SIPD621 MAJNOON SITE INVESTIGATION Morning Update HSE 08.04.14: 00002400hrs. STOP WORKS for Drilling activities since 07.04.14 @ 1350 HSE Weekly Walkabout postponed due that CSR vehicle breakdown. PSD escorted CTR’s PM and CSR for the jointly MOF site visit. After the site visit it was concluded by both parties that personnel will struggle to be under the heat during summer months with body armour. Extra precautions to be implemented. CTR had a problem (not disclosed yet) with the PTW for PCPT Rig. Unfortunately CTR didn’t report it CSR to seek for assistance. 09.04.14: 00000700 hrs. CTR to complete incidents investigations and undergo inspection from CPY’s HSE. CPY HSE and CSR to visit CTR’s Camp for equipment inspection related to the incidents and advice on closure in order to go back operational again. HSE Weekly Walkabout to be conducted.  ANTICIPATED  EVENTS  CTR  

Ø CTR  showed  reluctant  to  report  on  time  any  HSE  issues  or  work  problems  encountered  during  the  last  week  (e.g.  incidents  with  bailer  in  Drill  Rigs  leading  to  the  Stop  Works,  problems  obtaining  PTW,  etc).  CSR  discusses  this  problem  with  CTR’s  PM  and  he  committed  not  to  happen  again  

Ø HSE  Weekly  Walkabout  to  be  completed  once  CSR  vehicle  is  repaired    Ø CTR  to  amend  and  submit  again  the  EUPHRATES  Borehole  and  PCPT  Logs  Ø CTR  to  amend  and  resubmit  DPRs  from  01.04.14  to  07.04.14  Ø CTR   is   addressing   HSE   and   TECHNICAL   findings,   reported   by   CSR   back   to   CTR   on   01.04.14   (e-­‐mail  

“SIPD-­‐621_MAJNOON_20140401_1300-­‐  HSE-­‐Technical-­‐Admin  pending  issues  –  HG”)  Ø CTR   to   submit   pending   Survey   Reports   for   all   location   since   beginning   of   Fieldworks.   CTR   has   been  

submitting  Actual  Coordinates  and  Elevations  of  completed  locations  only  and  after  several  requests,  rather  than  producing  the  reports  as  Fieldworks  progress.  

CPY  

Ø Joint  visit  with  CTR’s  PM  on  08.04.14  to  JETTY  &  CONSTRUCTION  CAMP  Sites  to  assess  logistics  and  rig  access  to  proposed  locations.    

Ø CPY  to  provide  Site  Instructions  to  CTR  for  at  least  1  week  ahead  to  allow  CTR  to  plan  and  to  avoid  any  standby  times.  

Ø CPY  to  instruct  CTR  which  locations  will  require  down-­‐the-­‐hole  instrumentation  and  tentative  dates  to  mobilize  the  Geophysical  crews.      

PLANNING  Ø CSR   and   CTR’s   PM   joint   visit   to   MOF   on   08.04.14   showed   that   access   to   locations   is   better   than  

expected,  however  1  or  2  locations  will  be  offset  due  to  marshes.  Ø CTR  needs   extra   SOW   to  plan   ahead  properly   and   avoid   standby,   and  use   the  WELL  PADS   Site   as   a  

bumper  in  case  no  other  project  has  been  defined  in  order  to  carry  on  works  there.  CPY  is  providing  new  SOW  for  MOF  and  CONSTRUCTION  CAMP  Sites.  

Ø CTR  submitted  Gannt  Diagram   including   resources,   locations  of  Rigs  and  PTW  coverage.  To  be  done  before  every  Weekly  Meeting.  

Ø It   is   anticipated   that   with   the   new   arrival   of   an   assistant   to   PM,   paperwork   and   reporting   will   be  submitted  in  time  and  form.  

 ADMIN  

Ø CSR   submitted   New   DPR   Template   to   CTR   for   discussion.   Due   to   poor   Admin   skills,   CTR   hasn’t  submitted   the   New  DPR   Template.   As  well   they   are   very   disorganized  with   their   internal   reporting  (Drillers   to  PM  and  Surveyors   to  PM.  The  only   team   that   reports  efficiently  back   to  PM   is   the  PCPT  track).  CSR  assists  them  with  how  and  what  to  report,  as  well  as  with  a  new  Reporting  template  for  CTR   to  use   internally.   The  problem   is  not   lack  of   resources  but  poorly   administrated,   and   therefore  struggle  to  plan.

Ø CSR   and   Jaydeep   implemented   filing   system   for   received   data   from   CTR.   During   this   week   all  transferred  data  will  be  available  from  a  link  .  

Ø CPY  to  revert  with  signed  Site  Instruction  for  proper  filing  and  follow  up.      

DATA  RECORDING  &  TRANSFER  Ø Submitted  data  starts  to  show  consistency  between  different  Engineers  producing  the  reports.  Ø CTR  has  to  resubmit  EUPHRATES  data  as  its  not  properly
