The hospital is now operating in its 13th year. I note with pride that the hospital is fully self-dependent on operational costs for the hospital. The hospital has been able to even afford small investments on infra-structure development. However, Nepal Orthopaedic Hospital still needs support from its well-wishers. Without the support from them it would not have been possible to evolve from a small organization to this big specialized orthopaedic hospital. The doctors, nurses, paramedics and administrative and other support staff need to be again thanked for carrying out their responsibilities efficiently. Dr. Saju as well as other board members must also be thanked for guiding the hospital and continuing the path of service above self in the field of orthopaedics.
Congratulations to Dr. Saju’s leadership in successfully organizing the 5 kilometer disability awareness walk in conjunction with various Rotary Clubs. This helped the hospital raise funds for its ward extension programme as well as raise awareness about disability. The hospital also got media mileage out of this event which is very important. The challenge ahead in the future is to remain competitive so that more patients can be attracted and at the same time continue charity work and reduce treatment costs.
Hopefully, in 2011, Nepal Orthopaedic Hospital can claim to be a successful 100 bedded specialized care organization after the addition of 26 more beds.

I am extremely happy to announce that activities of the hospital last year has been very much successful. Many free surgical camps in conjunction with our partners were carried out last year. The six day a week surgery routine has also been very successful. Patients don’t need to wait for their turns. The patient flow has been good as well. However, we have to remain ever vigilant and keep our services very good, quality wise and cost wise. This is because there are so many other hospitals cropping up around the valley, which will be catering to the needs of the population. Our hospital is not located in a very accessible area and the only way to keep attracting patients is to keep the services cheap and at the same time give the best care possible.
It is very difficult for a hospital like ours to give charitable services to the poor patients and also give a satisfactory salary to our employees. It is also not easy to keep the hospital neat and clean and also at the same time not compromise on quality of care. We have been doing it since many years and we will strive to keep it that way. There is a lot of cost involved for this and I have to thank all the doctors, nurses, paramedics and administrative staff who have been doing all the hard work to keep the hospital’s economy still self-reliant.
The 5 kilometer disability awareness program was also successful in raising awareness for the plight of the disabled people and also at the same time we could generate some funds for the extension of the proposed ward. The staff union also must be thanked for the lottery program through which they were able to contribute to the construction fund. I must also thank the Rotarians, the pharmaceutical companies and the many business partners who contributed for the success of the fund-raising effort.
The plans for the future now include the addition of a male and female general ward, which will make NOH a 100 bedded hospital; computerization of admission and discharge of patients and wireless networking of the various sections of the hospital; repair and maintenance of the physical infra-structure of the hospital; expansion of laundry services; addition of public toilets, addition of patient waiting lounge etc.
The hospital also needs to send our surgeons for specialized training so that we are able to keep abreast with the developing trends in the field of orthopaedic and trauma service.
Finally, I would like to thank all the well-wishers and donors of the hospital who have been relentlessly helping us in times of need. Without you all this hospital would not have been what it is like today.

It is with immense pleasure and indeed a gratifying feeling to have been approached for the message in the newsletter issue, volume 12 on this great occasion of the 13th anniversary of Nepal Orthopedic Hospital.
I assure you that Rotary Club of Patan will continue all cooperation and support to NOH and the Hospital Management Board as was being done in the past.
RCP is dedicated to participate in further expansion activities of General Ward Extension plan as well as Rotary Matching Grants for bed and furniture plus laundry and washing machine.
Finally, on behalf of Rotary Club of Patan and on my own, I would like to thank entire members of the whole team of Nepal Orthopaedic Hospital for the friendly support and hard work to bring this hospital to this stage. I hope to receive the same team spirit in the days to come.

