INTERVENTIONAL PULMONOLOGY ...

Most promising and fastest growing branch of medicine ,having wide dimensions and full of innovations.

Apart from Videobronchoscopy,I stared some latest interventions in pulmonology first time in Ghaziabad .These are ENDOBRONCHIAL ULTRASOUND and MEDICAL THORACOSCOPY.

£ENDOBRONCHIAL ULTRASOUND....

EBUS combines an endoscopic image with an ultrasound probe giving sonographic images through the airway wall . There are two types of EBUS probes RADIAL and LINEAR .

We are doing LINEAR EBUS Which is to diagnose mediastinal lymphadenopathy and for lung cancer staging.

It is usually performed under local anesthesia and sedation like bronchoscopy.

Tolerance is very good and complications are almost negligible.

Conversely to the use of conventional bronchoscopy, direct transducer contact with the wall of trachea or bronchus is promoted and the image of airway wall and structures in the airway wall and outside airway wall are clearly seen in ultrasound image.

So under ultrasound guidance you can directly puncture the lymphnode or growth in mediastinum which is usually not possible with simple bronchoscope and similarly we can do the lung cancer staging also.

As per various studies it is the gold standard method to diagnose mediastinal lymphadenopathy and nowadays this method replaced the conventional mediastinoscopy.

Results of EBUS as per various studies are approximately 85% I am glad to say that by God’s grace our results are approximately 80 to 90% and till date we did around 45 cases without any complications.

£MEDICAL THORACOSCOPY...

Medical thoracoscopy or pleuroscopy is a minimally invasive procedure performed by pulmonologist using an endoscopic telescope in a non intubated patient under conscious sedation.

2. Therapeutic management.

Diagnostic Medical Thoracoscopy.......

The most common indication is undiagnosed pleural effusion to take biopsies from pleura,diaphragm and mediastinum.

Sensitivity of Medical Thoracoscopy is about 95% as compared with pleural fluid cytology at 57%.

Therapeutic Medical Thoracoscopy......

Commonest indication is PLEURODESIS with TALC( which is freely available ) for recurrent malignant pleural effusion and recurrent pneumothorax.

Resuits of pleurodesis are excellent with almost negligible complications.

I started medical thoracoscopy with semi-rigid thoracoscope about 4 months back and we are doing cases with good results.

£VIDEO-BRONCHOSCOPY......

Most commonly used modality for various indications.

I am doing bronchoscopy since last 10 years and did around 1000 bronchoscopies in various indication.

Almost all procedures like endobronchial biopsy, transbronchial biopsy, bronchoalveolar lavage and brushing etc. can be easily done with flexible bronchoscope.

PLASTIC SURGERY

Plastic surgery, by definition, is a problem-solving surgical specialty, which deals with the defect and deformity of human body arising out of congenital defects, trauma, infection, cancer and other problems. This branch is related with repair and reconstruction of missing and damaged tissue. The main aim is to restore the function of tissue as close to normal as possible. ‘Plastic’ is derived from the word ‘plasticos’ meaning to mold, or to reconstruct. Cosmetic surgery, a sub-part of plastic surgery consists of re-proportioning of body parts by surgical technique to achieve an appearance more pleasing according to the patient, which depends on his/her ethnicity because beauty as such has not been defined yet.

In our field, a sound knowledge of the anatomy and physiology of all tissues, health of the patient, the healing process, and precise planning, imagination,thought processing and

execution are all necessary for a desired outcome.

Plastic surgery involves solving the problems related to all specialties of medical sciences- from General Physician, whose patient may come with injection paralysis or facial paralysis, to Orthopedics where soft tissue injuries with nerve and tendon defects may be present with a fracture, to ENT Surgeons with patients with nose deformity requiring nose reconstruction; we work closely with the Oncosurgeon as reconstruction is required after extensive tissue excision, with the Neurosurgeon where cranial defects are present, with the Ophthalmologist for eyelid reconstruction and even beauticians for facial aesthetics. Gynecologists can involve us in treatment of vaginal agenesis and other related problems, while General Surgeons can refer surgical complications or chronic wounds to us. There is a great role of plastic surgeons in management of Diabetic foot and non-healing ulcers. So, the domain of the plastic surgeon is from head to toe and involves solving myriad surgical ‘problems’.

