Take Control 

At Take Control we believe that living well is within everyone’s reach. We’re a healthcare company that provides health coaching services FREE of charge to anyone covered by the Montana University System’s health care plan, with any of the following conditions*

  Diabetes -Type I, Type II, Pre-diabetes, or Gestational Overweight (High BMI) Tobacco User – Smoking, chewing tobacco, cigars, pipe High Blood Pressure (Hypertension) High Cholesterol (Hyperlipidemia)

*Participants must meet one or more of the listed criteria to qualify for our services.

Take Control Enrollment Criteria

  Cholesterol greater than 240 (CHOL > 240) Triglycerides greater than 200 (TRIG > 200) “Bad” cholesterol LDL greater than 150 (LDL > 150) “Good” cholesterol HDL less than 40 for men and less than 50 for

women (Male < 40) or (Female < 50)

Diabetes Fasting glucose count greater than 125 (GLUC > 125) Blood pressure greater than 140 systolic (top#) - or greater

than 90 diastolic (bottom #) (Systolic > 140) or (Diastolic > 90)

Body Mass Index greater than 24.99 (BMI > 24.99)

Take ControlBenefit Summary

• Monthly health coaching

• Up to three visits with your primary health care provider covered at 100%

• Fitness center or fitness class reimbursement

• Reduced-cost medication waivers for qualifying health conditions

• Assistance with tobacco cessation

• A $100 award is available after months 6 and 12 to assist in offsetting expenses related to your life style improvement.

WellBaby Pregnancy Support

Benefits: For managed care plan members: All out-of-pocket costs waived

for services provided by an in-network physician

Routine prenatal office visits

High risk nurse case management based on three strategically-timed risk assessments

MUS insured plan members eligible for 4 free counseling sessions per plan year

Must be an in-network provider Contact your insurance administrator or look at

administrator’s website for list of providers

After first 4 visits, 25% coinsurance on Traditional Plan or $15 copay on Managed Care

Wellness Champions

What is a Wellness Champion? Someone with a sincere desire to support healthy lifestyle

and disease prevention strategies in the workplace. This person has a strong commitment to health (regardless of where he/she is on the path towards optimal well-being) and a desire to help others achieve success as well.

What are the duties of a Wellness Champion? Help create a culture of Wellness on campus

Pass along information to colleagues and encourage participation in Wellness programs

May be asked to assist with room scheduling, walking groups, workshops, etc.

We accept applications each fall semester.