Happenings in 2010
Computerized System in OPD And Laboratory Department
A new OPD BOOK has been issued and computerized
billings started in OPD since January 1st 2010. Th is
new book has an introduction about the hospital and
also describes in brief the services given. Th e hospital
also started giving out computerized laboratory reports
from January 01, 2010. Th e billing of patients has also
been computerized since January.
“100% Safe Drinking Water Treatment Plant”
A Japanese water treatment plant was donated to our
hospital as a matching grant project (MG# 70299) by
Rotary Foundation in conjunction with the Rotary Club
of Shimodate, Japan (District 2820) and host club Rotary
Club of Kasthamandap, Nepal (District 3292). Th e plant
is able to dispense 100% safe drinking water. Th e unit is
able to eliminate all kinds of bacteria, viruses and fungii.
Th e water is currently being used for patients, hospital
staff and visitors for drinking putrposes. Some of the
water is diverted to the Operation Th eater and is used
in the autoclave machines for sterilization of instruments.
All these changes have given NOH a more modern
outlook. Also these changes have made work for our
staff much easier.
Th e water treatment plant was inaugurated on July
23, 2010 by the Japanese ambassador Mr. H.E. Tatsuo

Mr. Ratna Man Shakya. Many other Rotarians from
various clubs and hospital staff s were present during
the occasion.

machine and a C-arm fl uoroscope. Th ese units were
installed on June 19th, 2010. Major Yam Bahadur
Gurung representing the Kadoorie Foundation
facilitated the culmination of the project. Th e hospital
had applied for a grant to upgrade its radiological
services earlier in 2009.
Aft er the installation of CR machine, new portable
x-ray machine and C-arm fl uoroscope with image
intensifi er, the radiology services have become more
advanced. Th e quality of the X-rays in the hospital are
much better, the waiting time for the patients to collect
reports has been much shortened and also surgery has
been facilitated because of the availability of two C-arm
fl uoroscopes in the operation theaters.
Installation of New Autoclave:
A horizontal cylindrical high pressure, high speed type
steam sterilization (Flash Autoclave) was installed
on 1st October 2010 for a total cost is NRs.10, 93,450.
Th e hospital had three old autoclaves in its CSSD and
one of them was a real old autoclave which had been
used for nearly 15 years. These autoclaves needed
Wheelchair Donation :
Nepal Bangladesh Bank Ltd. Donated 10 wheel chairs
to NOH on the occasion of NB Bank’s 17th Anniversary
on June 06, 2010. CEO of NB Bank Mr. Govinda Babu
Tiwari handed over the wheelchairs to Mr. Shyam
Prasad Rupakheti who works in the administration
department. Th ese wheel chairs are currently being
used by the patients of the hospital.

The Kadoorie Charitable Foundation, Hong Kong
donated NRs. 5,660,000 for the installation of a
computerized radiography unit, a portable x-ray

constant repairing and maintenance and the hospital
was spending money all the time to get these old units
in working condition. Th ere was an urgent need to
either replace or add a new autoclave machine. Th us the
hospital decided not to wait for any donations to come
for a replacement and ordered this new generation
autoclave which is much faster and of higher capacity
than the old ones. Th e other older autoclaves have also
been repaired and thus now all four are in working
order.
Th e OR staff are fi nding it much easier to operate the
CSSD.
New Store Room
A new store room has been built near the ground floor ramp to house old and defunct equipment in the hospital. As the hospital is growing and getting older, ageing equipment and furniture need to be placed in a safe store for repair or before they are discarded as defunct waste. This store room has alleviated our storage potential to some extent.
Foot and Ankle Surgery
Th e Foot and Ankle Surgery camp 2010 was organized
with the help of Healing the Children, USA. The
camp was held for fi ve days from Oct-31, 2010- Nov
04, 2010). Th e team was headed by Rtn. Dr. Stephen
Miller with Dr. Tom Chang, Dr. Ryan Jense, Dr. Mark
Sinkey, Dr. Robert Apter, Ms. Bojen Kurre, Ms. Brenda
Graham Apter (Administrator), Ms. Marian Sinkey,
Ms. Maureen Waterbury, Ms. Teresa Alexander, Ms.
Catherine Ruggels, Dr. Nick Kassebaum, Dr. Jay Ryan
and Mr. Marc Estvold.
Many patients with disabilities in the foot and ankle
from Kathmandu valley and various parts of Nepal
came for the treatment. Twenty nine patients were
screened during the camp and 18 were operated
amongst them. The surgeries included clubfoot,
equinus contractures, hallux valgus, fl at foot etc. All the
patients were treated free of cost by the hospital. Even
bed charge, medicine charge and surgical charge were
not taken. Th e total cost of NRs. 405,365 was written
of as charity for the patients.
Total Knee Replacement
A Total Knee Replacement camp was held in Nepal
Orthopaedic Hospital with the help of Operation Walk
Chicago which is a philanthropic organization from the
USA. Th is team has been doing a great humanitarian
service by operating on hips and knees of poor patients
in the third world countries. Th e services provided
are free of cost for the patients and this team does not
charge anything from the patients.
Th e preliminary pre-visit team had come to NOH on
April 30, 2010 with team leader Dr. Victoria Brander.
Altogether 21 patients were screened and confi rmed
for surgery. They had inspected the conditions of
the operating rooms and also many other logistical
details were discussed before a fi nal date was fi xed for
operating on our Nepali patients.
Th e total knee replacement camp was held for 5 days
from November 10, 2010 till November 14, 2010. A total
of 45 team members including surgeons, anesthetists,
physiotherapists, nurses, administrators and other
para-medics had come for this huge task.

