Foreword Principal Director of Oral Health Ministry of Health Malaysia

It is my pleasure to pen a few words in this third publication of the ‘Milestones in Dentistry’. The first booklet was published in 2005, the second in 2013 and this year, extra efforts are undertaken to keep it updated. This publication is institutional memory. It serves as a single reference document on the evolution of the public sector oral health services in the Ministry of Health Malaysia from the start of organized dentistry in this country to the system it is today. Behind the lists lie untold the stories of perseverance and dedication by all who came this way before us - all for the sake of accessible quality oral healthcare for our people. This living document will continue to be periodically updated. It represents our appreciation for our predecessors’ efforts and continues to chronicle our own and those who come after us. 1899 First documented dental practice by aqualifieddentalpractitionerinPenang.

1929 First government dental clinic set up underMinistryofHealth(MOH)atJalanSilang(nowknown as Jalan Tun Tan Siew Sin KualaLumpur.

1938 ProfessorEdgarKTratmanwaselectedasthefirstPresidentofMalayanDentalAssociation.Establishment of this association laid thefoundation for organised dentistry anddevelopmentof thedental profession in thecountry.

1948 Establishmentoftheschooldentalservice.

By 1948 there were 20 government dentalofficers, 50 private dental practitioners andabout400registereddentistsinpractice.

IntroductionoftheDentalNurseSchemeforschoolchildren.

1950 Oral Surgery was established as a dentalspecialtydisciplineintheMinistryofHealth.

1953 Two dental technicians graduated from theDentalTrainingSchool,Penang.

1954 Orthodontics was established as a dentalspecialtydisciplineintheMinistryofHealth.

1959 Dentalmechanicswerere-designated‘dentaltechnicians’.

1961 Dental Public Health was established as adental specialty area in the Ministry ofHealth.

1964 TheMalayanDental Associationbecame theMalaysianDentalAssociation.

1965 TheMajlis JemaahMenteri approveda fixedmonthlyallowanceforholdersofDiplomasinPublic Health and Medical Officersundertaking public health duties. 1969 TheHonourableMinisterofHealthappointedthe Committee on Fluoridation of PublicWater Supplies in June 1969 ‘to study andreport upon the feasibility of introducingfluoridation of public water supplies as apublic health measure in all states of WestMalaysia’.

Early1970s

Establishment of the Oral HealthcareProgramme for Antenatal Mothers in theMinistryofHealth.

The Deputy Director of Medical Services(Dental) was re-designated as Director ofDental Services. State Principal DentalOfficers (PDOs) became State DentalDirectors(PengarahPergigianNegeri).

1971 The Dental Act (Act 51) was passed byparliament and gazetted on 30 September1971.ThisActgave long-awaitedrecognitionto the dental profession. The MalaysianDental Council (MDC)was establishedunderActtofunctionastheregulatorybodyforthedentalprofession.Under the Dental Act 1971, every dentalpractitionermustberegisteredwiththeMDC

and is required tohaveanAnnualPractisingCertificate (APC) to legally practise dentistryinMalaysia.TheMDCcontinues tobeunderthe administration of the Oral HealthDivision,MinistryofHealth.

1971 Conduct of the first Dental EpidemiologicalSurvey of School Children in West Malaysia(August1970-May1971).

1971 The ‘Report of the Committee Appointed toInquireintoandReportUpontheFluoridationof Public Water Supplies - West Malaysia1971’waspublished.The Committee strongly recommended ‘thatfluoridation of public water supplies beinstituted in West Malaysia as soon aspossible and further recommends that anoptimum level of 0.7 ppm fluoride bemaintainedinthereticulationsystem’.

1972 TheDivision IIDentistRegisterwasclosed in1972 following the enactment of the DentalAct1971.

The Cabinet Committee approvedfluoridationof publicwater supplies inWestMalaysia).

