2. Welcome to Milford Power, part of the GDF SUEZ Energy International fleet in North America To ensure the safety of you and all personnel during your visit, we ask that you watch this slide show and take a short exam as you follow along. At any time during this show, please feel free to stop and ask our operations group if there is anything you do not understand. NO SMOKING Smoking is PROHIBITED in all areas except under the awning just outside of the maintenance shop, at the north end of the Administration Building 6. Minimum Personal Protective Equipment (PPE) Required At All Times Hard Hat Class E Long Sleeved Cotton shirt Safety Shoes with Steel or Composite toe protection Safety glasses with side shields If you require prescription glasses then you may; Wear safety prescription glasses with side shields Wear safety glasses or goggles over your prescription glasses. 7. Other Forms of PPE (depends on the type of work being done) Hearing Protection Goggles / Face Shield Chemical Boots, Gloves & Apron Respirator Electrical Safety Equipment gloves, safety shoes, sleeves, etc. 8. Motor Vehicle Operators A valid drivers license is required for any persons who may be operating a motor vehicle on Milford Powers property. Your license must be valid for the type of vehicle being operated. Site speed limit 15 mph 9. Hazardous Areas 10. Gas Control Areas The gas areas are defined as Zone 2 where gas may become present in the atmosphere as the result of an abnormal plant event. The zones are clearly marked by red lines on the ground. Contractors are not allowed in these areas unless their specific task is inside these zones. If your work is inside these zones you must work with a Milford employee and follow local procedure LP 9.16 Gas Safety 11. Fuel Gas House 12. Bulk Hydrogen Storage Tanks 13. Emergency Information Hospital Name: Milford Regional Medical Center Hospital Address 14 Prospect Street/Rte. 140 Directions: Right out of plant (National Street) left on Depot Street, left at stop sign, right at lights (Rte. 140 North), thru first set of lights Emergency Room Entrance is 100 yards on right. Police, Fire, Ambulance Dial 9-911 Control Room Operator - 429 19. Plant Evacuation There is a plant-wide audible alarm. It is a constant siren. Notice to evacuate will also be given over plant radios and passed along by operations personnel to all employees and contractors on site. The site tests the evacuation alarm on the first Monday of every month @ 1100 20. Main Evacuation Point Outside Main Gate by Sign 21. Secondary Evacuation Point: Gate to Railroad Tracks - Located behind the Warehouse 22. When Notice is Given to Evacuate: All work permits are cancelled. Stop work immediately and turn off all tools and equipment. Exit to the designated evacuation point and remain there for roll count. DO NOT evacuate in your vehicle, no vehicular traffic is allowed during an emergency. 23. Site Safety PLEASE notify Milford Power personnel immediately if you Have found a hazardous condition. Have seen an unsafe act or condition. Have suffered any injury or illness. Have found an oil/chemical leak, spill or release. 24. Hazardous Materials All contractors must provide Milford Power with an up-to-date MSDS for any Hazardous Materials brought on-site. All hazardous material must be removed from Milford Powers property by the contractor at the completion of the work requiring the material. 25. Worksite Cleanup Contractors are responsible for disposal of their own waste. Contractors will clean their job site and stow all tools at the completion of the days work. 26. Waste Disposal A dumpster and a recycle bin are located behind the warehouse, for general trash & cardboard 27. Waste Disposal A waste oil tank & unibin are located on the east side of the turbine building for waste oil and oily rags. Do not put anything in these containers. If you have waste oil or oily rags see a Milford employee and they will dispose of this waste. 28. Safety Showers (6) Safety Showers and Eyewash Stations are located in the following areas: De-mineralizer Building Equipment Room Next to Empty Bulk Tank Ammonia Bulk Tank Bulk Boiler Chemical Tank Farm Chlorine Bulk Tank MCC Battery Room Note: An alarm will sound in the control room whenever a safety shower is activated with exception of the battery room 29. Lock-Out-Tag-Out (LOTO) All Equipment to be Worked on will be on a Safety Clearance. Milford Personnel will walk down all Isolation Points with the job Contractor before Work Begins Supervisors will sign on to the LOTO and be given a key to the job Lock Box. All Keys Must be turned in at the conclusion of the days work 30. Contractor Working Party Record Signed by all workers at the start of each shift. Supervisor maintains original. Copy left in Control room for evacuation roll call. All workers must sign out at end of shift or when leaving the plant. Supervisor returns completed original to Control Room at end of shift. 