Solving most of these problems involves a lot of innovation and some basic plastic surgical techniques such as tissue repair techniques, tissue adjustment techniques (Z-plasty, V-Y plasty, W-plasty), flap surgery and tissue expansion.

Plastic surgery is usually divided and discussed under three parts: first, corrective surgery, where tissue is only adjusted, repaired and modified; second, reconstructive surgery where tissue is added or replaced; and third, cosmetic surgery, where only shapes are redefined. Here, volume can be reduced or added.

We shall discuss corrective surgery today.

Common corrective surgeries are soft tissue injuries of face, limbs, etc, congenital anomalies like cleft lip, palate, nerve, tendon and vessel injuries without loss of segment, normal nose without loss of tissue, etc.

The commonest injuries in daily life are soft tissue injuries. In the management of soft tissue injuries, it is imperative to handle the tissue extremely gently while using the atraumatic technique while suturing. The discretion of when to dress and when to leave the wound open is also instrumental in deciding the fate of the wound, and hence the outcome. As a rule of thumb, we always leave a contaminated wound open, and do a re-look surgery after 48 hours.

Other technical aspects such as a thorough debridement, meticulous hemostasis, obliteration of any dead space, strict avoidance of tension in wound closure, and judicious use of buried sutures are also best understood by us, the ‘wound specialists’; To us, the classic 6-hour golden period makes little biological sense nowadays, as we understand the wound better, and with early intervention by antibiotics, wound infection is averted and healing is not compromised.

Plastic surgery as a branch provides solutions where hopes are bleak, and hence are a source of joy to patients and doctors alike.

DIABETES & EXERCISE

“Exercise is the best insulin sensitizer on the market; better than any medication we currently have available"

Diabetes is one disorder wherein lifestyle modification (physical activity and healthy eating) plays a s i m p o r t a n t a r o l e a s

medications to control sugar. Regular exercise could bring about a change from "medical management" to "lifestyle management" in patients with Type 2 diabetes. A Regular exercise regimen in the diabetes-afflicted helps in the better control of blood sugar and reduces the demand for medication by 20%. Patients with Type 2 diabetes, who regularly exercise, find that their blood glucose is better controlled, but on a day they miss their physical activity program, the glucose level tends to rise. Exercise is now considered an important tool for the long management of diabetes.

How Exercise help in Diabetes

EXERCISE is the physical activity that is planned, structured, repetitive and purposeful, usually aimed at improving or maintaining physical fitness

During the first few minutes of exercise glycogen in the muscles is broken down anaerobically

After 5-10 minutes of activity muscle glycogen breakdown decreases. Glucose broken down in the liver is released into the blood stream and is taken up by the muscles as fuel. This glucose becomes the major source of fuel (hepatic glycogenolysis).

After 20 minutes or more muscles glycogen stores are now depleted. Blood glucose is now maintained by hepatic glycogenolysis and triglycerides that are broken down from adipose tissue. As exercise continues fat breaks down to free fatty acids (FFA) and is used as a source of fuel for the muscles, through the process of hepatic gluconeogenesis rather than hepatic glycogenolysis.

low-to-moderately intensive exercise continue for a long period of time, the muscles will continue to use the glucose derived from hepatic gluconeogenesis with FFA as major fuel.

If carbohydrate is consumed during exercise, the decrease in blood glucose can be delayed and the exercise can be sustained for a longer period. This is often done by people with diabetes who are engaged in moderate to intensive

exercise for long periods.

Exercise Improves overall blood glucose control and HBA1c levels in type 2 diabetes. Improves insulin sensitivity and lows insulin requirements often leading to a reduced dosage of insulin and oral hypoglycemic agents especially in people with Type 2 diabetes, attainment and maintenance of ideal body weight, reduction of coronary risk factors, favorable changes in body composition (decreased body fat and weight, increase in muscle mass)and lowers cardiovascular and overall mortality.

Effect of exercise

In the acute phase, exercise results in translocation of glucose transporters to the plasma membrane and increased glucose uptake

With prolonged exercise there is up regulation of glucose transporter numbers, changes in capillary density which causes an overall improvement in insulin sensitivity.

