Important Advisory Guidelines on the Submission of the Required Document(s) The document(s) to be submitted on the test date shall depend on the educational status of the Examinee (i.e., Graduate or Graduating) at the time he/she registered for the NMAT. The following document(s) MUST be submitted to and will be collected by the Examiner: FOR GRADUATING STUDENTS 1. NMAT Identification (ID) Form A duly accomplished NMAT ID Form should bear the Examinee’s signature and 2” x 2” colored picture with complete name tag that matches with the name in the NMAT ID Form. It MUST also have the School Head’s signature across the attached picture and on the designated space for signature of the School Head. Sample Picture: FOR GRADUATES 1. NMAT Identification (ID) Form A duly accomplished NMAT ID Form should bear the Examinee’s signature and 2” x 2” colored picture with complete name tag that matches with the name in the NMAT ID Form. Signature of School Head is OPTIONAL. Sample Picture: 2. Transcript of Records A photocopy of the Examinee’s Transcript of Records (TOR) or Certified True Copy of Grades (if newly graduate and TOR is still unavailable) should be submitted. FOR EXAMINEES WHO MAY NOT BE ABLE TO COMPLY WITH THE DOCUMENTARY REQUIREMENTS ON THE TEST DATE If you were unable to present a valid ID, your personal appearance and presentation of a valid ID is required at the CEM Test Center office where you took the test, whereas, document(s) – duly accomplished NMAT ID Form and/or Transcript of Records, should be sent to the NMAT Secretariat through a courier. The cutoff day for submission of documents is every Thursday. CEM will update the database of test results every Friday and release your online copy of results and official NMAT Examinee Report Form by Monday. Please be advised that your NMAT answer sheets will still be checked and scored together with all the other examinees. However, the release of your test results (i.e., Online/Electronic Copy and Hard Copy/Official NMAT Examinee Report Form) will be withheld indefinitely until you comply with the documentary requirements. ABADA, ACE III ON THE DAY OF THE EXAMINATION, the Examinee MUST bring the following documents and materials: 1. NMAT Identification Form (To be submitted to the Examiner.
