opened 6/4/18:

GC Action: procure/install approved door bottoms to close the gaps)

3. The location of the toilet paper dispenser is not compliant with ADA. (opened

6/13/18: GC Action: relocate the TPD and repair wall)

o NOTE: To date, 14 NNC’s have been issued. 11 have been successfully resolved. We

anticipate that these 3 items will be resolved in the next week.

Infection Control Issues/Status:

o none

Overall Progress: o ORs: Installation of medical equipment is complete in all ORs.

o Cath Labs: Installation of medical equipment is complete in all Cath Labs.

o Corridors: Utility installations are complete.

o The fire sprinkler system is complete.

o Test and Balance of the mechanical system is almost complete.

Construction Status Photos:

o See below. Photos of the Cath Labs and Operating Rooms have been included in the

PowerPoint and have not been included herein.

Design Issues:

o There are no current, unresolved design issues.

Construction Issues:

o Connection from the Build Out to the Hospital’s newly renovated PACU/existing ORs is

designed to occur through 3 pairs of doors that cross an expansion joint spanning between

the existing hospital and the new Heart and Vascular building. The pairs of doors are in

place, but installation of the expansion joint enclosures, part of the Hospital Infill projects’

Scope of Work, was held up at OSHPD since 7-17. Issuance of an Occupancy Certificate

may be delayed until this work is performed. On 5-22-18 the Infill Contractor presented a

plan and schedule to complete 2 of the 3 door openings by 7-13-18. The 3rd

opening, for

convenience between the new ORs and the existing ORs, will be completed by Infill during

their Phase 4 work.

o In addition to the 3 open NNC’s there are 2 open NOC’s, 1 dating back to 8/2017. Both

pertain to mechanical issues. Swinerton continues to work actively to resolve and clear

these items as they occur.

o During testing of the humidification system, leaks developed in 3 locations on the West

Corridor. Trouble-shooting has resolved one location, but leaks persist in the other 2.

Swinerton and their mechanical sub-contractor continue to work to resolve this issue.

Corrective Action Plan (when applicable):

o Design: none

o Construction: Swinerton continues their efforts to correct open NOC’s and NNC’s.

Safety Issues:

o None again this month. Sharp Safety has discontinued their regular safety walks, and will

visit the site periodically as they perform their usual Hospital rounds.
