Group Term Life InsuranceChoose from a minimum of $10,000 to a maximum of $500,000 (in $10,000

increments) for both yourself and/or spouse. Rates based on age.

Dependent Child(ren): Age 14 days to 6 months: $1,000 Age 6 months to 20 years: ($2,500, $5,000, $7,500, $10,000)
