Terms and Conditions: By accepting the Dental Plan, Dr. Jahn Roedemeier will perform a comprehensive oral exam and members agree to follow prescribed maintenance program. Monthly membership fees are to be paid for a minimum twelve (12) month period and are non-refundable. Unless waived by the dentist, membership will automatically renew on the anniversary date and continue thereafter until cancelled, in writing. Qualified dependents are defined as a husband, wife, and any non-married children under age 22, living in the household as a full-time student. Any additional dependents after three (3) each will have an additional surcharge of $15 per month per dependent. Missed or broken appointments without 48-hour notice will be charged $10 for every 10 minutes chair-time lost. All member co-payments are due at time of service. Membership in the Dental Plan may be terminated for abuse and/or failure to pay membership fees or properly billed service. Dental plan benefits are not applicable until three (3) months after the initial date of acceptance except for Type I services. Frequently Asked QuestionsHow Does the Premium Plus Plan Work?The Premium Plus Plan is similar to many insurance plans on the market, only better! You pay a monthly membership fee comparable to traditional insurance premiums; this entitles you to discounts on your dental treatment. As a covered member you will receive preventative cleanings, exams, and diagnostic images twice a year at 100%. You will also receive up to 40% discount off all restorative treatment, and discounts on cosmetic treatment too.
Does a yearly maximum apply?Yes. A yearly benefit maximum of $1250 does apply.
Is there a deductible?There is not a deductible, preventative coverage starts immediately. There are no claims to file, therefore no claim will be rejected or denied. A third party will NOT determine your treatment. You will choose your quality of care! Dr. Roedemeier will make recommendations based specifically on your needs. Now anyone can have affordable dental coverage.
How does Specialty Dentistry Work with the Plan?Dr. Roedemeier may refer a patient to a specialist that participates in the Private Dental Plan. Members will receive a discount from these specialists. Dr. Roedemeier will have a complete list of participating specialists.
