1. Introduction MyHealth@Age has developed products and services for Mobile Safety Alarm, Prescribed Healthcare and Social Network. It makes it possible for elderly people to socialize and live a physically active life even when their health becomes fragile. This makes it possible to remain in good health several years longer. A win-win solution that improve Quality of Life for Citizens and reduce the cost for Healthcare and Welfare organizations. The project was 2010 awarded the price “eHealth to benefit the patient” by Dagens Medicin and Computer Sweden. A Swedish ICH Healthcare award.

2. Project Story Through increasing elderly population where many have chronic diseases, it is important to introduce new products and services that make it possible to remain in good physical condition and manage their own healthcare activities to a larger extent. It is also important to improve the availability to healthcare professionals and to make the communication more efficient in order for the healthcare professionals to manage increasing demand for their services. Field-trial sites was established in Boden/Luleå, Tromsø and Ulster during spring 2008. An important activity was to engage the elderly end-users and the healthcare and welfare staff. During fall 2008, the project focused on identification of the elderly participants needs. Through multi stakeholder dialogues, product and service ideas was discussed and enhanced to solve the needs. It was meaningful for everybody to share knowledge between the elderly people, the health- and welfare staff, ICT staff and researchers. The solutions was visualized and the end-users provided suggestions for improvements. By sharing knowledge this way, the solution becomes more usable and valuable. The product and service development began end of 2008 and continued until summer 2009. During this period, demonstrations was made where the elderly people could request improvements. The end-users selected the smart phone they preferred to use, medical diagnose equipment, preventive health equipment etc. The acceptance test began Q3-2009 where the elderly people and the healthcare and welfare staff made quality assurance evaluation of the products. The Prescribed Healthcare field trial began Q3-2009 followed by the Mobile Safety alarm and Social Network.

During Q4-2009 the end-users evaluated the new products and services. The requests for improvements was prioritized February 2010. The field trial continued and enhanced functionality was released Q2-2010. Since there was limited budget for enhancements, the elderly end-users enhancements was prioritized. During Q2-Q3 2010 the field trial continued with large emphasis on evaluation and result dissemination. During Q3-Q4 2010, the market analysis was intensified to evaluate if and how the results could be commercialized. The Prescribed Healthcare Product and Service have been very successful. It have caused the Healthcare organizations in Sweden and Norway engaged in MyHealth@Age to participate in the CIP project Renewing Health (see www.renewinghealth.eu). That is a project where 9 countries evaluates personal eHealth systems for patients with Type-2 Diabetes, Chronic Hearth Diseases and KOL. The Swedish field trial is based on enhanced version of the Prescribed Healthcare product. The study involve about 8.500 patients and the project budget is 14 m€. The Mobile Safety Alarm provides a high end-user value. It is however not obvious how this can be financed and commercialised (see delivery report D4.3). Dialogues have been established with the INTERREG IV A NORD Sensorband II project to develop a joint project application for a large scale European field trial. The Sensorband II project have focused on in-home based safety alarm system and it makes sense to develop a solution that combine the solutions from both these projects. Social Networks are an expanding area. The end-users felt that the market trial groups was to small to make the service really useful. It should also have been linked to relevant information and interest groups. It makes more sense through wider usage with critical mass for different interests. It is therefore worthwhile to continue to develop and evaluate it’s usage for elderly people. It might however be more useful to base this on widespread service like Face book that provides more functional-rich communication and which support both computer and mobile phone usage. The MyHealth@Age project have received award for providing most end-user value of eHealth projects in Sweden. The project results have got lot of media attention. The Healthcare and welfare sector is moving towards making the patient more in control of his/her own healthcare. That is made both by providing this kind of new products and services that makes it possible. But also through regulation changes stimulating a more competitive and patient oriented healthcare and welfare sector. The MyHealth@Age project have developed important personalized eHealth products and services that supports this new interaction between clients and the healthcare and welfare providers.

The mobile Safety Alarm makes most of the clients feel safer when they are outside their house, since the smart phone has GPS positioning. That made it possible for the clients to participate in social activities outside their homes. The alarm goes to an alarm centre

where professionals answer the alarms. That was a requirement from the clients. Since this was a test, no “real” alarms was received. Instead, the field trial participants made weekly alarms to evaluate the functionality and that the alarm centre and assistance staff could contact them rapidly and tell where they were located. That worked well. An important issue for the health organizations is that the alarm service need to be reorganized if the Mobile Safety Alarm shall be possible to offer commercially.

Prescribed healthcare service:

The clients were able to do medical checkups without visiting health care centre, which led to that the clients could be even more active and mobile. In order to make the clients self going and interested in their own health, it is important for the caregivers to communicate regularly. If the professionals fails to respond to input from the clients the rate of communication falls. This is to avoid unnecessary work for professionals and clients. Important success factors were that the clients felt closer to the professionals when the professionals were active.

Social network

If social network shall be valuable, it has to be applied on a larger scale. During the field trial, there was too few participants at each trial site to utilize this possibility. The elderly people that was used to use Face book felt also that this Social Network application lacked possibility to share pictures, links to articles etc. It is therefore more suitable to utilize a more widespread and content-rich Social Network application. If Prescribed Healthcare is introduced larger scale, Social network solutions are considered valuable to support groups of elderly people to make activities together.