31. Hot Work Permit Valid for 1 Shift only, must be re-issued with each crew change. Must be issued for all cutting, welding, grinding or heating operations. Authorized Employee doing the work and his/her Fire Watch Must sign the Permit 32. Safe Work Review Will be used for all contracted work done which is not covered under a Safety Clearance (LOTO). Some examples are: Building Maintenance, Painting, Landscaping etc; 33. Confined Space Entry Permit All workers entering a confined space or acting as hole watch for a Confined Space entry Must have a current Confined Space Training Certificate on file with Milford Power 34. Fall Protection Full body harness with lanyards are required when working on scaffolds Required in all work at or above 6 feet without proper platform with handrails 35. Fall Protection/Ladder Safety Use only Industrial Fiberglass stepladders Never stand on the top two rungs of a stepladder or top four rungs of an extension ladder. Should be inspected before each use 36. Fork Truck Operations Only persons holding a valid Massachusetts state hoisting license may operate a fork truck on Milford Powers Facility. A copy of this license must be on file with Milford Power before use. 37. Overhead Crane Operations Only persons holding a valid Massachusetts state hoisting license may operate the overhead crane on Milford Powers Facility. A copy of this license must be on file with Milford Power before use. 38. Electric Carts Electric work carts are used on this facility. Our operators are careful drivers, but the carts are very quiet in operation. Please be aware of where you are walking, especially when walking around corners or exiting a building. 39. Compressed Gas Cylinder Safety Cylinders should be stored in a well-ventilated area at least 20 feet from combustible materials. Oxygen cylinders must be separated by 20 feet from fuel-gas cylinders, such as acetylene. Cylinders should be stored upright and secured with a chain or cable. Caps should be on, and valves completely shut when not in use and/or moving cylinders. 40. Overhead Power Lines No vehicle traffic is allowed under overhead power lines without the permission of the Control Room Operator. Min approach = 16ft.8in. 41. This is what happens when a crane or large vehicle hits or gets too close to overhead power lines 42. Contractor personal vehicle parking is at the end of National Street in the area outside the fence at the end of the cooling tower. Supervisor and company vehicles will be allowed on site by permit only. Contractor Parking Areas 43. Access To The Plant Will Be Through The (West) Main Gate Only During Outages, a Security Guard will be stationed at the Main Gate; He/She will maintain a roster of all contractors on site. If the Security Guard is busy, please wait to ensure you have been marked on/off site. 44. Training All Contractors must provide current training documentation for: Respirators Confined Space Entry Welding Certification Electrical Certification Fire Watch /Fire Extinguisher Other Applicable Standards 45. ZERO FATAL ACCIDENT TARGET 45 46. ZERO FATAL ACCIDENT TARGET 46 Do not walk or stand under a load. 47. ZERO FATAL ACCIDENT TARGET Stay out of the path of moving vehicles, plant and equipment. 47 48. ZERO FATAL ACCIDENT TARGET Clip on your harness when working at height. 48 49. ZERO FATAL ACCIDENT TARGET Only enter a trench if the appropriate wall supports are in place. 49 50. ZERO FATAL ACCIDENT TARGET The atmosphere must be tested safe before entering a confined space and monitored as you work. 50 51. ZERO FATAL ACCIDENT TARGET Do not perform hot work unless the fire or explosion risks have been eliminated. 51 52. ZERO FATAL ACCIDENT TARGET Verify that there is no live energy (mechanical, chemical, electrical, fluids under pressure, etc.)before starting work. 52 53. ZERO FATAL ACCIDENT TARGET Do not handle your phone or any other communication device when driving. 53 54. ZERO FATAL ACCIDENT TARGET Do not drive under the influence of alcohol or drugs. 54 55.
