    1. Scope of Operation / Task

      Lifting / placing down of Concrete products from CPM Group Ltd of varying sizes and weights.

    PLEASE NOTE:

    This Risk Assessment and Method Statement does not eliminate the need to carry out a ‘Site Specific’assessment of the working area, and the type of lifting equipment used. The Customer is responsible for

    carrying out these assessments and must conduct a briefing with the Lorry crane driver and ALL the relevant

    personnel assigned to this task before the lift begins.

    2. Related Documentation

    (This Method Statement is to be read in conjunction with the following documents) :

      Crane / Lorry Crane user instructions and Customer Site Specific Risk Assessment and Safe System of Work.

      Any site specific instruction/information relevant to this task.

      All the relevant test documentation for work equipment and safety equipment.

    3. Pre Start Activities

    (Prior to work commencing on the Operation / Task, the following items must be ascertained):

      Lift Plan to be prepared by an Appointed / Competent person.

      Sketch Plan to be prepared showing:

    1.  Position and direction of crane

    2.  Demarcation areas for any overhead power lines

    3.  Demarcation areas for any buried cables/pipe work

    4.  Direction and rotation of crane operation

    5.  Area/s where products to be placed when off-loaded6.  Centre of radius

    7. 

    Outrigger locations8.  Max SWL at given radius

      Weight of load/s to be ascertained

      Personnel competency checks to be ascertained

      Lifting Equipment and accessory certification to be confirmed as current

      Crane servicing/maintenance schedule is current

    4. Approach / Methodology

    Please Note:  (After completion of all the above pre –  start activities, work will commence following theprocedure below. If at any point something changes which requires amendments to the following procedure,

    work must be stopped and the Methodology rewritten. Under no circumstances must work be carried on

    outside of this procedure).

    Control Measures

      Upon arrival and prior to commencing work activity, personnel (Driver/Crane operator) to report to Site

    Manager.

      The Client/Customer will cordon off the lift work area to prevent access for unauthorised entry/personnel and

    implement any extra safety control measures as defined in the Risk Assessment.

      The Lorry driver / Crane operator has the responsibility to assess the site & if they feel it is unsafe to proceed

    they will stop the work and discuss their concerns with the Site Manager

      Should any reversing be involved with this procedure the Site Manager will be responsible for providing a

     person to aid the lorry Crane operator, particularly when the reversing manoeuvre involves confined /blind

    movements.

     

    The lorry driver / Crane operator, satisfied with the condition of the working area will then set up the lorrycrane as per manufacturers instructions and within his / her training, experience and competency.

      The Lorry driver / Crane operator will raise the crane over the centre of the concrete product to be lifted

      If special lifting equipment such as gravity lock grab systems then these will be attached to the main lifting

    hook and lowered onto the product until the gravity locking device is locked and secure.

    The driver/operator will test this by slightly raising the product off the trailer bed to check on the locking

    security of the grab system. If it is secure then the lift can begin.

      Please Note:(If used) The ladder will be positioned and footed in order to allow the operative/slinger

    wearing the safety harness to attached it to the hook on the lorry mounted crane. This will allow him to

    safely insert lifting pins/lifting chains and hooks into the concrete unit.

      Satisfied the lifting attachments are secure; the operative / slinger descends partially down the ladder but will

    then release the safety harness from the hook before he reaches ground level from the ladder in order to allow

    the lift to begin.

     

    The lorry crane operator then lifts the product from the lorry bed and positions it down on the ground  –  avoiding crush zones.

      When all the selected units have been lifted and placed safely on the ground, the lorry Crane operator will

    remove the lifting tackle from the crane hook and stow away safely. The operation is now complete.

      Lorry Crane driver is then escorted from the site under instruction from the Site Manager.

      The operation is reversed when loading the vehicle.

    Unloading Flex Pipe

      When unloading Flex pipes ALL the safety control measures described above should be considered.

    Using a ‘WEB SLING’ and from ground level, the sling with lifting eyes at each end should be passed

    through the barrel of the pipe and one end connected to the lifting hook on the crane. The crane

    operator should then be directed to carefully swing the crane over the other side of the pipe in order

    for the other lifting eye on the web sling to be hooked onto the crane hook. (Please note: the web sling

    should be long enough for the crane operator to swing across the other side of the vehicle without

    becoming taught, creating an impossible attachment process). Satisfied that the web sling is securely

    attached to the crane hook, the banks man should signal the crane operator to slowly carry out the lift,

    making sure the pipe is raised clear and safely away from the vehicle, and slowly slew the crane around

    to where the pipe is required. When it is lowered to the ground it should be firstly secured using a

    wedge or scotch on each side to prevent it from rolling. his action should be done before the web sling is

    released. This completes the task of safely unloading flex pipe.