Twenty one patients were operated, which included
twelve bilateral knees and nine unilateral knees. Th is
meant 33 knees were implanted in fi ve days!
Th e surgical camp went very smooth and except for
some medical related hiccups, no real challenge was
seen during the entire camp duration. Surgeons from
the US and from NOH scrubbed together in most
cases and it became a great learning opportunity for
the local doctors and other OT staff . Th e whole camp
was a grand success, the patients and their families
were hugely grateful to the entire team from Operation
Walk Chicago and they were very happy. All patients
are now pain free and can walk independently. Th e
cost for total knee replacement is unaff ordable to most
people in Nepal.
Th e TKR camp was organized in Nepal Orthopaedic
Hospital with team leader Dr. David S. Stulburg and
Dr. Victoria Brander with Dr. Lalit Puri, Dr. Nasim
Rana, Dr. William Gilligan, Dr. Ronak Patel, Dr. Tony
Aram, Dr. William Brander, Dr. Nicole Higgins, Dr.
Kiran Chekka, Dr. Amit Darnule, Dr. Christopher
Cambic, Mr. Odell Woods, Ms. Sophie Sauser, Ms.
Celeste Carlin, Ms. Jane Scanlon and other members.
New Ambulance:
Th e hospital is acquiring a Nissan ambulance very
soon. Th e new ambulance should be delivered to the
hospital by April-March 2011. Th is acquisition is a
fruit of the Matching Grant Project (MG # 73090)
from the Rotary Foundation in conjunction with the
Rotary Club of Fidalgo Island, USA (District 5050)
and host club Rotary Club of Kathmandu West, Nepal
(District 3292).
Th e cost for the ambulance is US $ 34,200 whereas the
Matching Grant Amount is US$ 30,924. Th e diff erence
will be borne by the hospital. The ambulance we
currently have is a TATA jeep which is 11 years old and
the cost for the maintenance has been very high and it
doesn’t have all the equipment an ambulance should
have (oxygen cylinder, fi rst aid kit box, bed trolley). Dr.
Steve Miller from Rotary Club of Fidalgo Island was
instrumental in the realization of the project.
FUND RAISING PROGRAMME FOR NEW PAYING WARD CONSTRUCTION:
Dashain Tihar Lottery:
The Nepal Orthopaedic Hospital staff union had
organized a Dashain-Tihar Lottery. Many tickets were
sold which were worth Rs 50 each and a lucky draw was
done to announce the winners of the lottery.
The winners were given various prizes and the
remaining sum was handed-over to the Medical
Director Dr. Saju Pradhan to keep in the hospital
construction account. NRs. 1,01,111.00 was deposited
in this account. Th is gesture has shown the immense
responsibility our staff members have towards the
hospital.

