Initial Community Assessment Template

IIC and BA Services

A completed ICA is required to be submitted along with a Needs Assessment

IIC providers must be certified by DCBHS to conduct a Needs Assessment to complete the

Initial Community Assessment

Assessments must be performed in clinically appropriate, non-office settings according to

family need and preference

Assessments must be typewritten on the ICA Template and autofaxed to ValueOptions within 10 business days of referral date

What are the billing guidelines for these services?

IIC and BA Services

Providers may only bill for face to face services in the community, not in an office setting

Providers must make themselves available to provide the number of units that are authorized

according to family need and preference

Units billed may include face to face contact only, not travel time, paperwork, collateral contacts, etc.
