Why revised MnCHOICES application?

To assist our conversation as we transition from existing computer applications

• MnCHOICES Assessment or MnA

• MnCHOICES Support Plan or MnSP

• For the purpose of the launch: revised MnCHOICES application

• When MnA and MnSP are retired: This new computer application will be referred to as MnCHOICES

Terms: Lead agencies and their contracted agencies

1. Lead agency: County

• Contract: Contracted case management agencies work on behalf of the county

2. Lead agency: Tribal nation

3. Lead agency: Managed care organization

• Contract: Delegate agencies work on behalf of the MCO. For example, counties and other community organizations.

• When a county is in contract with an MCO, then the MCO is the lead agency and the county is a delegate of the MCO for those members enrolled in managed care7

FEI Systems and state-wide launches

• Statewide launch is FEI Systems practice

• Number of clients launched: 46 states, counties and federal agencies

• Numbers of users on the day of launch: 200 to 4500 users

• Estimated MnCHOICES user base: 5000 users

• Performance testing: FEI Systems will complete performance testing prior to launch

Building the revised MnCHOICES application

MnCHOICES Team has contracted with FEI Systems to build the revised MnCHOICES application

Assessor role: To view a person’s past support plan(s), complete acomprehensive assessment and assessment summary (CSP)

Case manager role: To view past assessment content, to create andcomplete a support plan (CSSP)

Care coordinator role: View past assessments and support plans andis able to create an health risk assessment and its support plan