A “5 KM WALK/RUN/WHEELCHAIR FOR
DISABILITY AWARENESS” in support of Nepal
Orthopaedic Hospital was organized on December
04, 2010 on the occasion of the 19th International
Disability Day with the joint eff ort of NOH, Rotary
Nepal Disability Awareness Campaign, RC Patan, RC
Kasthamandap, RC Kathmandu West and Craft Art
Studio.
Revenue collected from ticket sales for the event:
1. Nepal Orthopaedic Hospital Rs. 5,89,000
2. Rotary Club of Patan Rs. 32,000
3. Rotary Club of Kasthamandap Rs. 20,322
4. Rotary Club of Kathmandu West Rs. 64,000
Th e event became a grand success and Rs. 7,05,322 was
collected from this activity.
Board Member Change
Rtn. Mr. Umesh Shrestha has been nominated to the
member of NOH management committee in the place
of Rtn. Mr. Rajan Raut who just had fi nished his two
years tenure. Rajanji had been a very active member of
the management committee and his contribution will
be much valued in the years to come.

The objective of the event was to mark World
International Day for the Disabled and also to generate
revenue for the building of the Male and Female
General Ward above the administrative block.
Th e event started from Dashrath Stadium Tripureshwor
and ended at Patan Darbar Square. Around 400 runners
and walkers participated in the event. Representatives
from different banks, students from Rato Bangla
School and other schools, medical representatives
from many pharmaceutical companies, hospital
suppliers, Rotarians, disabled people, doctors, nurses,
administrators, other paramedical staff and family
members of hospital staff participated. Th e procession
moved from the Dashrath Stadium and participants
braved the cold weather at six in the morning and ran
till Pulchowk in front of the municipality building
and then again reassembled into a huge crowd waving
banners and placards. Th is group moved in unison
around the streets of Patan and ended in Patan Durbar
Square. Speeches were given by the Medical Director of
Nepal Orthopaedic Hospital, the Chairman of Nepal
Disabled Association, Rotary District Governor of
Nepal, Presidents of various Rotary Clubs. Finally
certifi cate distribution and prize announcements were
made for the participants.

Out-Sourcing of Cleaning Staff
Since its inception, the hospital had been recruiting its
staff s internally. Th e current trend is to hire staff s by
outsourcing. Th e hospital has outsourced six cleaning
staff s from the Nepal European Security Service Pvt.
Ltd. Outsourcing of human resources have proven to be
cost-eff ective because the outsourced staff have proven
to be more effi cient in cleaning the hospital. Th e house-
keeping department has evaluated them positively.
12th Anniversary Celebration:
Th e hospital celebrated its 12th Anniversary with the
chief guest Dr. Sudha Sharma, Secretary-Ministry of
Health, Nepal on August 13, 2010. She distributed
fruits to the admitted patients on the occasion. Cultural
shows were performed by the hospital staff s.
Also international students from Denmark, USA, UK,
Germany, Belgium, Netherland, Canada, Australia,
Austria, and France have come for their internship as
in the past.
Now the hospital is looking more decent and hopefully
we will not be requiring maintenance for at least fi ve
to ten years.
Learning Institute:
Many national and international students (Nursing
students, Medical students, paramedical students
and others) come to this hospital for their clinical
placement. In the year 2010 students from National
Vision Nursing Campus, Nepal Institute of Medical
Science and Technology (NIMST), Chakrabarti Habi
Nursing Campus, Binayak Nursing College, OM
Nursing College, Nobel Nursing College, Shan Nursing
College, St. Xaviers, Rato Bangala school, Nobel
College, College of Bio-medical engineering have come
to this hospital for their practicals.
Many Rotarians, doctors from different hospitals,
representatives from diff erent partner organizations
and hospital staff s were present during the occasion.
Tarring of the Road Around the Hospital Premises:
Th e tarred road in the hospital premises was getting
frayed and old and had many pot-holes. Water and
dust collected in these holes and the whole premise was
looking untidy. Th us the road needed to be maintained
and RS. 7,64,618.14 was spent on new tarring of the
road. Th is was done in October 2010 and no outside
sponsors were involved in fi nancing this.

