Updates from last workshop

New Chief Officer

• NHS South Norfolk and North Norfolk CCGs have appointed Frank Sims as its new Chief Officer

• Frank will join the CCGs on Monday 23rd July

• Frank was appointed following a rigorous recruitment process that included interviews with stakeholders and patient representatives.

Updates from last workshop

New Chief Officer

• Frank was most recently the Deputy Chief Executive at the Isle of Wight NHS Trust in July 2017.

• He has experience as Chief Executive across hospital and community providers and in commissioner organisations across London and the South East.

• He has been the national lead for the financial special measures programme with NHS Improvement and an advisor to the Department of Health.

Social Prescribing - South

South Norfolk District Council

• Service is up and running and is available in all 18 practices in the South Norfolk District Council area

• There are 5 Connectors plus 1 Senior Connector

• Connectors are aligned to GP Practices and serve a number of practices each

• Referrals can be made by GP practices on the basis that anyone patient facing in the practice can make a referral and these are then largely booked by the reception team in each practice.

• South Norfolk's Early Help Hub will support the connectors in their and referrals could be made through the Hub.

Social Prescribing - South

Breckland and West Norfolk

• There are 7 connectors in this model split across West Norfolk and Breckland District council areas.

• There are 7 providers each hosting 1 full time equivalent connector. post

• The connectors will be linked to specific GP practice but not necessarily based in the practice.

• The referral route will be through the use of a web based referral form will be available to all practices.

• Anyone patient facing will be able to refer in.

• Other service providers and local partners may refer clients into the social prescribing service via the early help hub in their area.

NCH&C – End of Life: Yellow Folders

‘Thinking Ahead’ Yellow Folders are used by health and social care professionals across Norfolk and Waveney to support ongoing conversations about future care planning for patients, and contain a leaflet to explain what advance care planning is and outlining the kinds of decisions people may want to make as they approach the end of their lives, as well as a form on which they can record their preferences about their treatment and any palliative care they may need.

• The leaflet entitled ‘What help is available’ will be distributed separately, explaining what standards of end of life care people should expect, and where people can go for help and advice.

• The documents have been revised as part of the Norfolk and Waveney STP (Sustainability and Transformation Partnership), which found that while there has been much good practice in palliative and end of life care, there has been variability in advance care planning and use of the yellow folders to date.

Expansion of Psychological Therapies for Long Term Conditions - Workshops

• 3 one-day workshops in July 2018.

• Central Norfolk, West Norfolk and Great Yarmouth & Waveney

• Service users including those with cardiac, diabetic, respiratory, arthritic and … long term conditions.

• Commissioning & quality representatives, clinicians from community, acute & primary care, senior managers and system leaders.

• Ongoing development within the areas of: Central; West; & Great Yarmouth & Waveney.

• Build on service users representation through LTC specific groups.

• Further work within LTC specific transformation programmes to ensure IAPT is embedded in services.

• ‘One model’ wherever possible across STP footprint, with aspects tailored to clinical pathways where needed.

• Building of momentum and enthusiasm