1973 Introduction of outreach dental servicesusing road and river transport, popularlyknownas‘flyingsquads’.

1974 Conduct of the first Dental EpidemiologicalSurveyofAdultsinPeninsularMalaysia(Sept1974-April1975).

1975 The scope of duties for dental nurses (DN)was expanded. DN were trained to providetooth-colouredrestorationsonanteriorteethwhere previously they were limited todeciduous teeth fillings and extractions,dentalhealtheducation(DHE)andprescribedpreventive services for schoolchildren aged12andbelow.Theywerenowalsoallowedtotreatchildrenuptobelow18yearsofage.

1976 Thefirstbatchof30studentsgraduatedwiththeBachelorinDentalSurgeryfromUM.

1978 The Dental Division, Ministry of HealthMalaysia published ‘A Report on theFluoridation Projects in the State of Johore,Malaysia1964–1976’.Thereportdocumenteda60.1%reductionindental caries in permanent dentition and

29.4% caries reduction in deciduousdentition, with the greatest benefit seenamong7-year-olds.Alsoreportedwasafive-fold increase of caries-free 7-year-olds fromthe1964baselinedata.

1979 Establishment of the National Dental HealthEducation Unit, Ministry of Health (laterrenamed the Oral Health Promotion Unit in1994).

1981 Conduct of the first Dental EpidemiologicalSurveyofSchoolChildreninSarawak1981.

1982 Start of the two-year Certificate course forDentalSurgeryAssistants(DSA)attheDentalTrainingSchool,Penang.

1983 Implementation of the Health ManagementInformation System (Dental Sub-system) intheMinistryofHealth.

1984 Introduction of the Oral Health Programmefor Preschool Children and the Pre-schoolMobileDentalTeams.Periodontology was established as a dentalspecialtydisciplineintheMinistryofHealth.

1985 Initiation of the Incremental Dental CareProgrammeforschoolchildren.

1992 Establishment of the first Paediatric DentalUnitatthePaediatricInstitute,KualaLumpurHospital.TheDirector-GeneralofHealth launched theDental Quality Assurance Programme (QAP)on4November1992.(The Dental QAP began with nine indicatorsin 1994 under the National IndicatorApproach (NIA). 1994 The Ministry of Health gave input into thereview of the UM dental degree curriculum.The programme was extended from 4 yearsto5years.

TheMinistryofHealthsentdentalofficerstothe first local postgraduate programme ofMasterinCommunityDentistryinUM.

ConductofthesecondDentalEpidemiologicalSurveyofSchoolChildreninSarawak1994.

The National Dental Health Education UnitwasrenamedtheOralHealthPromotionUnit.

Initiation of the electronic dental recordsystem for primary oral healthcare in theMinistryofHealth.

The training programmes for dental nursesand dental technicians were upgraded fromCertificatetoDiplomalevel.

Elaun Khidmat Luar Waktu Bekerja Biasa(Out-of-officehourallowances)wasextendedto all government medical and dentalofficers.

Recruitment of retired dental officers oncontract intotheMinistrytooffsetvacanciesindentalofficerposts.

1997 Launch of the national Primary Preventionand Early Detection of Oral Pre-cancer andCancer Programme at Tanah Merah Estate,NegeriSembilan.

1997 Conduct of the first National Oral HealthSurveyforSchoolChildren1997(NOHSS‘97).

The Doctor of Dental Surgery (DDS)programme began at Universiti KebangsaanMalaysia (UKM) with an intake of 24students.

1998

Formal initiation and conduct of post-basiccourses for dental nurses, with a 6-monthcourseinPaediatricDentistry.(Dental nurses had undergone post-basictrainingin1993buttheseweread-hoc,of3-month duration and were not officiallyendorsed as post-basic training by theMinistryofHealth).

1998 The Dental Services Division, Ministry ofHealth, produced its first Compendium ofAbstract,showcasingresearchprojectsby itsoralhealthpersonnel.

The Dental Services Division recommendedmandatory labelling of fluoridatedtoothpaste. The revised labelling wasapproved at the 90th Meeting of the DrugControl Authority, Ministry of Health on 26February1998.