OUT-REACH SURGICAL CAMPS
Camp in Seti Zonal Hospital, Dhangadi,
Kailali:
Th e camp was held from March 22, 2010 till March
28, 2010 in Seti Zonal Hospital Dhangadi, Kailai.
Th e Handicap International and Nepal Orthopaedic
Hospital had jointly organized the camp. During fi ve
days, 201 out-patients were seen, 42 surgeries were
performed and 75 patients were referred to Kathmandu.
In that camp, twelve team members were involved
which included doctors, nurses, administrative staff
and technical staff s.

3. Lab Assistant Pawan Kumar Chhetri is undergoing
his three years CMLT training in Yeti Health
Science Campus.
4. Senior Staff Nurses Regina Upreti and Anita
Gurung have completed their Bachelors in Nursing
and resumed work at the hospital.
5. Dr. Mukti Timalsena finished his MS in
Orthopaedics from China and has started
resuming duties. He is currently doing spinal
surgeries as well as routine surgeries.
FUTURE PLANS
Addition of a 26- bedded male and female
general ward
Th e hospital has evolved from a small time service
provider to a major service provider in the care of
Nepal’s trauma and orthopaedic needs. Th us more
and more patients are coming out of their own free
will because of the good reputation the hospital
has garnered in these past years of operation. Th e
peripheral health outlets and the central hospitals
are also referring cases to our hospital because of its
high standards in giving care to the patients. Th us the
hospital bed occupancy is always on the higher side.
Sometimes all the beds are occupied and it has been
very diffi cult to admit new patients or discharge old
patients because we have no more beds or the patients
are not yet ready for discharge.
Nowadays, surgery is being done 6 days a week. Most
of the operated patients are poor and they prefer to stay
in the General Ward because it is cheaper. Most of the
times the General Ward is full and poor patients have
to stay in the more expensive private ward or they have
to opt for another hospital where they have a cheaper
general ward. Th us we see this need to add one more
General Ward. Th e best place for that would be above
the administration building.
Th e preliminary design for the addition of 26 more
beds has already been made. Th e addition of these
extra beds would enable us to become a 100 bed
hospital and this would open up more opportunities
for the hospital.
Th is camp was conducted from September 26, 2010 till
September 30, 2010 in the District Hospital, Dipayel-
Silgadi, Doti. Th e Handicap International and Nepal
Orthopaedic Hospital had jointly organized the camp.
During fi ve days camp, 217 out- patients were seen and
53 patients were successfully operated and 47 patients
were referred to Kathmandu for further treatment.
TRAINING AND SEMINARS:

provide better services for the patients. Th e preliminary
design for the addition of laundry has already been
made. Th e estimated cost for extension of laundry,
water treatment plant, water boring and addition of
lift is NRs. 36,51,738. Financial assistance is being
discussed with Stichtung Hulp Voor Nepal for this
project.
Surgical Equipment Upgradation
Surgical equipment also needs to be periodically
upgraded and maintained. In 2011, the need for this
will be assessed and necessary action taken.
Lab Department Extension
Since almost thirteen years, the lab has been providing
essential services to the patients of the hospital in a
very low key fashion. Th e lab does not look that very
impressive if you look at the physical infrastructure
there. However, all tests necessary for pre-operative
assessment and also for orthopaedic and rheumatic
work-up are given by the lab. Th e lab has been adding
analyzers discreetly over the years and computerized
reports have been given to the patients since last year.
Th e room is too small and there is no separate room
for phlebotomy and other tests. Th us a lab expansion
project at a cost of Rs. 1,50,000 has been contemplated
for the future.
Other Important Projects
ICU Set-up/Ventilator Facility
The hospital currently has a post-operative ward
which essentially is a six-bedded post-operative
care unit managed by two nurses round the clock.
Th is needs to be up-graded into an Intensive Care
Unit. For this, patient monitors need to be available for
each bed and possibly a ventilator needs to be in hand
to be able to handle any emergencies. Six bedside
monitors were donated by Operation Walk Chicago
Team in November 2010. We are still working on
how to get a ventilator with some of our contacts
who have pledged to support this. Nurses and doctors
also need to be trained when the time comes to be able
to run this ward.
Th e total estimated cost for the construction is NRs.
1,17,27,793. Preparations are being made to raise
money both internally through fund-raising activities
and also asking for donations from our partners
externally. Th e construction of the ward is planned for
March-April 2011.