The alarm centre receive alarm calls wirelessly from the alarm customer. The alarm centre staff call the alarm customer to check-out which assistance is needed. The alarm centre staff then distribute the alarm call to assistance staff smart phone. The assistance staff locate the alarm customer and provides requested support service. If the alarm customer generate a mobile safety alarm outside the healthcare and welfare organization´s region, the alarm centre call the alarm customers smartphone and check-out what has happened. If appropriate, the alarm centre staff can contact the general national emergency centre (112 in Europe) and request assistance. Since the GPS position of the alarm customer works anywhere, the alarm centre staff can inform the national emergency centre about the persons position.

Max. 1500 characters Smart phone application for elderly people (alarm customers). It provides a graphical user interface where the user can initiate an alarm by pushing a soft alarm key. It also include a fall sensor connected to the smart phone by blue tooth. If the person fall, an automatic alarm is generated by the fall sensor and transmitted to the alarm centre through the smart phone. The alarm customer can also push an alarm button on the fall sensor to generate an alarm. False alarms can be cancelled through the smart phone and the fall sensor. The alarm centre staff application generate sound and visual alarm to make the staff aware that a new alarm is received. The alarm centre staff can change alarm status to received. The alarm sound is then turned-off. The alarm centre staff can view the alarm customers position on an electronic map. If there is no GPS-coordinates available at the moment for the alarm customer position, the application display the most recent positions of the alarm customer. The application also provide digital photograph of the alarm customer and contact information to the alarm customers and his/her relatives. The alarm centre application makes it possible to dispatch the alarm to welfare staff located close to the alarm customer. When the alarm customer have accepted to assist the alarm customer, the alarm status is changed to assist. When the welfare staff have finished the assistance, the alarm status is changed to finished. The assistance staff application provides the assistance staff with information about the alarm. That includes a photograph of the alarm customer and contact data to the alarm customer and his/her relatives. A GPS map application that display the location of the alarm customer and the assistance staff position. That makes it easy for the assistance staff to find the alarm customer also when the customer is located in the forest or at other places without roads or other easily located reference points.

Purpose What is the product/service for? What main challenge does it address? Max. 500 characters. Prescribed Healthcare is a service that engage the clients more actively in their own healthcare. The service stimulate the clients to live a healthier life with physical exercises and healthier diet. It also make drug treatment more accurate. Since the healthcare professionals get access to more complete and accurate information about patients health, the possible to adopt treatment based on the patients actual situation becomes improved. Main challenges is to engage the clients that have most need for the service. Many clients are active and eager to use this service to get even better health. But many of the people that would benefit most of the service are inactive and not easy to engage. And if they begin to use this service, it is a challenge to motivate them to continue to improve their lifestyle longterm. Health coaching is key to succeed with that.

Max. 1500 characters The healthcare staff develop a health activity plan together with the patient. The healthcare staff specify goals for the upcoming time period. When the time period is finished, the healthcare staff evaluate the outcome together with the patient and revise the plan for the next time period if needed. The healthcare staff offer secure e-mail communication with the patients to manage patients health questions. If the health plan includes specific exercises or utilization of own diagnose measurements, the healthcare staff can provides instructions through video messages, digital photographs and other kind of documents that explain how the health activity shall be performed. This is done through secure mail enclosures.

Step and pulse meters stimulate and evaluate physical activity. Goals can be set regarding exercises suitable for the actual persons interest and physical ability. The exercise programs are transferred to the mobile phone. Medication list is transferred to the elderly persons mobile phone. An alarm is received when it’s time to make medication. The client report performed medication, including a time stamp. That makes it possible to follow-up medication compliance. A schedule for own diagnose measurements are transferred to the elderly persons mobile phone. Supported measurements include glucose, blood pressure, PK and 2-channel ECG. An alarm is received when it's time to do the measurement and report the data. The physical exercises and the own diagnose measurements are displayed as diagrams on the mobile phone or web-browser. The recommended values are displayed making it easy to interpret the health progress. The elderly people can easily communicate with healthcare staff by secure messaging services through the mobile phone. That improves the healthcare staff availability. The services utilize secure authentication and data transmission. It is for example only approved IP-addresses that are allowed to login to the services.

Max. 1500 characters The MH@A Social Networking Product was developed by SwarmTeams LTD., who are based in Belfast in Northern Ireland. SwarmTeams LTD. specialise in creating customised Social Networks and were able to develop a stripped back interface for MH@A. The interface was accessed through the web browser on the chosen MH@A Mobile Handset, with a simple launch and login shortcut developed to simplify access to the product. Once logged into the application, the user can see the networks they are part of, for example the Regional Field Trial cohort, or personalised family & friends. When a network is chosen, the user can see who is in the network, what messages (or discussions) are open, create a discussion or send another user a private message. As the product is web-based, it is possible for others, such as Family & Friends or Healthcare Professionals, to access the network using any web browser. The network supports both group messages and private messages, creating a discussion stream as conversations take place. Group messages can only be viewed / participated in by other members of the group the conversation was initiated in. Therefore other Field Trial users could not view the conversations a user was having with their family & friends.
