Machines (as we think of them) are all systems of machines.

We like to think of our organization as a well-oiled machine. Clear, straight lines of communication.

When we begin to investigate we find something much different and often disconcerting.

People need to get things done. They are very creative about the processes. “The process” often differs little from that used by the very first person in the job. “That’s the way we do it!”

Responsibility for Process often lies with those who perform it. Accountability is for delivery (count, rate) performance.
