New User Quick Reference Guide – Opening your HSA This document outlines our user friendly tools for online enrollment for your Health Savings Account. Step 1: Log in to the Consumer Portal to begin on-line enrollment: Enter your Username and Password on the Secure Login Screen Your user name is the first 4 letters of your last name and the last 4 digits of your social security number and an underscore with abb. The first letter of your last name will be capitalized. For example: John Smith’s username is: Smit1234_abb Jane Doe’s username is: Doe1234_abb Your password, the first time you log in, is the same as your username. At your first log in you will be prompted to change your password for your security and also select a security question along with your answer.

Step 2: Begin online enrollment by clicking the HSA Enrollment link. Once you click the Enrollment link, you will see the Plan Description. Review this information to make sure you understand this plan. Then click Next to proceed.

Step 3: Enrolling in the HSA If you are enrolling in the Health Savings Account, you will need to click each of the links to “Read and agree” each document. You can print these documents if you want to keep a copy (note: these documents will always be available on this site for your further review). After reading each document, you will need to scroll to the bottom of each document and click the box next to “I have read and agree”. Then close that document and go to the next one to read and agree. After each document is reviewed click Next to proceed. Step 4: Verify your Consumer Profile information. Fields with a red asterisk (*) are required. Your email address allows you to receive notifications and other important information quickly. Click Next. Please note: Identity verification is required to open your HSA. If we are unable to validate your identity based on the information provided, you will be given the opportunity to make changes if applicable, and select Next. If you determine your information is correct, we will request verification documentation from you in the form of a driver’s license, utility bill, and/or social security card to complete your enrollment. Step 5 (if applicable):

If you have dependents covered under your plan, enter your Dependent(s) information and click Add Dependent. The added dependent displays under the Eligible Dependents list. Click Next to proceed. Step 6: Review the Plan Rules and then certify your eligibility (you must click the check box next to the statement “I certify that I meet the qualifications to open a Health Savings Account”). You must also select your coverage level. If there are no dependents loaded on your account, your coverage level will default to Individual.

You may notice that the only coverage level options on the Eligibility Screen are Employee or Family. If you are covering any dependents on your medical plan, regardless of that dependent being a spouse or child or both, please choose family as the coverage level.

Step 7: Enter your election amount in the My Election field.

NOTE REGARDING EMPLOYER “SEED” CONTRIBUTION AMOUNTS: You will note that the family tier shows $300 as the employer contribution amount. Please be aware that depending on the dependent you cover on your medical plan will determine the amount of the annual employer seed contribution as follows:

If you cover a spouse as a dependent (with or without covering children), the annual employer seed will be $300.

If you cover dependent children (and no spouse), the annual employer seed will be $400

Once your account information has been confirmed, you will see the appropriate employer seed contribution in your account balance and summary. Please note that the employer seed contribution is prorated for new hires based on their hire date. If you want to see the tier designations and the employer contributions, you

may see this on the main portal screen by selecting View More.

Click Next to continue. Step 8: Select your Payment and Reimbursement Methods. If you want a debit card issued to any of your dependents, please click the check box for “Issue Card” next to the name(s) of the dependent(s) you want to have a debit card. For Reimbursement Method select your method of choice. Click Next to continue.

If selecting Direct Deposit enter your bank account information.

The Bank Information may pre-populate based on the routing number entered. If not, enter the contact information for your bank.

Step 9: Select beneficiaries from the Dependents box to pre-fill the information or enter the beneficiary information manually. Note: During Enrollment, you must name your spouse as a primary beneficiary if you are married. Click Add Beneficiary. Add a contingent beneficiary by choosing them from the Dependents box or complete the fields manually, and click Add Beneficiary. Beneficiaries and Share Percentages display and you may Update or Remove as necessary. Click Next to continue.

Step 10: Verify your Enrollment Summary information carefully. Click Edit Information in any of the sections to make corrections. Click the Print link for a copy of your Enrollment Verification. Click Submit.

Step 11: You must now complete the “HSA Enrollment Creation Authorization" requesting that an HSA be opened in your name. Click Submit Enrollment.

Step 12: Congratulations! You have now successfully completed your HSA enrollment! Your account is activated on the first day of the month following the date you receive confirmation of your HSA enrollment. Please contact HealthSCOPE Benefits if you have questions.

Once your HSA enrollment is completed, you can update your election at anytime during the open enrollment period from the home page (or at any time during the year), by clicking Change HSA Election.
