Crystal Barter is the Director of Performance Improvement at the Michigan Center for Rural Health. She has been with the MCRH for over eight years and her current work focuses on programs that facilitate operational, financial and clinical quality improvement in Michigan’s Critical Access Hospitals, Rural PPS Hospitals, and Certified Rural Health Clinics. This includes managing the Medicare Rural Hospital Flexibility Grant Program, components of the State Office of Rural Health Grant, the Small Hospital Improvement Grant Program, and various sub-contracts focusing on quality improvement. Crystal serves on the National Organization of State Offices of Rural Health Board of Directors, and is a member National Quality Forum’s Measures Application Partnership (MAP) Hospital and Rural Health Workgroup. She holds a BS in Health Professions with a minor in Biology from Grand Valley State University and is finishing her Masters of Health Care Administration degree from Central Michigan University.

Ms. Flores has been a licensed Social Worker for over twenty years.Ms. Flores was appointed to the Ohio Attorney General’s Human Trafficking Commission in 2009 and has testified before the Ohio House and Senate in supports of Human Trafficking Legislation. Her efforts were a major part of the success of these bills being passed into laws. Additionally, the “Theresa Flores Law” was passed in Michigan that eliminates the statue of limitation for children who have been trafficked. Flores is a survivor of domestic child sex trafficking and was sold in an underground crime ring in an upper middle class suburb outside Detroit from the time she was 15-17 years old. She now works with other survivors and is the Assistant Investigator with Global Centurion researching the mental and physical health problems of over 200 domestic trafficking survivors. The study entitled, “The Health Consequences of Human Trafficking” will provide best practices and systemic recommendations for health care providers. She has received many awards including the University of Dayton’s Alumni Association 2013 Christian Service Award for her work with S.O.A.P. In January 2012 at the State of the State Address, Ms. Flores received The Courage Award from Ohio Governor Kasich for her work in human trafficking. Ms. Flores has published four books, “The Sacred Bath” and “The Slave Across the Street” (in the UK and U.S.) and “Slavery in the Land of the Free- A Student’s Guide to Modern Day Slavery”. The audio version of her memoir was nominated for The 2011 Audie Award, being in the top 5 of all memoirs and biographies and also has been on the Wall Street Journal and USA Today Best Seller list for e-books several times. Ms. Flores also has a very popular TED talk.

Amy Hoge has spent the last 25 years helping businesses and organizations with strategic planning and project management and currently serves as Director of Physician Services at the Michigan Health Council. Previously, she led communication strategy for Munson Healthcare working to preserve local access to care for residents across northern Michigan.

Having grown up in rural communities in Michigan and overseas, Amy understands firsthand the difference local access to care can make for individuals, families and children. With the goal of retaining and recruiting physicians to work in underserved areas of Michigan, she’s spent the last year brainstorming with various stakeholders throughout the state.

One idea that came out of those discussions is the Michigan Loan Reimbursement & Employment Solution (MiLES) -- a project that provides $50,000 in loan repayment to physician residents who agree to work for 2 years in an underserved rural or urban area of our state. The program is administered through the Michigan Health Council in partnership with the Michigan Department of Health and Human Services.

Brock is the 2015 recipient of the Calico Quality Leadership Award of the National Rural Health Resource Center and received the American Society of Healthcare Pharmacists (ASHP) Board of Directors’ Award of Honor for 2018. Patrick Sustrich, MS, is the chief integration officer at Hayes Green Beach Memorial Hospital, as well as the executive director of AL!VE, HGB’s experience-based destination health park. HGB is a 25-bed Critical Access Hospital located in Charlotte, Mich., that has been the community’s partner in health care for 85 years. Sustrich was instrumental in opening AL!VE in November 2011 to help address the need for hospital’s to play a critical role in influencing population health. The $15.5-million facility is 70,000 square feet and includes rehabilitation services, a healthy-eating café and demonstration-teaching kitchen, a fitness center, children’s recreational area, business conference space and more. AL!VE has been recognized regionally and nationally for the positive impact it has had in the community. Sustrich has worked at HGB since 2009. He has extensive knowledge in business and hospital administration, facility operations, population health and clinically integrated networks. He has played a key role in connecting HGB to regional population health efforts that involve robust care coordination programming and a focus on the quadruple aim: excellent care, excellent health, excellent experience and excellent value. He has acted as a consultant for numerous hospitals, country clubs and non-profit businesses interested in implementing health and fitness facilities and programs, especially in regard to population health efforts. Sustrich has been published in numerous research journals, and presented at national and international symposiums. He is also internationally recognized as a facilitator for Dr. Edward DeBono’s “Six Thinking Hats” and has been honored with the prestigious Alvin Whitfield Community Service Award. Sustrich previously opened and managed two other fitness and rehabilitation facilities – the Kelsey Community Wellness Center in Lakeview, Mich., and the University Club of Michigan State University in Lansing, Mich. Sustrich received his undergraduate and graduate degrees in exercise physiology from Northern Michigan University. While in Marquette, Mich., Sustrich obtained experience from his work with Marquette General Hospital, and testing and training athletes from the United States Winter Olympic Team. He lives in DeWitt, Mich., with his wife, Lara, and two daughters, Cassandra and Alexandra.