Even short-term (2-week), regular aerobic exercise in type 2 diabetic patients results in significant improvement in both aerobic capacity and whole-body insulin sensitivity.

Long-term endurance training in diabetic patients markedly improves whole-body insulin sensitivity and the expression of key muscle enzymes regulated by insulin. However, the maintenance of this effect seems to require dedication to a regu lar and un in ter rup ted exerc ise reg ime. Intramyocellular lipid accumulation, which is associated with insulin resistance in muscle, can be acutely decreased by even a single bout of sustained endurance exercise. So exercise is beneficial for both glucose uptake mechanisms and the anti-lipolytic effects of insulin.

How much and how to do exercise?

Exercise ECG(TMT) or achieving 85% of target Heart Rate (THR) is safe for deciding extent of exercise in initial phase. THR is very simple to calculate by formula 220 – age in yrs = THR Exercise session has to be minimum of 30 – 45 min. Intensity may be mild in beginning with built-up to a faster pace as time passes with weakly up gradation with a watch on THR

Warm up phase: This phase is the first 5-10 minutes at the start of the exercise and comprises low intensity, easy movements. For example, if one wishes to jog, walking prior to that is the warm up phase. Stretching and bending exercise before a game of badminton or tennis or golf for 5

to 7 minutes is another type of warm up. Stretching helps in making the body flexible and helps in preventing body soreness after exercise.

Aerobic phase: This phase comprises 20 -30 minutes of intense physical activity in people who are accustomed to a regular exercise plan. People who have just started the exercise program may not be able to maintain the aerobic phase for long, and should increase it gradually. During this phase, the heart beats faster and breathing becomes harder. Aerobic exercises helps to increase insulin sensitivity and restore normal glucose metabolism by decreasing the body fat.

Cool down phase: This phase is the last 5-10 minutes towards the end of the exercise program. The cool down phase is to allow the heart beat rate and the breathing to gradually slow down. One must never stop exercising abruptly. Walking after jogging is an example of cooling down phase.

Exercises should be done according to FITT principle.

FREQUENCY: Exercising 5 to 6 times a week.

INTENSITY: 30-40 min of exercise at 65- 85 % of target heart rate.

TYPE: SAFE exercises are recommended.

TIME: Morning is ideal but can be done at any time

Strengthening exercises: Anaerobic exercise uses large muscles that do not require oxygen for short periods of exercise. It helps build strong muscles; lowers blood glucose makes the action of insulin more effective

Aerobic exercises: Aerobic exercise is the type that moves large muscle groups and causes you to breathe more deeply and your heart to work harder to pump blood. It's also called cardiovascular exercise. It improves the health of your heart and lungs.

Flexibility exercises: Flexibility exercises are aimed at increasing or maintaining range of motion at joints, also improve tone in muscles and keep it supple. They develop better muscular and body control.

Endurance exercises: Low Resistance, High Repetition Exercises with example as brisk walking, cycling, swimming, or upper extremity ergometry that involve the use of the large muscle of the body.

No exercise program can be described as the ideal program. Each has its own advantages. Physical activity of any type (e.g. walking, jogging, swimming, badminton, tennis or similar activity) should be incorporated into the daily routine. Choosing an activity that is enjoyable and is fun helps to sustain it over time. A variety of exercises must

be selected and doing it with a partner or friend is helpful in avoiding boredom.

Walking: This is the safest, commonest and the cheapest form of physical activity. One needs only to invest in is a pair of good walking shoes and sports socks. It can be done by people of any age and anywhere. It is ideal for people not used to physical activity. It is the warming up or cooling down activity of choice.

Jogging: Some prefer to jog since it is more fun. It is more intense and requires lesser time than walking for the same amount of exercise. Comfortable pair of shoes, preferably with extra cushion should be used. Jogging on hard surface should be avoided. The footwear should be replaced much before it wears off. For one to get into a jogging habit, it is better to walk first (warm up) and jog for a few minutes and then walk again, repeat the process, and finally walk at the end as mentioned in cool down phase.

Using weights: This uses large muscles that burn more calories. This can include using small weights in the gym or health club. One should begin with light weights and then increase the weights gradually. As one becomes stronger more weights can be lifted. It helps to reduce osteoporosis and builds muscles especially in the elderly. This type of exercise increases glucose uptake by the muscles and enhances their ability to store glucose.

