FACTORS. . .
Factors leading to the magnitude of the gas leak include:
Storing MIC in large tanksPoor maintenance after the plant ceased MIC production at
the end of 1984Failure of several safety systems (due to poor maintenance)
Safety systems being switched off to save moneyincluding the MIC tank refrigeration system which could
have mitigated the disaster severityThe problem was made worse by the mushrooming of
slums in the vicinity of the plant, non-existent catastropheplans, and shortcomings in health care and socio-economic
rehabilitation.

SH&E - EVERYONES RESPONSIBILITY
It is everyone's responsibility to work safely.Whatever observations you make of the people working around you, you
must be comfortable about the situation you are witnessing. If you witness an unsafe act, and do nothing about it, then effectively you
have just become party to that act.If you see and ignore an unsafe act, then you have set a standard that others
may believe is acceptable. If you do an unsafe act in front of your workmates, then you condone the
act and have set a precedent for others to follow.
Remember, the standard you walk past, is the standard you get.
Dont walk past. Stop, and correct the behaviour.