Facilitiesand ServicesAct1998 - ‘AnActtoprovide for the regulation and control ofprivate healthcare facilities and services andother health-related facilities and servicesandformattersrelatedhereto’.UnderthisAct,allprivatedentalclinicsmustattain minimum standards of facilities,equipmentandinfectioncontrolprocessestoberegisteredwiththeMinistryofHealth.

1999 The Dental Services Division was renamedOral Health Division with approval of theMesyuarat Ketua Pengarah Kesihatan Khason 5May 1999 (Special CommitteeMeetingoftheDirector-GeneralofHealth).

Start of the School-Based Fissure SealantProgramme following on recommendationsfromNOHSS’97.

Conduct of the post-basic training inOrthodonticsfordentalnurses.

Dental nurses were trained in AtraumaticRestorativeTreatment(ART)Technique.

Service scheme of dental technicians wasupgraded with a name-change to ‘DentalTechnologists’on1January1999.

1999 ConductoftheFluorideEnamelOpacitiesin16-year-oldSchoolchildren1999utilisingDean’sIndexofFluorosis.

The dental degree programme UniversitiSainsMalaysia(USM)beganwithanintakeof40students.

2000 ApprovalgiventothefirsttwoOralSurgeonsfrom the Ministry of Health to begin theirmedicaldegreeprogramme(MBBS)inUM.

ConductoftheNationalOralHealthSurveyofAdultsYear2000(NOHSA2000).

FormulationoftheNationalOralHealthPlan2010 (NOHP 2010) with identified goals infour areas of dental caries, periodontaldisease,oralcancerandtrauma.

Conduct of the post-basic training in OralMaxillofacialSurgeryfordentaltechnologists.

Dentalnursesweretrainedtousehigh-speedcutting units to increase productivity and toenhancepatients’comfort.

The Ministry of Health was the majorsubscriber for thepostgraduateprogrammesofMasterinClinicalDentistryinOralSurgery,Periodontology, Paediatric Dentistry,

2000 TheMinistryofHealth sent4dentalofficersfor the first four-yearMaster in CommunityMedicine(OralHealth)USMforthespecialtydiscipline of Dental Public Health/PublicHealthDentistry.

The electronic dental record system in theMinistry of Health was expanded to includespecialistoralhealthcarebytheendof2000.

2001 Implementation of the three-year nationalservice for new registrants on and from 29June2001withtheAmendmenttoPartVIIofthe Dental Act 1971 on ‘SupplementaryProvisions for National Purposes’. This wasinstituted to overcome long-standing dentalofficer shortfalls in the Ministry of Health.‘Guidelines for Compulsory Service’ wereformulated.

2001 The ‘History of Dentistry in Malaysia’Exhibition was launched at the NationalMuseumfrom30August-15October2001inconjunctionwith theWorldDental Congress2001 (Federation Dentaire Internationale2001) held in Kuala Lumpur, Malaysia from27September-1October2001.

The Oral Health Division, Ministry of Healthproduced its firstcoffeetablebook ‘Throughthe Dental Mirror - History of Dentistry inMalaysia’incelebrationofFDI2001.

LaunchoftheMSISO9000qualitysystemforthe Oral Health Programme, Ministry ofHealth on 11 September 2001 by theSecretary-GeneraloftheMinistry.Melaka Tengah was the first district toachieveISOcertificationin2001,followedbythedistrictofKualaTerengganu.

2002 Conduct of post-basic training inPeriodontologyfordentalnurses.Formulation of ‘Guidelines for AccreditationofDentalDegreeProgramme for InstitutionsofHigherLearning’.Dentalnurseswere trained touseultrasonicscalers.

Approval of ‘critical allowance’ for dentalofficers at 5% of basic emolument inNovember2002.A Cabinet Memorandum decision on 9January 2002 approved the recruitment offoreign dental officers on contract in theMinistryofHealth.

2002 TheOralHealthDivision,MOHwasawardedtheMS ISO9002:1994 certification from theMalaysianAdministrativeModernisation andManagementPlanningUnit(MAMPU).