Yogic exercises cause the muscles to absorb the excess glucose in the blood, thereby reducing the blood sugar level. They help the pancreas and liver to function effectively, which regulates the blood sugar levels. Asanas help in rejuvenating the pancreatic cells, thereby assisting insulin secretion. The muscular movements also help in bringing down the blood sugar levels. Asanas induce relaxation, which also plays a key role in the healthy functioning of the internal organs of the body. Yogic exercises are perhaps the only exercises that bring us close to the inner being or the soul, which is essentially spiritual in nature. Stress plays a important role in making controlled diabetes into brittle diabetes Avoid exercise outdoors on very hot or humid days.

vTo prevent dehydration, drink a cup of cold water before and after you exercise.

As a non-profit organization/ association of good repute, utmost care should be taken while spending public money. Any negligence can have a detrimental effect on the image of association as well as of the office bearers. Sometimes intentions are honest but due to ignorance the expense is booked improperly.

For correct claim of expenditure an intelligent office bearer must focus on 4 broad aspects:-

1. Quotations, Approval and Budgets

To safeguard against the future allegations for over expenditure, one should ensure that any expenditure to be made is strictly within budget allocated. In case budget is exceeded, such over budget should be justified and approved in the following council meeting explaining the circumstances. Any major outlay should b e made only after inviting quotations from maximum possible vendors (minimum 3). Next is obtaining necessary approvals from the higher authorities within the o rganization for such planned expenditure. I

2. Invoice

Invoice is the most critical document for claiming expenditure. It should contain verified details like vendor's Name, Address, Telephone no, Registration nos. like VAT, PAN, Service tax, if any in a preprinted and serially numbered format.

Invoice should also mention the name and address of the association receiving the goods or services. The person or office bearers should specifically ensure that all invoices follow this criterion as it has been observed on number of occasions that the person who goes to the market or who deals with the vendor gets his own name mentioned instead of the association. This is completely unacceptable to the tax authorities and may result in uncalled tax liability by disallowance or expenditure. Moreover some vendors act over smart and in place of invoice the heading of the document given is 'Estimate' or 'Performa invoice' which is only a

document for approval to give an idea of how the final invoice shall be. It can't be used to justify the claim for expenditure made. Such documents should be refused to be taken like poison.

3. Payment

The other important aspect is proper payment. Any payment which is not made by an account payee cheque, RTGS or NEFT directly in the account of the vendor raises doubts in the minds of the auditors as well as the government authorities. It has been observed that vendors submit the invoices in their firm's name and accept cheques in their personal name or any other name. As a result no proper audit trail is available and such expenses are difficult to prove before the tax authorities. The golden rule is Issue cheques in the name of the firm name of the bill asmentioned". Sometimes payments are so small, issue of cheques is not possible, is such case any one time payment cannot be more than Rs. 20000 and proper receipts should be obtained from the vendor for such payments.

4. Accounting Entries

Proper recording of entries in account books is as important as obtaining proper bills. Unless an expenses is recorded correctly in books. its claim can't be justified. The office bearers should ensure that expenses are booked under proper heads of expenditure. If collections are made for some specific event (like Independence Day celebration), not only the collections be shown separately but also the expenses related to such event should also be recorded separately under different head of expenditure (like independence Day expenditure). In this way the members and government authorities can have clear understanding of working of the association.

Do We Know What IMA SATYAGRAHA Is?

IMA has waited for one year since it postponed its Satyagraha scheduled on 16th November 2015. In the last one year, the Ministry of Health, Government of India has not taken our demands seriously.

On the contrary, they have decided to abolish Medical Council of India and replace it with a National Medical Commission. National Medical Commission (NMC Bill) will be constituted by members nominated by Government of India and roughly half of them will be non-medical persons.

Ayurvedics, Homeopaths and others will get registration in NMC through Schedule IV. Private medical colleges will be let free to fleece the students.

Medical graduates will have to take a licentiate exam after final MBBS to practice as a doctor.

To recapitulate, IMA wants the following six main demands

1. Withdrawal of proposed NMC Bill and suitable amendments in Indian Medical Council Act, maintaining the professional autonomy.