BACKGROUND:
Som comes from a typically poor farming family from Sindhupalchowk. He sustained a supra-condylar fracture
of his left humerus while playing with his friends. He was fi rst taken to Hetauda hospital, which is near to his
district, where primary management was done. He was then referred to NOH. He needed surgery and thus was
admitted and operated upon. He comes from a poor family. His father has one goat and a cow. He sells milk to
earn a living and they have a small fi eld next to their house. His family also sells vegetables in the market to earn
some extra money. Th e hospital subsidized his treatment as his family was unable to pay for all the hospital dues.
Som is recovering now. His k-wires have been removed aft er the fracture healed.

Devika studies in grade eight in the local school in Dolakha which is north east of Kathmandu. Her father left the
family three years ago. Her mother works for other people to earn a living. Th ey have six members in the family.
Devika had a right neck of humerus fracture when she fell down accidentally while playing with her friends. She
was brought to NOH and she underwent surgery. Closed reduction and k-wires were placed to reduce and stabilize
her fracture. Th e k-wires were removed aft er four weeks and she is back to school studying. Now she is doing her
daily work and helps her mother to do household work. Partial charity was given for her treatment.

BACKGROUND:
Ashim suffers from a congenital disorder called
clubfoot. He had bilateral congenital clubfeet. Th is
sort of disorder needs treatment from birth. Normally
serial casting to correct the deformity and maybe a
small surgery would have been enough, had he been
seeing a good orthopaedic hospital for his care. Sadly
though, most children do not get adequate screening
and treatment in Nepal. Left untreated, this will lead
to a life of disability and disadvantage for children
aff ected by this disorder later in life. Ashim presented
to the hospital only aft er four years of his life for this
disorder. His mother had heard that NOH was the
best center to treat this disorder.
His father had abandoned his family two years ago.
His uncle brought him to this hospital.He lives in
a joint family having fourteen members. His uncle
works in a private offi ce and earns some money but
it is not suffi cient for his family to sustain a decent
living. Ashim needed a complicated surgery known as
posterior-medial release. Aft er surgery the feet were
casted to keep the ankle and foot straight. Almost three
months were spent on his treatment. Th e result of the
surgery was satisfactory. He now has two straight feet.
He walks normally and he has a bright future ahead
of him. Almost half of the hospital fees was waived
as charity. Clubfoot surgery would have been very
expensive in other hopitlas.

BACKGROUND:
Sujan comes from a very poor family. His father is a daily wage earner and his mother is a housewife. Altogether
they are fi ve in the family. Th ey live in a rented house in Kathmandu. His mother also has a knee problem.
Th eir fi nancial condition looks very poor. Sujan sustained a radius and ulna fracture in his left forearm. He was
admitted and a simple surgery known as Rush-nailing was done. Compared to other hospitals, the cost of care is
very aff ordable at Nepal Orthopaedic Hospital. Inspite of this, many patients need fi nancial assistance. Almost
half of the hospital bills were waived as charity for this hapless family. Sujan’s Rush nails have been removed and
his bones have healed very well. He is back in school studying hard to get good results.