2002 Thefirsteditionofthe‘PositionStatementontheUseofDentalAmalgam’wasacceptedbythe Malaysian Dental Council (MDC) on 6June2002.ThedocumentclarifiedthestandoftheMDCon the continued use of amalgam in thecountry.

Conductofthestudy‘CostAnalysisofPrivatePrimary Care Services in Malaysia’ incollaboration with the Medical PracticeDivision, MOH, UKM and the Primary CareDoctorsofMalaysia(PCDOM).(ThefindingswereusedtodiscusstheDentalFee Schedule under the Private Healthcare

Arrival of the first group of dentalpractitioners from Indonesia to serve theMinistryofHealthonacontractbasisofnotmorethan10years(renewableevery3years)pursuant to the Cabinet MemorandumapprovalofJanuary2002.

Establishment of the Malaysian Oral CancerTissue and Data Bank in collaboration withtheDentalFaculty,UM.

2004 Restructuring of the Oral Health Division,MOHon1February2004.

Establishment of the Oral Cancer ResearchandCo-ordinationCentre (OCRCC),Universityof Malaya on the 23 August 2005 - tocollaborateand coordinate activities allaspects of oral cancer researchin Malaysia.Partners includedtheMinistryofHealth,UM,USM and the Cancer Research InitiativeFoundation(CARIF).

Start of the dental degree programme atAIMST University, the first private DentalFaculty,withanintakeof48students.

The Oral Health Division, Ministry of Healthparticipated with an exhibition on outreachdental services and water fluoridation inMalaysia at the 27th Asia Pacific DentalCongress(27thAPDC),25-29May2005,KualaLumpur.

The Oral Health Programme was accordedProgramme status with the Director of OralHealth being directly responsible to theDirector-GeneralofHealth(fromitspreviousstatusasan‘Activity’underthePublicHealthDepartment).

Gazettement of two Regulations on 1 April2006 under the Private Healthcare FacilitiesandServicesAct1998(Act586)namely- The Private Healthcare Facilities and

Services (Private Medical Clinics orPrivate Dental Clinics) Regulations 2006and

- The Private Healthcare Facilities andServices (Private Hospitals and OtherPrivate Healthcare Facilities) Regulations2006.

Start of the dental degree programme atUniversiti Teknologi Mara (UiTM) with anintakeof30students.

StartofthedentaldegreeprogrammeatthePenangInternationalDentalCollege(PIDC),aprivate institution in collaboration with theVinayake Mission Research Foundation(VMRF) Deemed University in India, with anintakeof44students.

2007 Two officers were sent on scholarships forpostgraduate training at the University ofOtagoandtheUniversityofMelbourne.Post-basic Allowances were approved fordental technologists and dental nurses withpost-basic qualifications in the areas of OralSurgery, Orthodontics and Periodontics inMOH.

The Symposium on Dental ManpowerDevelopment in Malaysia 2007 was held,involving the dental fraternity from publicandprivatesectors.

Integration of Continuing ProfessionalDevelopment (CPD) and Competency LevelAssessment (Penilaian Tahap Kecekapan[PTK]) was approved by the Public ServicesDepartment.Dental officers formed one of the threepioneer groups (with medical officers andpharmacists).

The‘criticalallowance’fordentalofficerswasrevised to be on par with that of medicalofficers.

With the graduation of two Oral Surgeonswithmedicaldegrees -MBBS (Malaya) – theconcept paper for doubly-qualified OralMaxillofacial Surgeons (OMFS) in the MOHwas approved by the Director-General ofHealth.

2007 Dental officers were sent for postgraduatetraining in Oral Surgery and Orthodontics atHong Kong University and the OralMedicine/Oral Pathology programme at theUniversityofWesternAustralia.

StartofthedentaldegreeprogrammeattheInternational Islamic University Malaysia(IIUM)withanintakeof50students.

Start of the dental degree programme atUniversitiSainsIslamMalaysia(USIM)withanintakeof29students.