2. Insulation of doctors by bringing a Central Act against violence on doctors and hospitals.

3. Amendment in Consumer Protection Act (CPA) to cap the compensations.

4. Suitable amendments in Clinical Establishment Act (CEA) to change the licensing system to an accreditation system and to exclude single doctor clinics from CEA.

5. Suitable amendments in PCPNDT Act by which only acts linked with sex selective abortion are penalized and not clerical errors.

6. Proper implementation of rights to prescribe Scheduled Drugs exclusively by modern medicine doctors.

Our Branch observed IMA Satyagraha, members collected at IMA Bhawan in morning and organize a peaceful dharna before the DM on 16th November 2016 from 11 am to 1 pm.

We released a standard press release.

Submit the standard memorandum addressed to the Prime Minister to the district magistrate. Dharna was successful and a large number of doctors participated.

Photographs of the dharna along with the press note sent to regional and national press with a copy to IMA HQs.

Injustice to doctors has been taken so casually by the Government. Let us unite in the cause of generations of upcoming doctors.

If some body thinks Demonitisation was for the eradication of black money, I will say he or she is

very sadly mistaken. This act was multifaceted as was for multiple objects. I will like to label it first

step towards limitation of black money. If any body thinks black money all together will disappear

after this, then he or she is committing a grave mistake of under estimating the expertise of we the

black marketier Indians. Infact, since independence all the Governments could not provide a policy

driven state, so an atmosphere developed in the country to earn a case for oneself & not to expect any thing from the

government. So we could never develop a society which pays its taxes to the Government. A very small number of people pay

taxes to govt & this money is not sufficient to provide the all to services to public after bungling of major amount of money

collected in the service of Govt. At self General public, who pay taxes feels, cheated why they pay taxes? So it is a catch 22

situation who is at fault. Now every body knows there were Rs. 16.5 Lac Crores of currency notes, out of this 86% were in the

denomination of 500 &1000 which were stopped by this act of government. As per the experts statement Rs. 6 lac crores were

not in circulation or you can say they were hoarded by so called black marketeers eg. Politicians, Bureaucrats, Industrialists,

traders & Professionals, After 9th of Nov. practically Rs. 7 lac crores which were in saltation of high denomination were

neutralised & by 20th of Dec. Rs. 6 Lac Crores worth new printed notes were crenelated by service reserve bank & small

domination notes sent to Reserve Bank for cancellation were also temporarily recerculated but even then there is much

shortage of notes in the market which should not be according to my estimates. Why? Here i will mainly blame the inefficiency

& corruption in banks. Evidence are in front of us, more then 300 Bank officers are under scanner for illegal note exchange

rackets, Many tax consultants became currency exchange agents over night in every city. That is why all the so called black

money got exchanged except meager Rs. one to two lac crores. Before 8th Nov. governments estimate was 4-6 Lacs Crores.

So this proves, my opening sentence of this article is correct. Here I will like to add that we should not underestimate the

adamant nature of Our Respected Prime Minister Shri Narendra Modi Ji. I hope & wish most of the corrupt people who tried to

fail this campaign should & will be caught in near future.

Here I will like to conclude on a happy note that during last two months people from all walk of life have been caught in illegal

note exchange but not a single doctor in the country, so cheer up & brace up to contribute your share in the country's

development.

Spiritual health is an important dimension of health on which health can be enjoyed and is an impor tan t base o f o ther dimensions of health. The World Health Organization is also looking beyond physical, mental and social dimensions of the health i.e. the spiritual health and its impact on the overall health and happiness of an individual.

Spiritual health means various things to different people. To some, it means the order of organized religion, in some the freedom of spiritual experiences and to others the integration of the person for optimum wellness.

For some, Spiritual Health is the ability to practice moral or religious beliefs in your everyday life.

Spirituality is unique to each individual. Your “Spirit” usually refers to the deepest part of you, the part that lets you make meaning of your world. It is that innermost part of you that allows you to gain strength and hope.

Spiritual wellness is a personal matter involving values and beliefs that provide a purpose in our lives. It is important for everyone to explore what they believe is their own sense of meaning and purpose.

