available to improve their health regardless of race, religion, or socioeconomic status. Each child should be given equal opportunity to participate in activities that will improve their lifestyle helping them to live a healthy, longer, disease free life.

Assessment of External & Internal Environment and name of
Service/AgencyIn the United States 18% of children are
obese and childhood obesity has tripled in one generation (IOM, CDC, & NIH, 2012).
In most urban and suburban areas there are health clubs available but they focus on adults. The My Healthy & Active Child Center would be exclusively available to children.
Obesity is a serious health risk that effects children and should not be ignored

Job DescriptionsNurse Director: Supervises overall functioning of
the clinic, applies for grants, hires staff, oversees and documents outcomes. Handles financial coordinator duties, as well.
Lead RN: Provides support to the dietary expert and fitness coordinator. Collection of physical data on all participants including height, weight, and body mass index (BMI). Will also assess participant’s knowledge and assist in the development of individualized plan, and continuously evaluate progress.
Dietary coordinator: Provide education regarding healthy nutritional choices and meal preparation to participants. Develop simple reference pamphlets and brochures for participants to use at home.

Garden Supervisor: Organizes garden area in the spring, supervises children during planting, growth and harvest of planted food.
Fitness Coordinator: Coordinates activity, instructs participants on proper exercise technique and teaches about the benefits of exercise. Will also be responsible for the development of creative, fun, interesting weekly special activities.
Finance Coordinator: Coordinates and balances the budget, track expenses, and create advertisements for the center.

FUNDS DISTRIBUTIONMoney collected from grants will be used to
pay for operational costs, wages, supplies, equipment, and food.
A magazine subscription will be supplied for each family called, Chop Chop Cooking Magazine. This magazine is based on healthy, easy cooking instructions and ideas that children can prepare. The idea is that children who know how to cook and feed themselves will not have to rely on fast food and processed meals, and families who cook and eat together have healthier lifestyles overall (Kayal, 2013).
Some grant money will be used to support low income families with funds to assist with healthy food purchases and assistance to pay for organization fees to participate.

Structure of the OrganizationSchedule of activities:
Monday and Thursday- 5:00p.m. -cooking class with information, recipes, and ideas from the dietary coordinator.
Monday, Tuesday, Wednesday, Thursday- 6:30p.m. -organized exercise classes lead by the fitness coordinator, such as aerobics, yoga, and zumba.
Friday- family fun night with organized sports and activities for the whole family, such as swimming, soccer, volleyball, and baseball.
Daily-during hours of operation- open exercise and play.
