SUBJECT ADDENDUM TO MC No. 1, 5.2015 and MC No. 4, s.2016, RE:FILING AND SUBMISSION OF REVISED STATEMENT OFASSETS, LIABILITIES AND NETWORTH (SAlN) FORM FOR CY2014 and 2015; AND AMENDMENT OF MC No. 55, s.2013, RE:CREATION OF REVIEW AND COMPLIANCE COMMITTEE ONSALN
Compliance to the timely submission of the Statement of Assets, Liabilities and Networth(SALN) is one of the requirements for the Grant of Performance-Based Bonus (PBB).Administrative Department Manager, Regional Managers/Department Managers for UPRIISand MAR liS, Project Managers and IMO Managers are hereby authorized as AdministeringOfficer for SALN of personnel within their area of jurisdiction.
The Committee to review the compliance procedure in the filing and submission of SALN perCSC MC No. 10 s.2006, for the Central Office is as follows:

Regional Managers/Department Managers/Project Managers are hereby directed to createtheir respective committee and furnish the Central Office copy thereof. The Committee shallperform the following functions: -1. Evaluate the SALN Form submitted to determine whether it was submitted on time,
accomplished completely and proper in form;
• ;
2. Establish compliance procedures for the review of SALN.
Please be reminded that failure of an official or employee to file SALN is punishable underRule IV, Section 52 (8) (8) of the Uniform Rules on Administrative Cases in the Civil Service(CSC Resolution No. 99-1936 dated August 31,1999). of Suspension for one (1) month andone (1) day to six (6) months for the first offense, and dismissal from the service for thesecond offense. (Sec.4 of CSC Resolution 06-023-1, February 1, 2006.)
