We are excited to share our 2011 Appleton Medical Center Foundation annual report. Within these pages, you’ll see how the work of the Foundation allows patients to heal with dignity through major projects, as well as small gifts thatmake a big impact.

We are incredibly lucky in the Fox Valley and surrounding communities to have access to the best equipment, most recent research, great minds, and modern medicines necessary to provide outstanding healthcare. Your contributions tothe Appleton Medical Center (AMC) Foundation help make that possible.

But there’s more to healthcare than exams, equipment and medicine. Healthcare is about the human spirit and bringing people together to help one another. That’s where the Foundation comes in. Your contributions to the AMC Foundationprovide the financial and emotional support required for our friends, families,and neighbors to take advantage of the quality treatment AMC can provide.

Our mission is really two-fold:

1. Financially support the Hospital to make exceptional healthcare possible in our community.

2. Remove obstacles – tangible and otherwise – to healthcare so each person’s journey to better health is an experience that uplifts and encourages rather than drains and discourages.

Your Gifts in ActionMirror, Mirror on the Wall

Patients receiving chemotherapy have benefited from the opportunity to meet with ThedaCare Cosmetology Coordinator, Lisa Kellnhauser, to receive headwear and wigs at no charge. During the appointment, however, Lisa must shave any remaining hair to provide a proper fit for each wig. Watching in the mirror can bea very emotional and overwhelming experience for most patients.

In 2011, Appleton Medical Center Foundation provided funds to purchase a professional cosmetology chair, which allows Lisa to turn the patient away from the mirror to perform the makeover. Then, she is able to turn the chair back around and provide a big reveal, using the mirror to demonstrate and teach.

“This chair has made such a huge difference,” Lisa shares. “Now, people get so excited and they actually enjoy the experience. Simply being able to turn the chair around has created a whole new atmosphere of celebration.”

More than 130,000 people were admitted for treatment at Appleton

Medical Center in 2011. That is nearly twice the population of Appleton, Wisconsin, where AMC is located.

Last year, nearly 1,000 people donated more than half a million dollars* to support Appleton Medical Center

Foundation programs and services.*Actual number = $598,117.

AMC’s 550 volunteers contributed more than 47,000 hours of service in 2011,

saving the Hospital nearly $1 million*.*$979,950 was calculated using the 2011 estimated value of volunteer time established by independentsector.org.

The numerous ways and countless reasons people are moved to contribute to this cause are truly inspiring. Appleton

HiGH Flyin’ GOlF OuTinGThe 2011 High Flyin’ Golf Outingbrought 163 golfers togetheron July 18 for a fun day of food, golf, and on-course events. Due to an overwhelming response, the golf outingwas expanded this year to include an 18-hole option at North Shore Golf Club in Menasha as well as a 9-hole option at Riverview Country Club in Appleton. Those playing nine holes also enjoyed a wine tasting presented by Certified Wine Specialist Kate Merryfield from General Beverage Sales Co. and cooking demonstration by Chef Larry, owner of Big Tomatoes Restaurant in Green Bay and Executive Chef for the ThedaCare Hospital Division. Both groups of golfers started the day with lunch and ended with an hors d’oeuvre receptionand auction.

The event raised $78,474 to be equally distributed between Appleton Medical Center Foundation and Theda Clark Medical Center Foundation programs and services.

Cardiac and Pulmonary rehabilitation are serious programs vital to patient recovery. That’s why 79 people gathered on Saturday, February 26, 2011, for Appleton Medical Center Foundation’s “serious” fundraiser: laughter is the Best Medicine. $18,830 in net proceeds from this relaxing and fun-filled event benefit patients with insufficient insurance coverage or who lack the financial means to participate in necessary cardiac or pulmonary rehab at AMC.

Held at Trolley Square Waterfront Banquets in Appleton, the event included an hors d’oeuvre buffet, social and raffle, as well as entertainment by ComedySportz-Milwaukee. In addition, cardiac rehab patients Jeffrey Butz and Phil Palmer shared their stories of recovery and gratitude, helping attendees understand how important the Healing Hearts Fund and Pulmonary Rehabilitation Fund programs were in their own situations.

“I was so appreciative that I didn’t have to ask to receive the fund. It was offered to me,” explained patient Jeffrey Butz. “If it wasn’t for the Healing Hearts Fund, I would not have had cardiac rehab after my valve surgery.”

Phil Palmer agreed. “I am very grateful for the Healing Hearts Fund,” he shared. “I feel like I could never repay what cardiac rehab did for me.”

The Jayme reetz nursing Scholarship

The Jayme Reetz Scholarship is presented each year to AMC employees continuing their education to become a BSN, RN, nurse practitioner or clinical nurse specialist.

Congratulations to recipients Kim romenesko ($1000) and Dacia Hermes ($500).

The Sarah Schmitt Scholarship

This scholarship is offered in memory of clinical technician Sarah Schmitt to AMC Emergency Services Department staff and their dependents to continue education toward a BSN, RN or nurse practitioner degree.

The Sharon Stanlik nursing Scholarship

The Sharon Stanlik Endowment Fund awards this scholarship to a student pursuing a degree and/or certification in the nursing field or to a nurse seeking professional development.

Every gift – big or small – makes an impact. While donors generously contributed nearly $600,000 to Appleton Medical Center Foundation in 2011, individual gifts varied greatly. Taken together, the funds gain the power to make a difference and become a recipe for success.