Raj Kumar is from Dhading. He is a student of grade
fi ve He has two sisters and one brother and they all
are studying in government school. His father is a
farmer and mother helps him to farm. Th ey have a
small farm and income from that is not suffi cient for
their livelihood.
Raj Kumar had a supra-condylar fracture of his right
humerus and he required surgery. His operation was
successful and the pins have been removed from his
arm aft er the bones healed. He has now resumed
studies at school. More than half of the hospital bills

Tika Ram studies in grade two in the local school in Sindhuli. His father has some health problems so he cannot
work to earn a living. His mother is twenty eight years old and works daily as a labourer to earn for their family.
He fell down from a tree and sustained a complex open fracture of his right forearm bones. He was taken to a
local hospital where primary management was done. Th e bones were protruding out of the skin. Th e wound got
infected and thus he was referred to NOH. Th is case was very complicated and he needed a series of surgeries
to get him back to normalcy. He stayed for thirty four days in the hospital. He needed K-wire fi xation, fl ap
separation, fl ap inset and several debridements to clean the dirty wounds. His hand was luckily saved by our
skilled team of doctors. His hospital bills came to nearly ninety thousand rupees and they were not able to pay
for it. Almost ninety percent of the bills were waived as charity. What was important was that our hospital had
saved a young boys limb.

Tek Bahadur is a painter who works on a daily basis to earn a living for his wife and two daughters.He fell from
a two storey building while he was painting a wall. He was fi rst taken to Teaching Hospital for treatment but as
there was no vacant bed he was referred to our hospital.
He sustained communited fractures of his left distal tibia and fi bula. Th is was a complex fracture requiring
treatment with the Ilizarov frame. Th is is a costly procedure even by NOH standards. His contractor deposited
Rs.42, 000.00 at the time of admission. He was successfully treated but he had to stay in hospital for nearly six
weeks. During hospital stay, his wife and contractor didn’t come to visit him. He didn’t have any money for food
so the hospital provided food for him as well. At the time of discharge charity was provided to cover for the rest
of his bills. Now his implant has been removed and he is able to resume his previous work.

BACKGROUND:
Hajari is from the far-western part of Nepal which happens to be an under-developed region of Nepal. Her husband
had already died a few years ago. She fell down from a height of twenty fi ve meters. and sustained injuries to
multiple parts of the body. She was initially given primary care locally. Later on she was fl own to Kathmandu
and then a CT scan was done to rule out head injury. She was later referred to NOH aft er head injury was ruled
out. Her son-in-law is looking aft er her and he had brought her to Kathmandu. Her son-in-law works for the
Nepal Army and he deposited some money during admission but he said he could not clear all the dues. Th us, Rs.
8,265 was given as charity. She was treated for a left tibial plateau open Gustillo III A fracture. An Ilizarov device
was used in her case. She also had a mandible fracture which was treated conservatively. Her fracture has healed
completely and the frame has been removed. She is back in her village and is resuming normal activities.

one son and three daughters. One day he fell during an alcoholic stupor from his balcony and sustained injury
to his right hip region. He was initially taken to Bir Hospital but they were unable to manage him there, so he
was referred to NOH. He was operated for his inter-trochanteric fracture and he is doing well during his regular
follow-ups. Charity had to be processed for him as his wife was not able to aff ord the full cost of care.

BACKGROUND:
Tara Devi Rokka and her husband both work in their farm. Th ey have a small farm where they grow vegetables
and rice. She has fi ve members in the family. She has three children. Th e eldest daughter had to stop her studies
because of poor fi nancial condition. Other two children go to the local school. She fell from a height of eight feet
and sustained a Monteggia fracture in her right elbow. She also had a fracture of her radial head. She needed to be
operated and plating was done for the ulna fracture and the radial head was excised. Post-operatively she had no
complications and now she is doing well. Charity had to be processed so that she could clear all her hospital bills.

Hari Bahadur works as a daily wages worker. He has seven members in the family. His three children study in
school and his mother also stays with him. His wife also works as a daily wages worker to support him fi nancially.
Th eir income is Nrs.150-200 per day. He sustained a fracture to his right olecranon process of the ulna and he
needed to be operated. A tension band wiring procedure was done which addressed his problem. Post-operatively
he is doing extremely well. He had to be processed for charity to clear all hospital bills.

and the bone was fi xed with a plate and screws. She has recovered fully and she has gone back to work. Partial