The path to spiritual wellness may involve meditation, prayer, affirmations, or specific spiritual practices that support your connection to a higher power or belief system. Yoga and meditation can also help you develop spiritual wellness.

Spiritual health is a: State of being where an individual is able to deal with day-to-day life in a manner which leads to the realization of one’s full potential; and happiness from within. Spiritual health is known as the good connection between mind, body and soul.

Health is wealth and is considered ‘igyk lq[k’ in Hindu mythology. In May, 1984 WHO has included “spiritual dimension” in the definition of health as a fourth dimension. WHO definition of health is “Health is a state of complete physical, mental, social and spiritual well-being and not merely the absence of disease or infirmity.”

Dr. Kusum Lata Gaur, WHO fellow IEC and Dr. Mahesh Sharma, Principal Medical Officer have designed a scale for the measurement of Spiritual Health.

This scale is based on 03 domains to measure level of spirituality, i.e. Self-Development, Self-Actualization and Self-Realization.

Self-Development is analyzing and applying the moral values in their thoughts and actions.

Self-Actualization is consciously aware of you and your values. Self-Realization is consciously aware your own soul.

1. You can put CCTV camera in waiting room /lobby/entrance of clinic (No consent from patients required)

2. Never put CCTV in examination room or where some procedure is being done. Consent from patient is required if you have done so.

3. Never record audio conversation /advice tendered in doctor-patient relationship without express consent of patient.

Dr Shyam Garg had humble origins and was born in the pre-independence era in 1939. His

father was an officer in the Central Excise Department and the family moved abode rather

frequently to varied and distant places. Schooling was not as consistent, but he did well

enough to become one of the founding batch of medical students from G.S.V.M Medical

College, Kanpur and finally left in 1965 with an M.S. (General Surgery) qualification.

As a young, idealistic doctor, he harbored the dream of scaling the heights in academic

medicine, and this was realized when he was appointed Professor of Surgery at the young

age of 40 years. This was at Ahmadu Bello University Hospital, which remains the largest medical school in Northern

Nigeria, and one of the largest in Africa. He was famed for his surgical expertise and clinical acumen, and this reputation

was reflected in him being the surgeon of choice for any staff member at this university. He contributed significantly to

both international research and teaching.

He had many international publications. In particular, he made original contributions to the management of intersex, which

at that time was amongst the largest published series of intersex patients in the world. He presented this work at a urology

conferences in the United States in 1982, and this was well received by the scientific community there. His passion for

Urology started around this time, and he rapidly established the first dedicated Urology service in the whole of Northern

Nigeria. Being the only Urology unit in such a large population, he developed a large referral practice, and soon started the

first post-graduate training program in Urology in Northern Nigeria. His reputation as a surgeon and immense likeability,

together with his drive to teach led him to becoming one of the most popular teachers amongst surgical post-graduates

eager to learn Urology. This is reflected in the number of senior urologists that currently practice in Nigeria, and overseas,

mainly in the UK, that were trained by him.

A dedicated family man, he decided to leave all the above as soon as his father suddenly passed away in 1986. He moved

permanently to the town of Ghaziabad so that he could be with his widowed mother. A tireless surgeon, who operated on

the day he died, he established the first private dedicated Urology centre of Ghaziabad in 1995. He leaves behind his wife,

Savita Garg, to whom he had been married for 50 years, his son, Sudhanshu Garg, a Consultant Urologist in UK, daughter

in-law, Ruchi Garg and grand-daughter, Kimaya aged 9 years.

His life and works truly exemplified the adage that “We are Born to Serve”, and this is the legacy that he leaves behind for

his family and the doctor community at large.

IMA BHAWAN CHARITABLE TRUST

The IMA BCT which was started by 45 members of IMA Ghaziabad under the banner of IMA Ghaziabad in 1999 have now grown up to a strength on 102 life members.

The seed sown by our seniors have now grown up in a green herb, but it have to go a long way to develop into a tree. The IMA Bhawan of Ghaziabad have come a long way since it’s inception fifteen years back. Every team at IMA BCT contributed a lot in the present façade of the marvelous building of IMA Bhawan. But we have to work a lot harder to make financially viable and independent.

The facilities available at the IMA Bhawan are unique and still not available at any other place in the city. But they are still under utilized. We must give a serious thought about it.