While it would be impractical to share every story of where the money went in 2011, we hope you enjoy the real life impact described in the “your Gifts in Action” stories sprinkled throughout this report.

Make an immediate impact or provide long-term supportin a variety of ways:

GiFTS OF CASH. Cash gifts of any amount can be made in memory or honor of a loved one, if desired. These unrestricted funds often make a difference immediately by supporting programs, education, and research.

GiFTS OF APPrECiATED ASSETS. Non-cash assets, such as common stock, may offer additional tax advantages and also help avoid capital gains tax on the appreciated value.

TruSTS & AnnuiTiES. Significant tax advantages make charitable trusts and gift annuities an attractive option. With a trust, the donor receives a financial return on the donation throughout his/her lifetime.

EnDOWED nAMED FunDS. Donors can establish an endowment fund with $10,000 or more (payable over 3-5 years) to support a particular program or initiative. Only tapping into the interest, the principal remains intact, ensuring that important programs will be sustained into perpetuity.

GiFTS By Will. Inserting the sentence “I bequeath $_____ or _____% of my estate to Appleton Medical Center Foundation” may cost you nothing today, but will certainly help serve our mission in the future. A willed gift can also help reduce the donor’s estate taxes.

In 2011, Appleton Medical Center Foundation met our fundraising goals through successful events, charitable sponsorship relationships, and generous individual contributions. While the year proved difficult for investments, we continue to be positioned well for long-term success. In addition, responsible management of available funds maintained net assets at a nearly identical level to last year, which was up over the year prior. Finally – and perhaps most importantly – the Foundation provided grants and community support to a variety of worthy causes, forwarding our mission and providing nourishment for future growth and prosperity.

This number represents the funds raised by Appleton Medical Center Foundation through new contributions. This number does not reflect pledge payments on a capital campaign.

Our professionally managed investment portfolio reflects the condition of the overall economy in 2011. Long-term, we continue to average appropriate gains.

The Foundation’s Board of Directors uses approximately five percent of the total unrestricted assets to fund programs, new equipment, and initiatives outside of capital campaigns each year.

ThedaCare’slearning Center forCollaborative Care

The Collaborative Care Unit on the fifth floor serves as a centralized location to train nurses and other clinical staff in the methodology and practice of the Collaborative Care model. In fact, nurses spend their orientation time at this Learning Center in intense preparation. (See “Role Models for Care” on page 9 for more information.)

Appleton Medical Center Cardiac Program

The heart care program at Appleton Medical Center just got better. With the help of a $35,000 grant from the Foundation, a new piece of equipment was purchased that is designed to eliminate risky, abnormal heart rhythms, among other uses. The technical name for the equipment is a Cardiac Ablation System. The two experts who use it and the many patients who benefit from this system call it wonderful. We are the only medical facility in the area that has this equipment and patients and families no longer need to travel out of town to receive this type of medical care.

ThedaCare Cancer Care

Appleton Medical Center Foundation provides $200 gasoline gift cards for patients receiving treatment at the Martha Siekman Cancer Center. The gift cards are given to patients who have limited financial resources and have to come to the Siekman Center from outside the Fox Cities area for repeat visits and treatments. This amount is small, but the impact is huge for people who have too much on theirminds already.

Empower Me! Camp

The Empower Me! Summer Camp is a program designed to provide a fun and educational experience for youth on the autism spectrum. The camp offers programs for Kindergarten through 2nd grade, grades 3-5, and grades 6-8. AMC Foundation funded $11,435 in scholarships for campers who could not otherwise afford to attend the camp. In addition, the Foundation provided $380 for music supplies to enhance the camp experience.

Thanks to the generosity of donors who support the Appleton Medical Center Foundation, our Board of Directors is able to award grants each year to the Hospital and local community organizations for needed equipment and/or to establish new procedures.Grants awarded in 2011 include:

ThedaCare has taken a national leadership role to develop a better way to care for patients in the hospital setting. It’s called Collaborative Care.

In this model of care, the patient, his or her family, and the entire clinical care team work together to create a treatment plan designed specifically for that patient. In fact, the physician, nurse, and pharmacist are all present at the patient’s bedside in order for information to be given and received in a group discussion, rather than a series of one-on-one conversations or shared charting notes. Since implementing Collaborative Care at Appleton Medical Center, ThedaCare has documented shorter lengths of patient stays, lower costs per hospital stay, and improved patient satisfaction.

This model of care frequently puts the nurse in a lead role for communication since nurses often have the most contact and, therefore, the strongest relationship with each patient. Such an ambitious effort requires preparation and training.

Beginning with the 2011 graduate nurses, ThedaCare’sLearning Center for Collaborative Care will provide an evidence-based process for nursing orientation. It’s an innovative model where new nurses and clinical staff members receive focused training from clinical experts, guided by an advanced practice Learning Chair.

Appleton Medical Center Foundation is proud to support the new Collaborative Care Learning Center through a $238,000 grant paid out over the next three years. In addition to funding the Learning Chair position, the grant will help expand the unit and convert the facility into a classroom environment with proper equipment and upgrades to patient rooms, including live stream video where students can watch the Collaborative Care model in action without intruding on patient space. This professional development will strengthen the care delivered for every patient we serve.
