General Requirements

Every person operating a motor vehicle shall possess, at all times while operating such vehicle, a license/permit valid for the equipment being operated. - Licensing per Service regulation for military personnel and State regulations for civilian personnel, to include contractors. - Operator must present the license/permit to the GDA upon request. - Failure to do so will result in the immediate prohibition of the operator to operate motor vehicles.

18.A.02

Inspections, Tests, Maintenance, and Repairs

Before initial use, vehicles not otherwise inspected by State or local authorities, shall be inspected by a qualified mechanic and found in safe operating condition and in compliance with all required published vehicle safety standards. The inspection shall be documented and available for inspection on the work site. This is a one-time inspection.

18.A.03.b

Back-up Alarms

All self-propelled construction and industrial equipment, whether moving alone or in combination, shall be equipped with a back-up alarm.

18.B.01.a

Commercial cargo vehicles such as pick-up trucks, utility cargo/tool trucks, and flat bed cargo trucks intended for use on public highways with a normally clear view through the rear window are not required to have back-up alarms.

The removal or disabling of any back-up alarm is strictly prohibited.

Guarding

All belts, gears, shafts, pulleys, sprockets, spindles, drums, flywheels, chains, or other reciprocating, rotating, or moving parts of equipment shall be guarded when exposed to contact by persons or when they otherwise create a hazard.

18.B.03.a

Brake Systems

All vehicles, except trailers having a gross weight of 5,000 lbs or less, shall be equipped with service brakes and manually operated parking brakes.

Seatbelts and anchorages meeting the requirements of 49 CFR 571 shall be installed and worn in all motor vehicles (installation and usage on buses is optional).

FOPS 18.B.12 Falling object

protective structures (FOPS).

a. All bulldozers, tractors, or similar equipment used in clearing operations shall be provided with guards, canopies, or grills to protect the operator from falling and flying objects as appropriate to the nature of the clearing operations.

b. FOPS for other construction, industrial, and grounds-keeping equipment will be furnished when the operator is exposed to falling object hazards.

c. FOPS will be certified by the manufacturer or a licensed engineer as complying with the applicable recommended practices of SAE Standards J231 and J1043.

Operator Restrictions The use of any portable headphones, earphones,

or other listening devices (except for hands–free cellular phones) while operating Gov’t. motor vehicles or contractor motor vehicles (being used on Gov’t. projects) is prohibited.

Operators of government motor vehicles or contractor motor vehicles (being used on government projects) shall not eat, drink, or smoke while the vehicle is in motion. 18.C

GPS

GPS systems shall be mounted within the vehicle so that they do not create sight hazards for the operator. Programming of dashboard GPS systems while driving is prohibited. The use of non-mounted GPS systems may only be used by the vehicle operator while the vehicle is in a stopped position.

18.C.01.d

Transportation of Personnel

The number of passengers in passenger-type vehicles shall not exceed the number that can be seated.

No person will be permitted to ride with arms or legs outside of a vehicle body, in a standing position on the body, on running boards, seated on side fenders, cabs, cab shields, bed of the truck, or on the load.

Motor Vehicles

Motor vehicle is defined as a sedan, van, SUV, truck, motorcycle, or other mode of conveyance intended for use on public roadways. This includes construction equipment that is driven on public highways. 18.E.01

Machinery & Mechanized Equipment

Machinery & mechanized equipment is defined as equipment intended for use on construction sites or industrial sites and not intended for operations on public highways.

18.G.01

Machinery & Mechanized Equipment

Before any machinery or mechanized equipment is placed in use, it shall be inspected and tested in accordance with the manufacturer’s recommendations and requirements of this manual and shall be certified in writing by a competent person to meet the manufacturer’s recommendations and requirements of this manual.

18.G.02

Records of tests and inspections

The Contractor shall keep records of tests and inspections. These records shall be made available in a timely manner upon request of the GDA and, when submitted, shall become part of the official project file.

18.G.02.a

Notice of Entry

The Contractor shall provide the GDA ample notice in advance of any equipment entering the site so that he/she may observe the Contractor’s inspection process and so that spot checks may be conducted.

18.G.02.d

Equipment Modification

No modifications or additions that affect the capacity or safe operation of machinery or equipment shall be made without the manufacturer's written approval.

18.G.03

Safe Operating Condition

Whenever any machinery or equipment is found to be unsafe, or whenever a deficiency that affects the safe operation of equipment is observed, the equipment shall be immediately taken out of service and its use prohibited until unsafe conditions have been corrected.

Caught Between Two employees were attempting to adjust

the brakes on a backhoe. The victim told the backhoe operator to raise the wheels off the ground with the front bucket and the outriggers, put the backhoe in gear at idle speed and step on the brakes. The victim then crawled under the machine and began to adjust the brakes. There was a 36-inch space from the ground to the drive shaft. Five minutes later another employee discovered the victim limp under the backhoe with the hood of his rain jacket wrapped around the drive shaft. The employee's neck had been broken by the jacket wrapping around the backhoe drive shaft.

Applicability. The requirements of this section are in addition to other requirements identified in Section 18 and are applicable to rock, soil, and concrete drilling operations.

Drilling Equipment

18.H.01

Authorized Operator

Drilling equipment shall

be operated only by qualified (by training and experience) personnel who are authorized by their respective employer to operate subject equipment.

18.H.02

Site Survey

Prior to bringing earth drilling equipment on the job site, a survey shall be conducted to identify overhead electrical hazards and potential ground hazards, such as contact with unexploded ordnance, hazardous agents in the soil, or underground utilities. 18.H.03

Drilling AHA Requirements The AHA for an earth drilling activity will not be

accepted unless: a. It contains a copy of the MSDS for the drilling fluids, if

required; a. It meets the requirements of 01.A.13; and a. It indicates that the site layout plan specified in 18.H.03

will become a part of the analysis, and will be covered at the preparatory inspection (pre-activity safety briefing), when the plan has been completed.

18.H.04

Training a. Members of drilling crews shall

be trained in: (1) The operation, inspection, and

maintenance of the equipment;

(2) The safety features and procedures to be used during operation, inspection, and maintenance of the equipment; and

(3) Overhead electrical line and underground hazards.

b. This training will be based on

the equipment operating manual and the AHA.

18.H.05

Section 18.I All Terrain Vehicles (ATVs)

Every ATV operator shall have completed a nationally recognized accredited ATV training course (such as provided by the Specialty Vehicles Institute of America or by in-house resources that have been certified as trainers by an accredited organization) prior to operation of the vehicle. 18.I.02

Operator Qualifications

The operator must pass an operating skills test prior to being allowed to operate an ATV. Proof of completion of this training shall be made available to the GDA upon request.

Section 18.J Utility Vehicles

For the purposes of the section, utility vehicles are defined as specialty vehicles designed to perform off-road utility tasks such as passenger and cargo transportation (e.g., Rangers, Rhino, MGators, Gators, Mules, etc.).
